The obesity paradox: Underweight patients are at the greatest risk of mortality after cholecystectomy.

BACKGROUND: Elevated body mass index is a risk factor for gallstone disease and cholecystectomy, but outcomes for low body mass index patients remain uncharacterized. We examined the association of body mass index with morbidity, mortality, and resource use after cholecystectomy. METHODS: The 2005 to 2016 American College of Surgeons National Surgical Quality Improvement Program was retrospectively analyzed for adult patients undergoing laparoscopic and open cholecystectomy. Patients were stratified into 5 groups: body mass index <18.5 (underweight), body mass index 18.5 to 24.9 (normal weigh

BMI, Mortality, and Cardiovascular Outcomes in Type 1 Diabetes: Findings Against an Obesity Paradox.

OBJECTIVE: Low weight has been associated with increased mortality risks in type 1 diabetes. We aimed to investigate the importance of weight and weight gain/loss in the Swedish population diagnosed with type 1 diabetes. RESEARCH DESIGN AND METHODS: Patients with type 1 diabetes (n = 26,125; mean age 33.3 years; 45% women) registered in the Swedish National Diabetes Registry from 1998 to 2012 were followed from the first day of study entry. Cox regression was used to calculate risk of death from cardiovascular disease (CVD), major CVD events, hospitalizations for heart failure (HF), and total

Obesity paradox disappears in coronary artery bypass graft patients during 20-year follow-up.

AIMS: Although obesity is a risk factor for coronary heart disease (CHD), it might be associated with a favourable prognosis in patients with CHD. The aim of the study was to evaluate this so called 'obesity paradox' during a follow-up period of 20 years in patients who had undergone coronary artery bypass grafting (CABG). METHODS AND RESULTS: The study population consisted of 922 CHD patients who had undergone CABG between 1993 and 1994. Pre and perioperative data was collected from patient records and supplemented with patient questionnaires, telephone contacts and data from national archiv

Applying family stress theory to the food insecurity childhood obesity paradox: path model results from a rural pediatric obesity intervention study.

OBJECTIVE: Food insecurity impacts mental and physical health for individuals of all ages. This study evaluates associations between food insecurity and weight status, diet, and mental health, as well as tests a path model to evaluate proposed patterns (i.e., parent BMI, parent distress, and child diet) of food insecurity's relationship with child obesity. METHODS: This study includes a cross-sectional analysis of baseline data from parent-child dyads enrolled in rural, family-based obesity intervention. Dyads in rural Kansas (N = 148; 25% food insecure; child age 6-10 years) were recruited b

Impact of obesity after cytoreductive surgery and intraperitoneal hyperthermic chemotherapy for rare peritoneal malignancies: Paradox of obesity or poor prognostic factor?

BACKGROUND: Obesity is a public health concern with an increasing occurrence worldwide. Literature regarding impact of obesity on results after management of peritoneal carcinomatosis is poor. Our aim was to compare postoperative and oncological outcomes after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for rare peritoneal malignancies according to the body mass index. METHODS: All the patients managed by cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for rare peritoneal malignancies (including mainly pseudomyxoma peritonei and peritoneal mesotheli

Circulating and dietary advanced glycation end products and obesity in an adult population: A paradox of their detrimental effects in obesity.

BACKGROUND: The detrimental role of advanced glycation end products (AGEs) against cardio-metabolic health has been revealed in several previous reports. However, the results of studies regarding the association between AGEs and obesity measurements are inconsistent. In the current meta-analysis, we aimed to quantitatively summarize the results of studies that evaluated the association between circulating and dietary AGEs with obesity measurements among the adult population. METHODS: A systematic search from PubMed, Embase, and Scopus electronic databases until 30 October 2022 retrieved a tot

Obesity paradox in patients with non-small cell lung cancer undergoing immune checkpoint inhibitor therapy.

BACKGROUND: The obesity paradox in patients with advanced non-small cell lung cancer receiving immune checkpoint inhibitor therapy has been observed, but its underlying mechanism is not fully understood. We aimed to investigate whether body composition affects the prognostic impact of obesity, as determined by body mass index (BMI), on survival. METHODS: This retrospective study evaluated the data collected from Asian patients who were treated with immune checkpoint inhibitors for advanced non-small cell lung cancer between October 2015 and October 2021. We used abdominal cross-sectional imag

"Obesity Paradox" in Acute Respiratory Distress Syndrome Among Patients Undergoing Cardiac Surgery: A Retrospective Study.

BACKGROUND The "obesity paradox" exists in many diseases. It is unclear whether it also exists in acute respiratory distress syndrome (ARDS). The purpose of our study was to clarify the relationship between obesity and the development of and hospital mortality from ARDS among patients who underwent cardiac surgery. MATERIAL AND METHODS This retrospective case-control study included 202 patients with ARDS and 808 matching patients without ARDS. We clarified the relationship between obesity and the development of ARDS after adjusting for confounding factors by multiple logistic regression analys

The obesity paradox in critically ill patients: a causal learning approach to a casual finding.

BackgroundWhile obesity confers an increased risk of death in the general population, numerous studies have reported an association between obesity and improved survival among critically ill patients. This contrary finding has been referred to as the obesity paradox. In this retrospective study, two causal inference approaches were used to address whether the survival of non-obese critically ill patients would have been improved if they had been obese.MethodsThe study cohort comprised 6557 adult critically ill patients hospitalized at the Intensive Care Unit of the Ghent University Hospital be

Impact of body mass index on outcomes following mitral surgery: does an obesity paradox exist?

OBJECTIVES: This study was conducted to clarify the relationship between body mass index and mitral valve (MV) surgery and to determine whether an 'obesity paradox' exists in the context of surgery for degenerative MV disease. METHODS: In this retrospective single-centre study, we analysed data from 715 patients who underwent mitral surgery for degenerative disease from 2000 to 2015. Patients were classified according to body mass index: underweight (<20 kg/m2), normal weight (20-24.99 kg/m2), overweight (25-29.99 kg/m2) and obese (≥30 kg/m2). Early and long-term results were investigated. Mu

An obesity paradox: an inverse correlation between body mass index and atherosclerosis of the aorta.

BACKGROUND AND AIMS: Morbid obesity generally has been associated with higher morbidity and mortality for a variety of diseases. However, a number of exceptions to this have been reported and referred to as the "obesity paradox." The purpose of the present study was to obtain objective data on aortic atherosclerosis and its relationship to body mass index (BMI, kg/m2), based on autopsy findings in a large cohort of overweight and obese decedents. METHODS: Decedents were ≥18 years who had autopsies between 2003 and 2014, a subset of whom were morbidly obese (BMI≥40). Autopsy findings were revi

Weighing the evidence: The obesity paradox and its impact on lower extremity bypass surgery outcomes.

BACKGROUND: The obesity paradox, a reduced risk in overweight/obese patients but higher risk at body mass index extremes, has uncertain applicability for lower extremity bypass. We analyzed postoperative outcomes by body mass index category in patients undergoing lower extremity bypass. METHODS: We analyzed 67,842 lower extremity bypass patients using the TriNetX platform. Body mass index stratification included underweight, normal, overweight, and obesity class I-III. Each body mass index group was 1:1 propensity score matched to normal body mass index patients. Outcomes included mortality,

Unmasking Risk Behind the Obesity-Paradox Signal: Prognostic Value of CONUT Score Combined with BMI.

The "obesity paradox" suggests better outcomes in overweight cardiovascular patients, yet the prognostic role of poor nutritional status-even in obese individuals-remains unclear. We evaluated whether combining body mass index (BMI) with the Controlling Nutritional Status (CONUT) score refines interpretation of the obesity-paradox signal in patients undergoing percutaneous coronary intervention (PCI).We analyzed 631 SAKURA-PCI2 Registry patients (2020-2022) and classified them into four groups: Group 1 (CONUT 0-1/BMI < 25 kg/m2), Group 2 (CONUT 0-1/BMI ≥ 25 kg/m2), Group 3 (CONUT ≥ 2/BMI < 25

Revisiting the Obesity Paradox: Stroke Subtype-Specific Associations of BMI and WHR with Functional Outcome.

INTRODUCTION: While obesity is a known risk factor for ischemic stroke, its prognostic value remains uncertain. We examined the independent and combined effects of body mass index (BMI) and waist-to-hip ratio (WHR) on early stroke outcomes across subtypes. METHODS: In this prospective cohort study, 714 patients with acute ischemic stroke or transient ischemic attack were enrolled over 6 months. BMI and WHR were assessed on admission. Stroke severity (NIHSS) and functional outcome at discharge (modified Rankin Scale, mRS) were recorded. Stroke etiology was classified using TOAST and ESUS crite

Cachectic biomarkers as confounders behind the obesity paradox in patients with acute decompensated heart failure.

BACKGROUND: Obesity is a risk factor for heart failure (HF) development but is associated with a lower incidence of mortality in HF patients. This obesity paradox may be confounded by unrecognized comorbidities, including cachexia. METHODS: A retrospective assessment was conducted using data from a prospectively recruiting multicenter registry, which included consecutive acute heart failure patients. A low, normal, and high body mass index (BMI) was defined as <20 kg/m2, 20-25 kg/m2, and ≥25 kg/m2, respectively. Cachexia was defined as a combination of BMI < 20 kg/m2 and any biochemical abnor

Obesity paradox in uveal melanoma: high body mass index is associated with low metastatic risk.

BACKGROUND: Metabolic factors and obesity may influence the development and progression of cancer. In this study, we examine their association with the risk of developing metastases of uveal melanoma. METHODS: Data on metabolic factors, medications, serum leptin levels, tumour leptin receptor RNA expression and clinical outcomes were examined in three cohorts. HRs for metastasis and cumulative incidences of melanoma-related mortality were calculated, and the levels of tumour leptin receptor expression were compared with prognostic factors including BAP1 mutation, and tumour cell morphology.

The obesity paradox exists for perioperative complications and mortality following lower extremity arterial bypass surgery.

OBJECTIVE: The obesity paradox refers to a phenomenon by which obese individuals experience lower risk of mortality and even protective associations from chronic disease sequelae when compared with the non-obese and underweight population. Prior literature has demonstrated an obesity paradox after cardiac and other surgical procedures. However, the relationship between body mass index (BMI) and perioperative complications for patients undergoing major open lower extremity arterial revascularization is unclear. METHODS: We queried the Vascular Quality Initiative for individuals receiving unila
