Transcutaneous PCO2 Measurements With Long-Term Noninvasive Ventilation.

Background: Current guidelines recommend that monitoring of long-term noninvasive ventilation (Long-term NIV) should include measurement of nocturnal transcutaneous PCO2 (PtcCO2). This has previously required an elective hospital admission. However, owing to a growing patient population and advancements in monitoring technology, there is a general trend toward moving specialized health care from the hospital to the home. The aim of this study was to compare the success rate and causes of failure of PtcCO2 monitoring at home versus at the hospital and to discuss the criteria used for the evalua

Noninvasive Ventilation in the Cardiac Intensive Care Unit.

Over the last several decades, the cardiac intensive care unit (CICU) has seen an increase in the complexity of the patient population and etiologies requiring CICU admission. Currently, respiratory failure is the most common reason for admission to the contemporary CICU. As a result, noninvasive ventilation (NIV), including noninvasive positive-pressure ventilation and high-flow nasal cannula, has been increasingly utilized in the management of patients admitted to the CICU. In this review, we detail the different NIV modalities and summarize the evidence supporting their use in conditions fr

Noninvasive and invasive mechanical ventilation for neurologic disorders.

Patients with acute neurologic injuries frequently require mechanical ventilation due to diminished airway protective reflexes, cardiopulmonary failure secondary to neurologic insults, or to facilitate gas exchange to precise targets. Mechanical ventilation enables tight control of oxygenation and carbon dioxide levels, enabling clinicians to modulate cerebral hemodynamics and intracranial pressure with the goal of minimizing secondary brain injury. In patients with acute spinal cord injuries, neuromuscular conditions, or diseases of the peripheral nerve, mechanical ventilation enables respira

Helmet noninvasive ventilation in acute hypoxic respiratory failure.

PURPOSE OF REVIEW: Invasive mechanical ventilation is a lifesaving intervention for patients with severe acute hypoxic respiratory failure (AHRF), but it is associated with neuromuscular, cognitive, and infectious complications. Noninvasive ventilation (NIV) may provide sufficient respiratory support without these complications. The helmet interface for NIV could address concerns raised for the use of NIV as first-line therapy in AHRF. This review will summarize and appraise the current evidence for helmet NIV in AHRF. RECENT FINDINGS: There are only six randomized controlled trials comparing

Obesity and perioperative noninvasive ventilation in bariatric surgery.

The incidence and prevalence of obesity continues to increase globally. Physicians will therefore provide care for an increasing number of obese patients in their clinical practice. Optimal management of these patients is required to minimize the risk of perioperative complications that increase morbidity and mortality. Obesity affects the respiratory function. It is generally associated with reduced lung volume with increased atelectasis, decreased lung and chest wall compliance, increased airway resistance, and moderate to severe hypoxemia. These physiologic alterations are generally more pr

Results of noninvasive ventilation in very old patients.

BACKGROUND: Noninvasive ventilation (NIV) is frequently used for the management of acute respiratory failure (ARF) in very old patients (≥ 80 years), often in the context of a do-not-intubate order (DNI). We aimed to determine its efficacy and long-term outcome. METHODS: Prospective cohort of all patients admitted to the medical ICU of a tertiary hospital during a 2-year period and managed using NIV. Characteristics of patients, context of NIV, and treatment intensity were compared for very old and younger patients. Six-month survival and functional status were assessed in very old patients.

Noninvasive positive-pressure ventilation in patients with malignancy.

Noninvasive positive-pressure ventilation (NIPPV) is now accepted as the treatment of choice for subgroups of patients with acute respiratory failure. Noninvasive positive-pressure ventilation has traditionally not been considered in the management strategy of patients with malignancy; however, this mode of ventilatory support may be appropriate in some specific situations. Noninvasive positive-pressure ventilation is the preferred initial mode of ventilatory support in patients with hematologic malignancies or after bone marrow transplantation who develop acute respiratory failure. In these p

Noninvasive ventilation in Plasmodium vivax related ALI/ARDS.

Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS), complications of P. falciparum malaria are associated with a very high mortality but are rarely seen in P. vivax malaria. Herein, we report a 28-year old woman who developed ARDS after infection with P. vivax. In a systematic review the literature for the use of noninvasive ventilation (NIV) in cases of ALI/ARDS related to P. vivax, from among 45 reports noninvasive positive pressure ventilation (NIPPV) was used in the management of three cases of ALI/ARDS related to P. vivax. The use of NIV in vivax malaria related ALI/AR

Noninvasive ventilation for chest wall and neuromuscular disorders.

Neuromuscular and chest wall disorders are individually uncommon but together form an important group of conditions that can lead to chronic ventilatory failure. This is best recognised in scoliosis, kyphosis, following a thoracoplasty, in muscular dystrophies, such as Duchenne muscular dystrophy (DMD), and myotonic dystrophy, after poliomyelitis and with motor neurone disease (amyotrophic lateral sclerosis). If bulbar function is impaired, tracheostomy ventilation may be required, but in other situations, noninvasive ventilation is preferable. Positive pressure techniques using nasal and face

[Noninvasive ventilation for decompensation of chronic respiratory failure].

UNLABELLED: MASK DELIVERY: Noninvasive ventilation designates ventilation methods which do not use the endotracheal route. Noninvasive ventilation is generally delivered via a facial mask (generally for patients with acute respiratory failure) or a nasal mask (widely used for home-delivery systems). INDICATIONS: Following work conducted by Rideau in the eighties who made the first ventilation trials in patients with myopathy, Meduri then Brochard focused interest on noninvasive ventilation for the management of acute episodes in patients with chronic respiratory failure. Following a simple al

Patient-Ventilator Interaction With Noninvasive Proportional Assist Ventilation in Subjects With COPD.

BACKGROUND: To investigate patient-ventilator interaction during different levels of noninvasive proportional assist ventilation (PAV) compared with noninvasive pressure support ventilation (PSV). METHODS: Fifteen subjects with severe COPD and hypercapnia were consecutively recruited. After the baseline assessment of unassisted spontaneous breathing, 3 levels of ventilatory support were applied. The proportional assist (PA) and pressure support (PS) levels were set by subject comfort. PA-, PS- or PA+, PS+ were set at 25% more or less of PA or PS (PA- = 75% PA, PA+ = 125% PA, PS- = 75% PS, PS+

Noninvasive ventilation for acute respiratory distress syndrome: the importance of ventilator settings.

Noninvasive ventilation (NIV) is commonly used to prevent endotracheal intubation in patients with acute respiratory distress syndrome (ARDS). Patients with hypoxemic acute respiratory failure who fail an NIV trial carry a worse prognosis as compared to those who succeed. Additional factors are also knowingly associated with worse outcomes: higher values of ICU severity score, presence of severe sepsis, and lower ratio of arterial oxygen tension to fraction of inspired oxygen. However, it is still unclear whether NIV failure is responsible for the worse prognosis or if it is merely a marker of

Patient-ventilator asynchrony during noninvasive ventilation: the role of expiratory trigger.

OBJECTIVE: Air leaks around the mask are very likely to occur during noninvasive ventilation, in particular when prolonged ventilatory treatment is required. It has been suggested that leaks from the mask may impair the expiratory trigger cycling mechanism when inspiratory pressure support ventilation (PSV) is used. The aim of this study was to compare the short-term effect of two different expiratory cycling mechanisms (time-cycled vs flow-cycled) during noninvasive inspiratory pressure support ventilation (NIPSV) on patient-ventilator synchronisation in severe hypoxemic respiratory failure.

Prediction of noninvasive ventilation failure: a narrative review.

With the increasing clinical application of noninvasive ventilation (NIV) and the accumulation of evidence-based medical data, NIV has become an important therapeutic intervention for various types of respiratory failure. However, a subset of patients still experience NIV failure. Compared with those with successful NIV, patients who develop NIV failure tend to have a poorer prognosis and an elevated risk of mortality. Therefore, the accurate identification and early prediction of NIV failure risk are crucial for guiding clinical decision-making and optimizing treatment strategies. This articl
