The Challenges of Establishing an Optimal Link Between Nutritional Requirements, Dietary Knowledge and Culinary Skills in Chronic Kidney Disease: An Exploratory Review of the Literature.

Background/Objectives: Chronic kidney disease (CKD) requires complex Medical Nutrition Therapy, yet dietary non-adherence remains high (20-70%) due to a gap between theoretical knowledge and practical application. This scoping review aims to map the literature exploring the intersection between nutritional requirements, dietary knowledge (food literacy), and culinary skills in adults with CKD (stages 3-5 and dialysis). Methods: Following JBI methodology and PRISMA-ScR guidelines, a systematic search was conducted across PubMed, Web of Science, Scopus, and EBSCO. Studies were screened using the

Polycystic ovary syndrome (PCOS) management using a nutrition recommender mobile application: identifying key requirements.

OBJECTIVE: Polycystic Ovary Syndrome (PCOS) is a complex endocrine disorder that affects 5-18% of women worldwide. Adopting a healthier lifestyle, especially by making nutritious dietary choices, can significantly improve the management of symptoms and reduce complications associated with this condition. Despite the growing number of mobile health (mHealth) applications, most existing tools for PCOS lack scientific validation and dietary recommendations. This study aims to identify key requirements for developing a nutritional recommender mobile application for PCOS management. MATERIALS AND

The Toxoplasma oxygen-sensing protein, TgPhyA, is required for resistance to interferon gamma-mediated nutritional immunity in mice.

As Toxoplasma gondii disseminates through its host, the parasite must sense and adapt to its environment and scavenge nutrients. Oxygen (O2) is one such environmental factor and cytoplasmic prolyl 4-hydroxylases (PHDs) are evolutionarily conserved O2 cellular sensing proteins that regulate responses to changes in O2 availability. Toxoplasma expresses 2 PHDs. One of them, TgPHYa hydroxylates SKP1, a subunit of the SCF-E3 ubiquitin ligase complex. In vitro, TgPHYa is important for growth at low O2 levels. However, studies have yet to examine the role that TgPHYa or any other pathogen-encoded PHD

Acute severe pancreatitis and multiple organ failure: total parenteral nutrition is still required in a proportion of patients.

AIMS: Enteral nutrition (EN) is increasingly advocated as the favoured means of nutritional support in patients with multiple organ failure resulting from acute severe pancreatitis. In a proportion of patients, however, EN may not be feasible either because of gastrointestinal stasis and high nasogastric aspirates or because of superseding complications of pancreatitis such as fistulation. In these cases total parenteral nutrition (TPN) is indicated. METHODS: Patients with acute severe pancreatitis admitted to the intensive care unit (ICU) were commenced on either nasogastric or, where establi

Propofol-induced hypertriglyceridemia in critically ill trauma patients requiring nutrition therapy: A descriptive cohort study.

BACKGROUND: This study aimed to assess the incidence and identify risk factors associated with the development of propofol-induced hypertriglyceridemia in critically ill trauma patients who required nutrition therapy. METHODS: Adult patients (aged >17 years) admitted to the trauma intensive care unit who received propofol for >24 h and had a serum triglyceride concentration determination during the infusion were retrospectively evaluated. Hypertriglyceridemia was defined as a serum triglyceride concentration > 400 mg/dL. Potential risk factors for hypertriglyceridemia as well as linear and no

Influence of dosimetric and clinical criteria on the requirement of artificial nutrition during radiotherapy of head and neck cancer patients.

PURPOSE/OBJECTIVE(S): Intensification of radiotherapy and chemotherapy for head-and-neck cancer (HNC) may lead to increased rates of long term dysphagia as a severe side effect. Mucositis and consequent swallowing problems require artificial nutrition in many HNC patients undergoing radiotherapy or chemoradiation. It is unknown, which predict factors for prophylactic PEG tube insertion appear useful. MATERIALS/METHODS: From an institutional database, 101 patients (72 male, 29 female, mean age 59.5years) were identified who underwent radiotherapy or chemoradiation for HNC. Primary end point of

Whole-blood-free choline and choline metabolites in infants who require chronic parenteral nutrition therapy.

BACKGROUND AND AIM: Choline deficiency is associated with hepatic dysfunction. Parenteral nutrition (PN) and lipid emulsions contain phosphatidylcholine (PtdCho) but insignificant free choline (FCho). PtdCho is sequentially degraded to glycerolphosphocholine (GPCho), phosphocholine (PCho), and finally to FCho. Biosynthesis of FCho may be insufficient during PN therapy. The aim of the study was to examine the status of FCho and related metabolites in infants on prolonged (> or =4 weeks) PN. METHODS: Whole blood concentrations of FCho, PtdCho, GPCho, and PCho were measured and compared in infan

Impairment of mucosal immunity by total parenteral nutrition: requirement for IgA in murine nasotracheal anti-influenza immunity.

Secretory IgA (SIgA) is the primary mucosal Ig and has been shown to mediate nasotracheal (NT) mucosal immunity in normal immune BALB/c mice. This finding has been challenged by a report of NT immunity without IgA in knockout mice, suggesting that IgA may not be necessary for the protection of mucosal surfaces. Although other protective mechanisms may become active in the congenital absence of SIgA, these mechanisms are not the primary means of protection in normal mice. In this paper we show that feeding chemically defined total parenteral nutrition (TPN) to genetically normal, immune ICR mic

Complications Requiring Intensive Nutritional Care After Bariatric Surgery Result in More Long-Term Weight Loss but Has No Impact on Nutritional Deficiencies and Depression-Anxiety Scores.

PURPOSE: Bariatric surgery is associated with significant weight loss and improvement in comorbid conditions but in rare cases can expose to complications requiring intensive nutritional care (INC). INC in this context is poorly described and no data are available concerning long-term impact. MATERIALS AND METHODS: We retrospectively reviewed charts of bariatric patients who were hospitalized in our institution between 2013 and 2018. We identified patients with a postoperative complication requiring INC and we described their nutritional management (INC group). These patients were compared wi

A Novel Nutritional Index Serves as A Useful Prognostic Indicator in Cardiac Critical Patients Requiring Mechanical Circulatory Support.

BACKGROUND: A poor nutritional status has been gathering intense clinical interest recently as it has been suggested to associate with adverse outcomes in patients in the intensive care unit (ICU). However, there is still no established nutritional index dominantly used in clinical practice. We have previously proposed a novel nutritional index, which can be calculated using serum levels of triglycerides, total cholesterol, and body weight (TCBI). In this study, to expand the application of TCBI for critical patients, we investigated the usefulness of TCBI to predict prognosis in hemodynamical

The Effect of Glutamine Supplementation on Microbial Invasion in Surgical Infants Requiring Parenteral Nutrition: Results of a Randomized Controlled Trial.

BACKGROUND: To determine whether parenteral plus enteral glutamine supplementation influences microbial invasion in surgical infants who require parenteral nutrition (PN). METHODS: An prospective double-blind randomized controlled trial studying surgical infants receiving PN for at least 5 days for congenital or acquired intestinal anomalies (2009-2012) was used. Infants were randomized to receive either glutamine supplementation (parenteral plus enteral; total 400 mg/kg/d) or isonitrogenous control. The primary end point was microbial invasion evaluated after 5 days of supplementation and de

Pharmacokinetics of dabigatran etexilate and rivaroxaban in patients with short bowel syndrome requiring parenteral nutrition: The PDER PAN study.

BACKGROUND AND AIMS: Patients on parenteral nutrition for short bowel syndrome (SBS) have a high risk of thrombotic complications and are often treated with parenteral anticoagulation. Direct oral anticoagulants are absorbed proximally in the digestive tract and may represent alternative regimens in selected SBS patients. In our pilot study, we provided pharmacokinetics parameters of dabigatran etexilate and rivaroxaban in this setting and compared peak (Cmax), trough (Ctrough) concentrations, and areas-under-the-concentration-time-curve (AUC0-t) to reference values retrieved from phase I-III

Prolonged enteral feeding is often required to avoid long-term nutritional and metabolic complications after esophagogastric dissociation.

BACKGROUND: Total esophagogastric dissociation (TED) was first described in 1997 by Bianchi as a new surgical procedure to treat severe gastroesophageal reflux disease (GERD) in children with neurological impairment. Recently, TED has been proposed in other conditions, such as esophageal atresia, esotracheal cleft, or caustic esophageal lesions. Although the long-term results in terms of GERD control have been previously reported, those regarding the nutritional and metabolic status have never been documented. PATIENTS AND METHODS: All patients without neurological impairment with TED between

Is replacement therapy with nutritional and active forms of vitamin D required in chronic kidney disease mineral and bone disorder?

PURPOSE OF REVIEW: Apart from regulating mineral metabolism, recent evidence suggests that both the 'active' 1,25(OH)2D and the 'storage' 25(OH)D forms of vitamin D have distinct and important actions on many organ systems, bringing to light the systemic importance of both forms of the vitamin. This review discusses the current evidence related to the etiology and consequences of reduced levels of both 25(OH)D and 1,25(OH)2D in the chronic kidney disease population. RECENT FINDINGS: Both 25(OH)vitamin D and 1,25(OH)2vitamin D therapy have actions on parathyroid gland, cardiac, bone, and immun

NUTrition therapy in PAEdiatric patients requiring ExtraCorporeal Membrane Oxygenation: Protocol for a prospective observational multisite study (PAENUT ECMO).

INTRODUCTION: Extracorporeal membrane oxygenation (ECMO) provides life-sustaining gas exchange and haemodynamic support for critically ill patients with severe respiratory or circulatory failure. Paediatric patients supported on ECMO often represent the sickest and most vulnerable patients admitted to the paediatric intensive care unit (PICU). Despite an assumption that nutrition is important in paediatric patients receiving ECMO, data informing optimal nutrition intake during ECMO support and its impact on outcomes are scarce, resulting in limited evidence-based guidelines. Our objective is t

The Bradyrhizobium japonicum fsrB gene is essential for utilization of structurally diverse ferric siderophores to fulfill its nutritional iron requirement.

In Bradyrhizobium japonicum, iron uptake from ferric siderophores involves selective outer membrane proteins and non-selective periplasmic and cytoplasmic membrane components that accommodate numerous structurally diverse siderophores. Free iron traverses the cytoplasmic membrane through the ferrous (Fe2+ ) transporter system FeoAB, but the other non-selective components have not been described. Here, we identify fsrB as an iron-regulated gene required for growth on iron chelates of catecholate- and hydroxymate-type siderophores, but not on inorganic iron. Utilization of the non-physiological
