Coccidioidomycosis is a fungal disease with a wide variety of manifestations. The systemic infection is a product of airborne spore inhalation released from the soil. This once-endemic disease is steadily increasing in incidence, geographic location, and severity. The rare coccidioidomycosis cases requiring surgical intervention present unique challenges to anesthesia providers. This case report describes a 45-year-old woman with no relevant medical history admitted for lobec-occidi tomy with decortication because of aggressive coccidioidomycosis. Anesthetic considerations included attention t

The Department of Veterans Affairs (VA) is amending its medical regulations to permit full practice authority of three roles of VA advanced practice registered nurses (APRN) when they are acting within the scope of their VA employment. Certified Registered Nurse Anesthetists (CRNA) will not be included in VA's full practice authority under this final rule, but comment is requested on whether there are access issues or other unconsidered circumstances that might warrant their inclusion in a future rulemaking. The final rulemaking establishes the professional qualifications an individual must po…

With the ongoing debate on New Ways of Working in the National Health Service, the role of nurses and other perioperative practitioners in anaesthetics is becoming an increasingly prominent issue. The opening up of new anaesthetic roles to nurses and their non-medical colleagues raises questions as to their own acceptance of these new roles and their acceptance by their medical colleagues. In this article, the author addresses the question as to whether perioperative practitioners are ready for the nurse anaesthetist, and whether the UK as a whole is ready for this initiative.

BACKGROUND: Presurgical blood orders are important for patient safety during surgery, but excess orders can be costly to patients and the healthcare system. We aimed to assess clinician perceptions on the presurgical blood ordering process and perceived barriers to reliable decision-making.
METHODS: This descriptive qualitative study was conducted at a single large academic medical center. Semi-structured interviews were conducted with surgeons, anesthesiologists, nurse anesthetists, nurse practitioners working in preoperative assessment clinics, and transfusion medicine physicians to assess …

General anesthesia increases the morbid risk for both mother and infant during a cesarean delivery. Failed or asymmetric blocks can force providers to turn to a general anesthetic precipitously. A true one-sided spinal block, with complete absence of anesthesia on one side despite correct subarachnoid technique, is rare and not well characterized in the literature. In this case, a 32-year-old multiparous woman presented for elective repeat cesarean delivery. After an uncomplicated spinal anesthetic, she developed a complete left-sided sensory and motor block, with no block on the right. A supp

A 57-year-old man with myotonia congenita underwent elective laparoscopic appendectomy after perforated appendicitis treated with two drainages. Quadratus lumborum blocks were preoperatively placed bilaterally under ultrasound guidance. Anesthetic gases and succinylcholine were avoided, and total intravenous anesthesia was used for maintenance. Neuromuscular blockade was minimized and reversed with sugammadex. The operative course was uneventful. In the postanesthesia care unit, the patient reported a pain level of zero throughout his stay, required no opioids, and was discharged the same day

This case report details the onset of masseter muscle rigidity, elevated creatine kinase levels, and rhabdomyolysis following a sevoflurane mask induction and succinylcholine administration in a 12-year-old boy. The patient had no family or personal history of neuromuscular disease or malignant hyperthermia. Hyperkalemia, metabolic acidosis, and rhabdomyolysis occurred within 75 minutes of masseter muscle rigidity. Subsequent to this event, it was recommended that the patient undergo a workup for neuromuscular disease and malignant hyperthermia with muscle biopsy. Until this workup is complete

The following is a case report of a patient who had an intravenous (i.v.) catheter inserted into her cephalic vein and thereafter sustained an injury to the superficial branch of the radial nerve. When an i.v. catheter penetrates a nerve, it can cause temporary or permanent damage. After sustaining an injury, a nerve will regenerate in an attempt to reconnect with the fibers it once innervated. Recovery from nerve damage may take only weeks or a year or more. Some patients, however, may sustain lifelong damage depending on the severity of the needle stick to the nerve. To avoid injury to perip

Dentists in the state of Illinois who possess a permit to administer sedation or general anesthesia were surveyed. A 71% response rate was achieved. Of the respondents, 86% held permits for deep sedation/general anesthesia and 14% held permits for parenteral conscious sedation. By practice specialty, 84% were oral and maxillofacial surgeons, 11% were general dentists, 5% were periodontists, and fewer than 1% were dental anesthesiologists. Advanced Cardiac Life Support training was possessed by 85% of the respondents. The most common anesthesia team configuration (82%) was a single operator-ane

INTRODUCTION: The number of interhospital transports with intubated patients or where intubation readiness is required is increasing in Sweden and globally. Specialist nurses are often responsible for these transports, which involve numerous risks for critically ill patients.
AIM: The aim of this study was to describe nurse anaesthetists' and intensive care nurses' strategies for safe interhospital transports with intubated patients or where intubation readiness is required.
METHOD: A qualitative study was conducted using the critical incident technique. During March and April 2020, 12 semi-

The anesthetic management of patients undergoing tracheoesophageal fistula repair often involves lung separation, usually selective bronchial intubation with a double-lumen endotracheal tube. However, in patients with airway fistulas arising below the tracheal lumen, selective lung ventilation and separation may require unusual methods of airway management. In the patient described in this report, the airway fistulas involved the distal 3 cm of the trachea, approximately half of the left main bronchus 1.5 cm beyond the carina, and the proximal 0.5 cm of the right main bronchus. To separate and

Polyuria has been reported as a side effect of sevoflurane administration, but because of its relative rarity, many practitioners are not aware of this potential phenomenon. Polyuria in its extreme form can cause undesirable hemodynamic changes. A case study, in an 18-year-old man, is presented highlighting polyuria as a probable side effect of sevoflurane administration.

PURPOSE: To evaluate the efficacy of video-intuboscopic assisted tracheal intubation in a difficult intubation setting.
METHODS: In 50 pediatric patients (mean age 12.8 +/- 3.1 yr, range 6-16 yr) a grade 3 direct laryngoscopic view was simulated. Eight certified registered nurse anesthetists without experience in endoscopic intubation performed tracheal intubation on five or more patients using the video-optical intubation stylet. Time from insertion of the tube into the oral cavity until the tip had passed the vocal cords was recorded. Failed intubation was defined as intubation >60 sec, art…

A 31-old male nurse of anesthesiology was exposed to rubber gloves over a period of ten years. An atopic diathesis was known. From time to time he had episodes of pollinosis. Since nine months he had suffered from conjunctivitis, rhinitis and shortness of breath while wearing latex gloves. During a night shift ten minutes after consumption of tropical fruit (bananas, kiwi) he had to care for a patient in the emergency room. He wore rubber gloves and had to clean the emergency room. Initially, his palms and feet itched. Then he experienced angor and increased perspiration 90 seconds later he su

This case illustrates the anesthetic management of radiographically assisted embolization and surgical excision of a large arteriovenous malformation (AVM). Excision resulted in significant blood loss and cerebral edema. This prompted the use of cerebral protection and induced hypotension. The patient was kept in a barbiturate coma postoperatively. On emergence from the barbiturates, the patient demonstrated elevated intracranial pressure and resolving deficits. She was discharged 3 weeks after surgery for rehabilitation. Arteriovenous malformations can compromise the cerebral parenchyma by ma
