Long-acting Release Octreotide for Pediatric Upper Gastrointestinal Bleeding.

INTRODUCTION: Upper gastrointestinal bleeding (UGIB) secondary to portal hypertension (PHT), without endoscopic or surgical treatment options due to an ectopic or unidentified bleeding site or the patient's anatomic characteristics, is challenging in pediatric hepatology. The usual treatment in these cases includes intravenous Octreotide. Recently, the availability of long-acting release Octreo tide (OCT-LAR) for monthly intramuscular administration has become an interesting therapeutic alternative. OBJECTIVE: To report the case of an infant with UGIB due to PHT who was successfully treated w

Chylous Ascites and Pleural Effusion Treated With Intravenous Octreotide.

Chylous ascites (CA) is uncommon in cirrhosis. It often presents as diuretic-resistant ascites and is associated with increased mortality. Diagnosis is done by the detection of triglyceride-rich ascitic fluid. There are no published guidelines on the management of CA. We describe the case of a middle-aged female who presented with CA secondary to cirrhosis, and the challenges associated with her treatment and her management with the use of intravenous octreotide.

Octreotide-related exocrine pancreatic insufficiency (EPI) in congenital hyperinsulinism.

Background Congenital hyperinsulinism (CH) is the most frequent cause of persistent hypoglycemia in the newborn. Octreotide, a long-acting somatostatin receptor analog (SSRA), is a second line treatment for diazoxide unresponsive CH patients. Although it has been found to be a safe and effective treatment, long-term benefits and side effects, have not been thoroughly evaluated. Case presentation Some authors have indicated that exocrine pancreatic insufficiency (EPI) is a common but under-recognized adverse reaction in adults treated with octreotide. However, no pediatric patient with SSRA-ind

Congenital neonatal chylothorax with hydrops fetalis treated with octreotide.

Congenital idiopathic chylothorax is the most common form of pleural effusion in neonates. It results from collection of lymphatic fluid in the pleural space secondary to leakage from the thoracic duct or one of its main tributaries. Chylothorax complicated by hydrops fetalis in a premature or term infant is an uncommon, serious clinical problem. We report on two cases with congenital chylothorax both treated with octreotide. One of the cases responded well to octreotide treatment, while the other did not. We conclude that octreotide should be considered in the treatment of neonatal chylothora

Octreotide ameliorates gastric lesions in chronically mild stressed rats.

AIM: To evaluate the effect of chronic mild stress (CMS) on the emergence of gastric ulcers and possible modulation by octreotide, a synthetic somatostatin analogue. METHODS: Adult male Wistar rats were subjected to nine different unpredictable random stress procedures for 21 d, a multifactorial interactional animal model for CMS. Octreotide was administered daily for 21 d at two dose levels (50 and 90 μg/kg) before exposure to stress procedure. Macro- and microscopical assessments were made, in addition to quantification of plasma corticosterone and gastric mucosal inflammatory, oxidative st

[Management of postoperative pancreatic fistula resistant to octreotide therapy].

Pancreatic fistula is a rare postoperative complication, usually occurring after pancreatic surgery. Majority of them heal spontaneously, some patients require somatostatin/octreotide treatment. The authors have presented 11 patients with postoperative pancreatic fistula, in whom octreotide therapy in dose of 0.1 mg t.i.d./10 days has been ineffective. The causes of pancreatic fistula have been as follows: necrosectomy of the infected pancreatic necrosis--5 patients, distal pancreatic resection--2 patients, insulinoma enucleation--2 patients, gastrectomy with partial pancreatectomy--2 patients

Octreotide inhibits pancreatic exocrine secretion and prevents pancreatoenterostomy leakage.

The effects of octreotide, a synthetic somatostatin analogue, were examined in 43 Japanese patients with pylorus-preserving pancreatoduodenectomy. The 43 patients were divided into two groups: 13 patients to whom a daily dose of 100 microg of octreotide was administered continuously and subcutaneously for the first 10 postoperative days (octreotide group); and 30 patients to whom octreotide was not administered (control group). Within 10 postoperative days, pancreatoenterostomy leakage was evident in one (7.7%) of the 13 patients in the octreotide group, but in five (16.7%) of the 30 patients

Octreotide and bleeding oesophageal varices: new indication. Nothing new.

(1) Octreotide, a synthetic somatostatin analogue, is now also indicated for emergency treatment of ruptured oesophageal varices pending endoscopic treatment. (2) The assessment file in this indication is limited. (3) The two available comparative trials show no difference in efficacy compared with terlipressin, but their statistical power was low. Octreotide has not been compared with somatostatin, the efficacy of which is not clearly established either. (4) A placebo-controlled trial, published only as an abstract, showed no specific activity of octreotide on bleeding oesophageal varices. (5

Somatostatin receptor imaging in CNS tumours using 111In-octreotide.

This study evaluates the in vivo visualization of somatostatin (SS) receptors in central nervous system (CNS) tumours using 111In-octreotide imaging and discusses the clinical implications. Ninety-five patients with histologically confirmed diagnosis of CNS tumours were imaged 2-4 and 24 h after the intravenous injection of 111-185 MBq of 111In-octreotide. An uptake index was computed using tumour/non-tumour ratios evaluated using a standard region-of-interest method. Semi-quantitative immunohistochemical studies of SS binding sites were performed on frozen tumour sections. All meningiomas, mo

Insulin sensitivity test using a somatostatin analogue, octreotide (Sandostatin).

To assess the validity of the steady-state plasma glucose (SSPG) method using octreotide (Sandostatin), we compared the SSPG method with the euglycemic hyperinsulinemic clamp technique in ten non-obese, insulin-dependent diabetic patients. The SSPG method was performed by intravenous infusion of octreotide (0.5 microgram/min), insulin (2 mU/kg/min) and glucose (9 mg/kg/min) for 180 min. Octreotide suppressed endogenous growth hormone, glucagon and insulin secretion. A steady state condition was reached by 90 min, and the mean value of SSPG at 150 and 180 min was used as the index of insulin se

Pancreatic cancer presenting with resolving pseudocyst during octreotide therapy.

Pancreatic cancer presenting as acute pancreatitis is relatively uncommon. Pancreatic cancer should be included in the differential diagnosis of "idiopathic" acute pancreatitis, particularly in the elderly. The following case report describes a patient in whom pancreatic cancer presented as acute pancreatitis with pseudocyst formation and subsequent resolution with octreotide therapy. Various implications of this case are reviewed.

Unveiling the therapeutic potential of octreotide in treating morphine dependence.

The potential of octreotide (OCT), an octapeptide synthetic somatostatin agonist, to mitigate addiction, specifically morphine dependence, was evaluated in a comprehensive approach involving behavioral, molecular, histological, and docking studies. OCT decreased addiction-related behaviors in Wistar rats previously exposed to morphine, normalizing their exploratory activities and reducing drug-seeking behaviors. Histological assessments revealed octreotide-induced reductions in opioid-induced neuronal damage, suggesting a neuroprotective function. Octreotide's behavioral and histopathological

Drugs in Focus: Octreotide Use in Children With Gastrointestinal Disorders.

Octreotide, a somatostatin analogue, has been used for more than 20 years in children with gastrointestinal bleeding, chylothorax or chylous ascites, intestinal lymphangiectasia, pancreatitis, intestinal dysmotility, and severe diarrhoea; however, until now, there is a lack of randomised clinical trials evaluating the efficacy of this compound in childhood. Hence, we aimed to review the literature in order to determine the evidence of its use and safety in children, using PubMed from 2000 to 2021 with the search terms "octreotide" and "children" and "bleeding or chylous ascites or chylothorax

Octreotide use for rescue of vision in a pregnant patient with acromegaly.

Pregnancy in acromegaly is rare and generally safe, but tumour expansion may occur. Managing tumour expansion during pregnancy is complex, due to the potential complications of surgery and side effects of anti-tumoural medication. A 32-year-old woman was diagnosed with acromegaly at 11-week gestation. She had a large macroadenoma invading the suprasellar cistern. She developed bitemporal hemianopia at 20-week gestation. She declined surgery and was commenced on 100 µg subcutaneous octreotide tds, with normalisation of her visual fields after 2 weeks of therapy. She had a further deterioration

The effect of Resveratrol and Octreotide on peritoneal adhesions in a rat model.

INTRODUCTION: The aim of this study was to investigate the efficacy of resveratrol and octreotide, agents that are used to prevent intra-abdominal adhesions in experimental models, in preventing intraperitoneal adhesions when used alone or in combination. MATERIALS AND METHODS: The study employed 28 young female Wistar albino rats weighing 250-300 grams. An experimental adhesion model was created in each rat using serosal abrasion and peritoneal excision. They were divided into four groups, each comprising seven rats: Group 1, adhesion induction only; Group 2, resveratrol administration only;

(111)In-octreotide SPECT-CT in head and neck paragangliomas.

Head and neck paragangliomas are rare neuroendocrine tumors expressing somatostatin receptors on their cell surface, particularly subtype 2. Due to this distinctive feature, images can be obtained in Nuclear Medicine using synthetic analogues of somatostatin, mainly octreotide, which allow selective display by planar scintigraphy, SPECT-CT or PET-CT imaging after radiolabeling with (111)In or (68)Ga. Three cases have been selected and presented from a series of patients that illustrate the utility of SPECT-CT studies with (111)In-octreotide in the diagnosis and monitoring of this type of tumor
