Reducing extravasation and infiltration in the pediatric population: a best practice implementation project.

INTRODUCTION: Peripheral intravenous (PIV) therapy, the most common invasive procedure in hospitals, frequently results in complications. Infiltrations occur when infused fluids enter the surrounding tissue without harming the skin, while extravasations irritate the skin and tissue, occasionally necessitating surgical intervention. Evidence-based practices recommend the use of quality standards, such as Assess, Compare, Touch (ACT) and a 60-minute rounding tool to address the rising incidence of intravenous complications. OBJECTIVES: The objective of this evidence implementation project was t

Advocacy in Action: Advancing State Policy Change to Strengthen School Nurse Salaries in New Mexico.

School nurses are vital advocates for student health, yet the New Mexico workforce has long faced staffing shortages and significant pay inequities. This article describes a successful policy advocacy campaign by the New Mexico School Nurses Association to achieve salary parity. By collecting and leveraging data, including statewide surveys and salary schedule audits, advocates demonstrated that school nurse compensation failed to match the state-mandated three-tiered pay scale guaranteed to teachers. This effort culminated in the passage of House Bill 195 in 2025, standardizing state-wide sch

Implementing alcohol screening and intervention in a family medicine residency clinic.

The purpose of this study was to evaluate the impact of simultaneous systems interventions and clinician training on management of hazardous and harmful drinking in a residency clinic. Systems interventions included forming a multi-disciplinary implementation team, training registration clerks to distribute health risk questionnaires using the AUDIT-C alcohol screen, and training nurses to score the AUDIT-C and administer the AUDIT to screen-positive patients. Clinicians were trained to perform brochure-based interventions on screen-positive patients. Research staff provided compliance feedbac

Preventing retention of surgical items in intra-abdominal surgeries: a best practice implementation project.

INTRODUCTION: Retention of surgical items (RSI) is a "never event" and has serious consequences for patients, providers, and the organization. OBJECTIVES: This project aimed to implement best practices for surgical counts to reduce the incidence of RSI in patients undergoing intra-abdominal surgery. METHODS: This project followed the seven-phase JBI Evidence Implementation Framework. The framework recommends a before-and-after audit and feedback approach. A baseline audit was conducted to determine current compliance with best practices. Strategies were then implemented to address gaps in co

Medication absorption for patients with an ileostomy.

The Association of Stoma Care Nurses UK (2013) has stressed that it is the role of the stoma care nurse to provide education to patients, carers, prescribers and other nurses. This includes the area of medication management. Ensuring patients with an ileostomy receive their medication in a form they can absorb is of importance to every stoma care nurse. Prescribing for patients with a stoma calls for special care. Each patient with an ileostomy requires assessment of their medication regimen for the purpose of identifying potential medical absorption issues. However, from an extensive literatu

The efficiency of fluid balance charting: an evidence-based management project.

This study began following an observation that fluid balance (FB) charts seemed to be overloading medical records in Hong Kong hospitals. The major issues for this study were the efficiency and appropriateness of FB charting. The study design included an analysis of 250 medical records and an opinion survey. Seventy-five doctors and 98 nurses participated in the survey. The results indicated that 50% of all medical records had FB charts and in these cases they made up between 12 and 16% of the whole medical record. Intravenous infusion and urinary catheterization accounted for almost 70% of th

How to improve the quality of care for women with postpartum haemorrhage at Onandjokwe Hospital, Namibia: quality improvement study.

BACKGROUND: Postpartum haemorrhage (PPH) is the leading direct cause of maternal morbidity and mortality worldwide. The sustainable development goals aim to reduce the maternal mortality ratio to 70 per 100,000 live births. In Namibia, the ratio was reported as 265 per 100,000 live births in 2015 and yet little is published on emergency obstetric care. The majority of deliveries in Namibia are facility-based. The aim of this study was to assess and improve the quality of care for women with PPH at Onandjokwe Hospital, Namibia. METHODS: A criterion-based audit cycle in all 82 women with PPH fr

Quality-of-care audits and perinatal mortality in South Africa.

PROBLEM: Suboptimal care contributes to perinatal mortality rates. Quality-of-care audits can be used to identify and change suboptimal care, but it is not known if such audits have reduced perinatal mortality in South Africa. APPROACH: We investigated perinatal mortality trends in health facilities that had completed at least five years of quality-of-care audits. In a subset of facilities that began audits from 2006, we analysed modifiable factors that may have contributed to perinatal deaths. LOCAL SETTING: Since the 1990s, the perinatal problem identification programme has performed quali

Do we really know how to respond to an unexpected event during the fitting of an intra-uterine contraceptive device?

OBJECTIVE: To assess and improve knowledge of correct resuscitation procedures if an unexpected event occurs during an intra-uterine device (IUD) fitting. DESIGN: Previous literature on resuscitation in the family planning clinic was reviewed to provide a basis for a questionnaire for doctors and nurses within the service to assess knowledge of correct procedures. Outcome of first questionnaire: Seventeen questionnaires were returned in the first round, of which 12 were from nurses. The results to the first question gave a 70% correct response rate. The response to the second gave a correct r

Flushing and locking management related to central venous catheter occlusion rate among adult patients in acute care: a best practice implementation project.

INTRODUCTION AND OBJECTIVES: A central venous catheter (CVC)-a type of central venous access device (CVAD)-is the most common intervention for critical illnesses. Obstruction of the CVC can lead to fatal consequences. Thus, it is critical to maintain catheter lumen patency. The CVC occlusion rate in a hospital in Taiwan was 33%. This project aimed to decrease the CVC occlusion rate in acute care. METHODS: This project was conceptually informed by the JBI Evidence-Based Model of Healthcare, in particular, the conceptualization of evidence implementation as inclusive of context analysis, implem

Mixed-methods evaluation of a multifaceted heart failure intervention in general practice: the OSCAR-HF pilot study.

AIMS: Heart failure (HF) is an important health problem for which multidisciplinary care is recommended, yet few studies involve primary care practitioners in the multidisciplinary management of HF. We set up a multifaceted prospective observational trial, OSCAR-HF, piloting audit and feedback, natriuretic peptide testing at the point of care, and the assistance of a specialist HF nurse in primary care. The aim was to optimize HF care in general practice. METHODS AND RESULTS: This is an analysis at 6 month follow-up of the study interventions of the OSCAR-HF pilot study, a nonrandomized, nonc

Reducing the cost of lower limb wound management through industry partnership and staff education.

Following an audit of practice in North East London Foundation Trust (NELFT), obstacles in the management of lower limb conditions were identified. An appraisal of needs in terms of skills and theory updates for staff led to a fixed-term 'honorary contract' between the trust and a wound-care company to facilitate a rolling programme of education, to upskill staff in terms of assessment and treatment, and develop standardised care pathways. After 3 months, a repeated practice audit revealed a reduction in nurse contact hours of 1156 hours. The partnership with industry proved to be beneficial a

Analgesics in the management of chronic pain. Part One: general principles.

This article is the first in a series of five reviewing the management of chronic, intractable pain which is invariably associated with malignant or wasting disease. Audits continue to show that pain of this type is often poorly controlled, despite the availability of effective treatments and the fact that it is perceived by patients as one of the most distressing aspects of their illness. The first article covers the general principles which underly pain management, including the concept of the Analgesic Stepladder.

Fluid balance monitoring in congestive heart failure patients in hospital: a best practice implementation project.

OBJECTIVES: This aim of this project was to promote best practice in fluid balance monitoring in congestive heart failure patients. INTRODUCTION: Fluid overload or pulmonary/vascular congestion is a common clinical feature in patients with heart failure and is associated with adverse outcomes. Maintaining records of patients' fluid intake and output (I&O) has long been considered an important aspect of nursing care to assess hydration status. In clinical practice, a prevalence of incongruence between I&O results and clinical signs and symptoms has been noted. We postulate that this incongruen

Preventing pressure ulcers from paediatric femoral traction: use of an audit cycle to assess a new concise manual and a daily care chart.

INTRODUCTION: Femoral shaft fractures are common in children up to 14 years of age and traction is frequently used during their treatment. A lack of training and unfamiliarity of junior doctors and nursing staff with this treatment modality may lead to unfavourable skin complications, especially in the absence of regular monitoring. We introduced and audited a simple and reproducible way of monitoring these patients. METHODS: An initial audit was conducted of all children with femoral shaft fracture treated in skin traction. A new traction manual and daily care chart were introduced, and a re

The provision of urethral catheters: an equipment audit.

Although medical and nursing audit is attaining a high profile in UK hospitals, the development of objective measures remains a challenge. The availability and storage or urinary catheters was recorded by a point prevalence survey in a District General Hospital. A "long-term" and "short-term" catheter were provided in all areas, but seven further brands of catheter were found. Female length catheters were rarely available. With some limited exceptions there were no written or verbal policies concerning selection of catheters in the wards. Excess numbers of catheters were stored by wards respon

When urinary catheters stay too long: the hidden link between constipation, prolonged indwelling urinary catheterisation and urinary tract infections in neurosurgical inpatients.

OBJECTIVE: Urinary catheterisation is common in neurosurgical perioperative care, but inconsistent guideline adherence contributes to variable practice and complication risk. Over five years, linked audits and quality improvement projects (QIPs) at the National Hospital for Neurology and Neurosurgery investigated high urinary tract infection (UTI) rates, identified root causes and implemented targeted interventions. METHODS: This single-centre prospective quality improvement initiative included three sequential audits and six linked QIPs from January 2020 to December 2024. Projects assessed U
