Laparoscopic vaginal radical trachelectomy: a treatment to preserve the fertility of cervical carcinoma patients.

BACKGROUND: Cervical carcinoma occurs frequently in young women who would like to preserve their childbearing potential. For those with early stage invasive lesions, the authors designed and performed radical trachelectomy, a surgical procedure that preserves the functions of the uterus. METHODS: Radical trachelectomy combines laparoscopic (for pelvic lymphadenectomy) and transvaginal approaches. Between April 1987 and December 1996, 56 patients were scheduled for this procedure, and 47 underwent it. The charts of these patients were retrospectively reviewed for medical and obstetric history,

Necrotizing fasciitis following postpartum tubal ligation. A case report and review of the literature.

Necrotizing fasciitis is a rare, but devastating subcutaneous bacterial infection which occurs following breaks in skin integrity, either natural, post traumatic or post surgical. Although it has been described following many surgical procedures, necrotizing fasciitis has not been previously described following postpartum tubal ligation. Necrotizing fasciitis was diagnosed four days after an uncomplicated postpartum tubal ligation via an infraumbilical incision. Rapid surgical debridement with broad spectrum antibiotic coverage provided successful therapy. Postpartum tubal ligation is one of t

Lower urinary tract injury in women in the United States, 1979-2006.

OBJECTIVE: We sought to determine age-adjusted rates (AARs) of lower urinary tract injury and incidence in selected inpatient gynecologic and obstetric procedures. STUDY DESIGN: We utilized the National Hospital Discharge Survey, 1979-2006. AARs of nonobstetric bladder and ureteral injuries and incidence of lower urinary tract injury for various hysterectomy types and deliveries were calculated for women>18 years old. RESULTS: Overall AARs of ureteral injury decreased from 0.06-0.03 per 1000 women (1979-2006). AARs of inpatient gynecologic procedures decreased from 24.9-11.8 per 1000 women (

Pregnancy Outcomes of 512 Laparoscopic Transabdominal Cerclages: The Australian Cohort.

STUDY OBJECTIVE: Cervical insufficiency is recognized as an important cause of obstetric morbidity and mortality, leading to midtrimester loss and preterm birth. Laparoscopic transabdominal cerclage is one of the proposed interventions to manage this condition. The study reports on a large series of pregnancies with laparoscopic transabdominal cerclage to assess its effectiveness. DESIGN: This study evaluates laparoscopic transabdominal cerclage as a procedure to improve obstetric outcomes in patients diagnosed with cervical insufficiency. It is a retrospective observational study of consecut

Patient characteristics, surgery outcomes, presumed aetiology and other characteristics of fistula surgeries and related procedures supported by Fistula Foundation from 2019 to 2021: a multicentre, retrospective observational study.

OBJECTIVES: Obstetric fistula is a devastating childbirth injury primarily caused by prolonged, obstructed labour. It leaves women incontinent, severely stigmatised and isolated. Fistula repair surgery can restore a woman's health and well-being. Fistula Foundation, a non-profit organisation, works in partnership with local hospitals and community organisations in Africa and Asia to address key barriers to treatment and to increase the number of women receiving surgical care. This paper presents data on fistula and fistula repair surgery across a large global network of hospitals supported by

Obstetric Hysterectomy at a Greek Tertiary Referral Center: Differences in Maternal and Neonatal Outcomes Between Elective and Urgent Procedures.

Background: Obstetric hysterectomy remains a life-saving intervention for placenta accreta spectrum and uncontrolled obstetric hemorrhage. Evidence regarding the outcomes of planned versus urgent obstetric hysterectomy in contemporary tertiary practice in Greece remains limited. We compared maternal and neonatal outcomes between elective and urgent procedures at a tertiary referral center in Thessaloniki. Methods: We retrospectively reviewed 30 women who underwent obstetric hysterectomy between January 2020 and June 2025. The elective group included women with prenatal PAS diagnosis and planne

Moderate factor XI deficiency and neuraxial procedures in an obstetric cohort: a retrospective study (2014-2021).

BACKGROUND: Factor XI (FXI) deficiency is a rare inherited bleeding disorder potentially associated with an increased risk of bleeding, particularly in traumatic or surgical situations. The correlation between FXI levels and bleeding risk is weak, leading to ongoing debate regarding the FXI threshold for authorizing neuraxial procedures in obstetric patients. We report data on a cohort of pregnant women with FXI deficiency. METHODS: This retrospective study included all pregnant women admitted to two high-risk obstetric departments between 2014 and 2021, with FXI levels ≤0.50 IU/mL from the s

The reconstructive strategy for improving elbow function in late obstetric brachial plexus palsy.

Children with previously untreated obstetric brachial plexus palsy frequently have abnormal elbow function because of motor recovery with aberrant reinnervation, or because of paresis or paralysis. From 1988 to 1997 (9-year period), 62 children with obstetric brachial plexus palsy with resulting elbow deformity underwent various methods of palliative reconstruction to improve elbow function. For motor recovery with aberrant reinnervation, release of aberrantly reinnervated antagonistic muscles and augmentation of paretic muscles form the basis of surgical intervention. The surgical procedures

Fertility and Obstetric Outcomes in Asherman Syndrome: Assessing the Impact of Hyaluronic Acid Post-Hysteroscopic Adhesiolysis.

BACKGROUND: Installation of hyaluronic acid following hysteroscopic adhesiolysis is suggested to reduce intrauterine adhesion (IUA) reformation in women with Asherman's Syndrome (AS). We aimed to assess the impact of hyaluronic acid following hysteroscopic adhesiolysis on menstrual, surgical, fertility, obstetric and neonatal outcomes. METHODS: We identified 128 women treated hysteroscopically for AS over an 11-year period and invited them to participate in a questionnaire-based study. A total of 43 women were included; 21 treated surgically with hyaluronic acid administered immediately follo

Laparoscopic transabdominal cervical cerclage: a 6-year experience.

BACKGROUND: Cervical cerclage has been used as a treatment for cervical insufficiency for over 60 years. Transabdominal cerclage is indicated for cervical insufficiency not amenable to a transvaginal procedure, or following previous failed vaginal cerclage. A laparoscopic approach to abdominal cerclage offers the potential to reduce the morbidity associated with laparotomy. AIMS: To evaluate the obstetric outcome and surgical morbidity of laparoscopic transabdominal cerclage. METHODS: An observational study of consecutive women undergoing laparoscopic transabdominal cerclage from 2007 to 201

Preoperative Antibiotic Prophylaxis in Elective Minor Surgical Procedures among Adults in Southern Italy.

Little is known regarding the factors associated with surgical antibiotic prophylaxis (SAP) compliance in elective minor surgery. The purposes of this cross-sectional study were to identify the frequency of inappropriate SAP administration and to understand the characteristics associated with such inappropriateness in a sample of elective minor surgical procedures. The study was performed between May and July 2019 among a random sample of patients aged 18 years and older in seven public hospitals randomly selected in the Campania and Calabria Regions of Italy. Globally, only 45% of SAP approac
