The National Institute of Mental Health has proposed a shift toward classifying clusters of disorders on the basis of underlying biomarkers and neurological correlates. The present study sought to determine whether cognitive flexibility represents one such construct underlying obsessive-compulsive behaviors (OCBs), a cluster of behaviors characteristic of OCD and other body-focused repetitive behaviors (BFRBs), including trichotillomania, pathological skin picking, nail biting, and tic disorders. One-hundred and twenty-four undergraduate students completed the Depression Anxiety and Stress Sca

Obsessive Compulsive Disorder (OCD) is characterized by recurrent, anxiety-producing thoughts accompanied by unwanted, overwhelming urges to perform ritualistic behaviors. Pharmacological treatments for this disorder (serotonin uptake inhibitors) are problematic because there is a 6-8 week delayed onset and half of the patients do not adequately respond. The present study evaluated whether Ritualistic Chewing Behaviors (RCBs) induced by the serotonin agonist mCPP in the rat is a behavioral model for OCD. The effects upon the RCBs induced by mCPP (1 mg/kg) were evaluated following treatments wi

This case report outlines the cognitive-behavioral treatment of obsessive-compulsive disorder in a 7-year-old female with Asperger syndrome. Interventions were based upon the work of March and Mulle and were adapted in light of the patient's cognitive, social, and linguistic characteristics. Obsessive-compulsive symptoms improved markedly after approximately 6 months of treatment. Issues regarding symptom presentation, assessment, and treatment of a dually diagnosed patient are discussed.

The increasing use of visual social media has intensified societal pressure to achieve flawless skin and may contribute to excessive preoccupation with appearance and skincare. The emerging terms dermorexia and cosmeticorexia have been used to describe potentially compulsive skincare behaviors extending beyond conventional self-care. This narrative review examines the proposed concept of dermorexia, its conceptual overlap with recognized psychiatric conditions, potential risk factors and clinical manifestations, dermatological and psychological consequences, and implications for aesthetic prac

Cognitive-behavioral therapy (CBT) is the first-line treatment for obsessive-compulsive disorder (OCD), yet a significant number of patients do not achieve remission or substantial symptom relief. This study aims to enhance the prediction of CBT outcomes in OCD by integrating demographic, clinical, and neuroimaging data using machine learning (ML) models. We conduct a comprehensive analysis on a well-characterized clinical sample, employing a rigorous validation scheme to avoid data leakage, and comparing multiple ML algorithms to minimize bias. Out of four different ML models trained on demog

Exposure and response prevention (EX/RP) is a first-line treatment for obsessive-compulsive disorder (OCD), yet many patients fail to achieve full remission. Among the strongest predictors of treatment outcome is patient adherence-particularly to between-session exposure exercises and ritual prevention. However, limited research has examined factors that influence adherence. This study aimed to replicate and extend prior findings by identifying predictors of both treatment outcome and adherence among medicated adults with OCD undergoing EX/RP, and by evaluating adherence as a potential mediato

OBJECTIVE: This review will identify and compare diverse approaches to group cognitive behavioral therapy (CBT) for obsessive-compulsive disorder (OCD).
INTRODUCTION: CBT is an established treatment for OCD. Although the benefits of group CBT are well known, its practice varies widely. Despite the growing number of studies on group CBT, the extent of heterogeneity in its structure and content has not been examined.
ELIGIBILITY CRITERIA: Eligibility criteria will include adult patients (18 years or older) of any gender, with a primary diagnosis of OCD. Articles must report on group CBT implem

This study describes the implementation of an Internet-based cognitive-behavior therapy program (ICBT) developed in Sweden for individuals seeking OCD treatment in Sao Paulo, Brazil. After translating and adapting the treatment manual (OCD-NET), we conducted an open-label trial including 35 adults with OCD. The ICBT treatment was conducted in 10 modules and was based on Exposure and Response Prevention (EPR). The RE-AIM implementation science framework was used to evaluate Reach, Effectiveness, Adoption, Implementation, and Maintenance. Our primary outcome was the reduction of the Yale-Brown O

Defense mechanisms (DMs) are strategies used by the individuals to protect the ego. Therefore, compulsive behaviors in obsessive-compulsive disorder (OCD) can be recognized as DMs. We analyzed how DMs changed in a brief cognitive behavioral therapy (CBT) for OCD. This was a quasi-experimental study with 92 OCD patients (aged 18-60 years). We used the Mini International Neuropsychiatric Interview to confirm OCD diagnosis, and we assessed the DMs with the Defense Style Questionnaire at three time points. Through a latent change score modeling, we found that the mature mechanism presented a const

This article proposes a multicomponent hypnotherapeutic approach for obsessive-compulsive disorder (OCD) treatment. This new approach combines hypnosis with exposure and response prevention, cognitive reappraisal, principles of acceptance and commitment therapy, and other components. In the presented case study, the patient was treated for four months with both biological and psychological first-line therapy with minor effects; the patient was then treated with hypnotherapy. The patient listened to a recorded hypnosis session, which was constructed according to the new proposed principle. Afte

The generation of appropriate behavioral responses involves dedicated neuronal circuits. The cortico-striatal-thalamo-cortical loop is especially important for the expression of motor routines and habits. Defects in this circuitry are closely linked to obsessive stereotypic behaviors, hallmarks of neuropsychiatric diseases including autism spectrum disorders (ASDs) and obsessive-compulsive disorders (OCDs). However, our knowledge of the essential synaptic machinery required to maintain balanced neurotransmission and plasticity within the cortico-striatal circuitry remains fragmentary. Mutation

Exam-related unwanted intrusive thoughts (UITs) and related neutralizing behaviors are common experiences among students. The present study investigated in what ways these UITs and behaviors are analogues to clinical obsessions and compulsions. Twenty-nine students completed three ecological momentary assessment surveys per day over 7 consecutive days, assessing the severity of exam-related UITs and related neutralizing behaviors, obsessive-compulsive (OC) symptoms, anxiety, distress, urge to neutralize, depressed mood, and stress in the week immediately before an exam period. BACKGROUND: Cognitive behavioral therapy (CBT) is an effective, first-line treatment for pediatric obsessive-compulsive disorder (OCD). While neural predictors of treatment outcomes have been identified in adults with OCD, robust predictors are lacking for pediatric patients. Herein, we sought to identify brain structural markers of CBT response in youth with OCD.
METHODS: Twenty-eight children/adolescents with OCD and 27 matched healthy participants (7- to 18-year-olds, M = 11.71 years, SD = 3.29) completed high-resolution structural and diffusion MRI (all unmedicated at time of scanning). P

BACKGROUND AND OBJECTIVES: This study examined the occurrence of sudden gains (or reversal of gains) among children with obsessive-compulsive disorder (OCD) during the course of cognitive-behavioral therapy (CBT), as well as the association of sudden gains with treatment response, treatment group, and pre-treatment clinical characteristics.
METHODS: The sample consisted of 136 youth (ages 7-17) with a primary diagnosis of OCD who were randomized in a double-blinded fashion to 10 sessions of CBT with augmentation of either d-cycloserine or placebo. Sudden gain status was determined based on cl

Cross-sectional studies have reported failures in goal-directed planning in obsessive-compulsive disorder (OCD). It remains unclear whether these deficits confer vulnerability to developing OCD, or are a consequence of symptoms. The present study examined goal-directed learning before and after cognitive behavioral therapy (CBT), using treatment as a tool to reduce symptoms. Eighteen adult OCD patients undergoing 17 sessions of CBT completed an established task of model-based (i.e., goal directed) versus model-free planning as well as measures of OCD and depression before and after treatment.

This review paper critically examines literature regarding restricted and repetitive behaviors (RRBs) in Autism Spectrum Disorder (ASD) and Obsessive-Compulsive Disorder (OCD). The similar behavioral profiles of these disorders presents the potential for confusion regarding diagnoses and intervention efforts. As such, this review highlights the similarities and differences between RRBs in ASD and OCD. The developmental trajectories of RRBs are presented, followed by an exploration of three constructs implicated in RRB manifestation: anxiety, executive functioning, and sensory phenomena. While

OBJECTIVE: The pathophysiology of obsessive-compulsive disorder (OCD) involves increased activity in corticostriatal circuits connecting the anterior cingulate cortex with other brain regions. The error-related negativity (ERN) is a negative deflection in the event-related potential after an incorrect response that is believed to reflect anterior cingulate cortex activity. This study examined the relation of the ERN to OCD symptom dimensions and other childhood symptom dimensions.
METHOD: The ERN, correct response negativity, and accuracy were measured during a flanker task to assess performa

BACKGROUND AND OBJECTIVES: Obsessive compulsive (OC) phenomena such as contamination fears may lead to significant impairment in daily functioning. In this research, we examined whether threat to moral self-perceptions can trigger contamination-related behavioral tendencies.
METHOD: Three experiments examined the influence of subtle priming of morality-related information on contamination-related behavioral tendencies.
RESULTS: Subtle suggestions of incompetence in the morality self-domain led to heightened OC-related behavioral tendencies. These effects were specific to self-relevant (versu

Eating disorders are believed to range across a spectrum of varying degrees of obsessive-compulsive and impulsive behavior. Sixty anorexic (mean age = 19.8; sd = 5.9) and 109 bulimic (mean age = 26.9; sd = 11.3) female patients completed self-report questionnaires assessing obsessive-compulsiveness, impulsivity, depression and anxiety, as well as two eating disorder scales. Results yielded significantly higher levels of impulsivity and negative body image in the bulimic compared to the anorexic group. Regression analysis predicting impulsivity showed that bulimia and negative body image were t

BACKGROUND: Deep brain stimulation (DBS) of the ventral capsule/ventral striatum (VC/VS) can alleviate symptoms of obsessive-compulsive disorder (OCD) refractory to conventional therapies. Overstimulation, however, can elicit pathological behaviors marked by disinhibition and recklessness.
OBSERVATIONS: Two patients with OCD underwent DBS of the VC/VS region and met responder status within 1 month of stimulation onset, as evidenced by a ≥ 35% reduction in their Yale-Brown Obsessive Compulsive Scale score. Notably, both patients exhibited signs of disinhibited behavior soon after DBS activatio
