Malignant pheochromocytoma: pain, palpitation, perspiration and perplexities.

A 60-year-old man presented with headache, giddiness, abdominal pain and palpitation. When evaluated outside for the same, the patient was diagnosed to have hypertension and started on antihypertensives for which he did not respond. ECG was suggestive of non-ST elevation myocardial infarction. The patient was subjected to a coronary angiogram, which was normal. Patient had multiple episodes of fluctuating blood pressures. CT of the abdomen showed a 7.1×5.6×8.2 cm mass in the left adrenal gland suggestive of a pheochromocytoma. Serum, urine metanephrines and normetanephrines were elevated. Afte

Bladder paraganglioma presenting with atypical abdominal pain: A diagnostic challenge.

A 70-year-old man presented with a 10-day history of intermittent abdominal pain. Initial evaluation revealed a bladder nodule on imaging, but cystoscopy showed no tumor. Subsequent investigations including elevated urinary normetanephrine levels and MRI identified a hypervascular bladder mass supplied by the right internal iliac artery. Laparoscopic partial cystectomy confirmed a paraganglioma with positive immunohistochemistry (synaptophysin+, chromogranin A+). Postoperatively, symptoms resolved, and antihypertensive therapy was discontinued. This case highlights the importance of considerin

Retroperitoneoscopic resection of a primary retroperitoneal paraganglioma occurring between the aorta and vena cava: A case report.

A 67-year-old man presented with lower limb weakness, palpitations, and insomnia. His urinary total metanephrine and normetanephrine levels were high. Computed tomography showed a 48 × 34-mm oval mass on the dorsal side of the pancreas and portal vein between the aorta and vena cava. Because of the tumor location, we performed laparoscopic surgery using a retroperitoneal approach. We described a case of a patient with primary retroperitoneal paraganglioma between the aorta and vena cava who underwent retroperitoneoscopic resection. Our case suggests that this procedure can be safely performed.

Biological and psychological aspects of a case of Gilles de la Tourette's syndrome.

The case of a 12-year-old girl with Gilles de la Tourette's syndrome is presented, and the following factors are discussed: psychological aspects, urinary excretion of 3-methoxy-4-hydroxyphenylethylene glycol (MHPG) and unconjugated amines, and CT scan findings. The most noticeable differences from normal controls seen in the urine studies were elevated levels of beta-phenylethylamine and the catecholamine metabolites normetanephrine, MHPG, and 3-methoxytyramine, and a decrease in 5-hydroxytryptamine. A CT scan revealed a slight increase in density in the caudate nuclei bilaterally; this was t

An adrenal cortical adenoma with neuroendocrine-type granules mimicking pheochromocytoma.

Clinical and biochemical presentation of adrenal tumors may rarely conflict with their histologic features. In the present report, we describe a rare case of adrenal neoplasm clinically and biochemically labeled as pheochromocytoma which at histologic examination resulted adrenal cortical tumor. The neoplasm was examined with the electron microscope which revealed the presence of electron-dense neuroendocrine-type granules next to intracytoplasmic lipid droplets. The patient underwent laparoscopic left adrenalectomy which leads to normalization of 24 h urinary metanephrine and normetanephrine.

Quantitative analysis of catecholamines and their metabolites in 491 patients with adrenal tumors: a retrospective single-center cohort study.

BACKGROUND: Adrenal tumors, including benign cortical adenoma (BCA) and pheochromocytoma (PCC) or paraganglioma (PGL), have been more frequently detected during imaging examinations in recent years. However, the associated clinical or laboratory characteristics, especially on the Chinese population, still need to be investigated. METHODS: We conducted a retrospective analysis of 491 patients pathologically diagnosed with adrenal tumors, from Jan 19, 2018 to Dec 17, 2019, at a tertiary referral hospital in Wuhan of China. Our findings including 247 (50.3%) BCA cases, and 92 (18.7) PCC/PGL case

Changes in Body Mass Index in Pheochromocytoma Patients Following Adrenalectomy.

Catecholamine excess from pheochromocytoma results in cardiovascular symptoms such as arterial hypertension and tachycardia and induces metabolic alterations including glucose intolerance and increase in resting metabolic rate. The objective of our study was to investigate the effect of surgical cure of pheochromocytoma on body-mass-index and the correlation of body-mass-index changes to preoperative endocrine parameters. Pheochromocytoma patients from the Munich ENSAT Registry were matched (1:2) for age and gender to patients from the German Conn's Registry, who had undergone surgery for aldo

Somatostatin Receptor 2 Negative Pheochromocytoma Masked by Normal Adrenal Gland on Gallium-68 DOTATATE.

BACKGROUND/OBJECTIVE: Gallium-68 DOTATATE (68Ga-DOTATATE) positron emission tomography/computed tomography (CT) is a somatostatin receptor (SSTR)-based imaging with high sensitivity that can be used for detection of pheochromocytomas and paragangliomas. We report a pheochromocytoma with negative SSTR2 expression and low uptake on 68Ga-DOTATATE, whose detection was masked by the uptake of normal adrenal tissue. CASE REPORT: A 50-year-old man presented with a right adrenal incidentaloma. He had mildly elevated plasma normetanephrine levels of 194 pg/mL (ref. 0-145 pg/mL). Adrenal CT scan showed

Risk factors for intraoperative complications in pheochromocytomas.

We aimed to identify presurgical and surgical risk factors for intraoperative complications in patients with pheochromocytomas. A retrospective study of patients with pheochromocytomas who underwent surgery in ten Spanish hospitals between 2011 and 2021 was performed. One hundred and sixty-two surgeries performed in 159 patients were included. The mean age was 51.6 ± 16.4 years old and 52.8% were women. Median tumour size was 40 mm (range 10-110). Laparoscopic adrenalectomy was performed in 148 patients and open adrenalectomy in 14 patients. Presurgical alpha- and beta-blockade was performed i

Unilateral mixed corticomedullary tumor of the adrenal gland with a synchronous contralateral pheochromocytoma: A case report and literature review.

A 37-year-old man had two years of fatigue, weakness, and mood changes, initially misdiagnosed as depression. Workup showed hypertension, elevated urinary normetanephrine, elevated free cortisol, and suppressed ACTH. Imaging revealed bilateral adrenal masses (7.5 cm left, 4.7 cm right) with left adrenal vein thrombosis; MIBG scintigraphy showed bilateral uptake. After alpha-blockade, he underwent bilateral adrenalectomy. Inhibin positivity in the left mass's cortical region suggested a possible first-reported unilateral mixed corticomedullary tumor with synchronous contralateral pheochromocyto

Mediastinal paraganglioma: time to panic?

Paragangliomas (extra-adrenal phaeochromocytomas) are tumours arising from extra-adrenal chromaffin tissue. The authors describe a case of a 54-year-old woman presenting with shortness of breath and chest pain. CT pulmonary angiogram demonstrated a mediastinal mass. Further history taking revealed spontaneous attacks of headaches and palpitations. Subsequent imaging and biochemical testing confirmed the presence of a rare posterior mediastinal paraganglioma. The patient was prepared for surgery. A left thoracolaparotomy was performed and the mass was excised in its entirety. Postoperatively, 2

Hypertensive emergency masquerading as phaeochromocytoma: a report of two cases.

We report two cases of patients who were hospitalised for hypertensive emergency. In both cases, laboratory testing during admission revealed elevated normetanephrines in the range of 4-5 times the upper limit of normal using liquid chromatography with mass spectrometry. Imaging studies did not localise any phaeochromocytoma or paraganglioma (PPGL). Repeat biochemical testing after discharge was within normal limits in both cases. These observations suggest that hypertensive emergency should be recognised as a potential cause of 'false-positive' laboratory findings in the diagnostic assessment

A Pilot Study on Plasma and Urine Neurotransmitter Levels in Children with Tic Disorders.

BACKGROUND: Tic disorders (TDs), including Tourette syndrome, are childhood-onset neuropsychiatric disorders characterized by motor and/or vocal tics that commonly affect children's physical and mental health. The pathogenesis of TDs may be related to abnormal neurotransmitters in the cortico-striatal-thalamo-cortical circuitry, especially dopaminergic, glutamatergic, and serotonergic neurotransmitters. The purpose of this study was to preliminarily investigate the differences in the three types of neurotransmitters in plasma and urine between children with TD and healthy children. METHODS: W

Catecholaminergic activity among young adults after high phosphorus intake during childhood and adolescence.

PURPOSE: Elevated catecholamine secretion has been reported after high dietary phosphate intake in experimental studies in animals and humans. This study thus aimed to examine the prospective relationship between long-term dietary phosphorus intake during childhood and adolescence-assessed via 24-h urinary phosphate excretion-and catecholamine levels in adulthood. METHODS: A total of 159 healthy participants of the DONALD Study (Dortmund, Germany) were examined, who had repeatedly provided 24-h urine samples between ages 3 and 17 years and from whom a 24-h specimen was collected again in youn

Inpatient Measurements of Urine Metanephrines are Indistinguishable from Pheochromocytoma: Retrospective Cohort Study.

BACKGROUND: Pheochromocytoma is a rare cause of acute cardiovascular disease; however, any severe illness may have high catecholamines, simulating pheochromocytoma. We determined the spectrum of urine metanephrines from inpatient and outpatient collections without pheochromocytoma, compared with confirmed pheochromocytoma patients. METHODS: Retrospective analysis using centralized laboratory data serving all outpatients and hospitals in southern Alberta. The analysis comprised 24-hour urine normetanephrine and metanephrine (UNM-UMN) results collected from hospital inpatients, community outpat

Association Between Urinary Catecholamine Excretion and Urine Volume.

Several confounders must be considered in the evaluation of urinary catecholamine excretion. However, literature is contradictory about potential confounders. The aim of the present study was to assess correlations between catecholamine excretion and anthropometric or clinical parameters with special attention to urine volume. A total of 967 24-h urinary catecholamine measurements were performed in 593 patients for diagnostic purposes. The indication for urine examination was suspicion of secondary hypertension, phaeochromocytoma, or paraganglioma. From the patients examined, 57% were females
