Endoscopic endonasal anatomy of the ophthalmic artery in the optic canal.

BACKGROUND: The endoscopic endonasal opening of the optic canal has been recently proposed for tumors with medial invasion of this canal, such as tuberculum sellae meningiomas. Injury of the ophthalmic artery represents a dramatic risk during this maneuver. Therefore, the aim of this study was to analyze the endoscopic endonasal anatomy of the precanalicular and canalicular portion of this vessel, discussing its clinical implication. METHODS: The course of the ophthalmic artery was analyzed through five endoscopic endonasal dissections, and 40 nonpathological consecutive MRAs were reviewed.

Monocular elevator paresis in neurofibromatosis type 2.

A retrospective review of 29 consecutive unselected patients referred for neuro-ophthalmic evaluation after the diagnosis of neurofibromatosis type 2 (NF2) showed that four of them had a monocular elevator paresis. In two of the four MRI demonstrated lesions, presumed to be schwannomas, of the third nerve. These findings indicate that monocular elevator paresis is a common neuro-ophthalmic finding in NF2, which the authors suspect is probably a sign of third nerve infiltration or compression by a schwannoma.

Prevalence and impact of neuro-ophthalmic multimorbidity on patient disposition and outcomes.

PURPOSE: As neuro-ophthalmologists often deal with uncommon disorders, the prevalence of comorbid neuro-ophthalmic diseases is unknown. This study sought to characterize the prevalence of patients diagnosed with at least two separate neuro-ophthalmologic disorders and the impact on patient disposition and outcome as of identifying the secondary diagnosis. METHODS: Retrospective chart review at two university-affiliated neuro-ophthalmic practices between July-December 2023. Sociodemographic and clinical characteristics were compared between patients with and without neuro-ophthalmic comorbidit

A previously undescribed branch of the pterygopalatine ganglion.

BACKGROUND: Endonasal and infrazygomatic pterygopalatine ganglion (PPG) block for facial pain provides pain relief in a broader area than expected on anatomic grounds. The aim of this study was to search for neural structures in the pterygopalatine fossa (PPF) that could explain unexpected pain relief after PPG blockage. METHODS: The neural PPF content was explored through human cadaver study and nerve-specific staining. Five human PPF specimens were dissected as whole-mount preparations with the aid of an operation microscope and stained for acetylcholinesterase. One of these specimens was p

Compressive Optic Neuropathy Due to Dolichoectatic Ophthalmic Artery.

A 63-year-old man experienced progressive vision loss in his right eye over five years. Ophthalmologic examination showed right optic neuropathy with optic nerve pallor. Prior MRI findings of gadolinium enhancement near the prechiasmatic optic nerve had suggested acute optic neuritis. A repeat MRI revealed a dilated vascular structure compressing the optic nerve, confirmed by Time-of-Flight MRA as dolichoectatic ophthalmic artery. Compression-related optic neuropathy caused by a dolichoectatic ophthalmic artery can have similar appearance to acute optic neuritis on MRI. When the clinical histo

Neuro-ophthalmic sarcoidosis.

Neuro-ophthalmic disease occurs in about a third of patients with neurosarcoidosis. Optic nerve involvement is the most common manifestation, but other cranial nerves and the optic chiasm can be involved. However, there are several other common diseases that cause optic neuropathy, including multiple sclerosis. The diagnosis of sarcoidosis can often be made based on the multi-organ nature of the disease and ancillary testing. Most patients with neuro-ophthalmic sarcoidosis require systemic therapy. While corticosteroids are usually the first step in therapy, cytotoxic agents such as methotrexa

Somatotopic organization of trigeminal ganglion: three-dimensional reconstruction of three divisions.

Clearing the somatotopic organization of trigeminal ganglion can help us to improve the precision of treatment for trigeminal neuralgia. The distribution of primary afferent perikarya of 3 branches of trigeminal nerve in the trigeminal ganglion was investigated in the rabbit, and 3D model was reconstructed then. After application of wheat germ agglutinin-horseradish peroxidase and DiI to the cut endings of the 3 branches of trigeminal nerve, ophthalmic cells were found in the anteromedial part of the trigeminal ganglion, mandibular cells in the posterolateral part, and maxillary cells in the m

[Horner's syndrome secondary to ophthalmic herpes zoster].

INTRODUCTION: Ophthalmoparesias is a frequent complication of ophthalmic herpes zoster. It occurs in 31% of all cases. However, the presence of Horner's syndrome during viral reactivation is a rarity which has only been previously described on two occasions, and never associated with cranial nerve involvement. CLINICAL CASE: We describe a patient with the first case of Horner's syndrome secondary to ophthalmic herpes zoster, with simultaneous, homolateral lesions of the third and sixth cranial nerves. Clinical evaluation, the course of the disorder, negative magnetic resonance studies and tes

Ophthalmic herpes zoster with contralateral hemiparesis: a case report.

Herpes zoster of the ophthalmic division of the left fifth cranial nerve with contralateral hemiparesis was observed in a 30-year-old man. Left carotid angiography showed segmental constrictions consistent with cerebral arteritis, possibly provoked by direct viral infection along the intracranial part of the ophthalmic nerve. An ischaemic lesion revealed by computed tomographic scan was considered secondary to arteritis and responsible for the hemiparesis. The presence of an immune response within the blood-CSF barrier was suggested by an increase of oligoclonal CSF IgG and IgA.

Corneal Confocal Microscopy Demonstrates Corneal Nerve Loss in Patients With Trigeminal Neuralgia.

Background: The diagnosis of trigeminal neuralgia (TN) is challenging due to the lack of objective diagnostics. Corneal confocal microscopy (CCM) is a non-invasive ophthalmic imaging technique, which allows quantification of corneal nerve fibers arising from the trigeminal ganglion and may allow the assessment of neurodegeneration in TN. Methods: CCM was undertaken in 11 patients with TN and 11 age-matched healthy controls. Corneal nerve fiber density (CNFD), corneal nerve branch density, corneal nerve fiber length (CNFL), corneal nerve fiber width, corneal nerve fiber area, and dendritic cell

Precisional modulation of translaminar pressure gradients for ophthalmic diseases.

The translaminar pressure gradient (TLPG) refers to two forces at the lamina cribosa of the optic nerve: the anteriorly acting intracranial pressure (ICP), and posteriorly-acting intraocular pressure (IOP). It has been proposed that controlling the translaminar pressure gradient at regular intervals may preserve the optic nerve and slow the course of glaucoma. The precisional modulation of this TLPG is a recently introduced concept that may play a role in the treatment of ophthalmic diseases such as glaucoma. In this manuscript, we review the applications of pressurized goggles on ophthalmic d

Utility of Optical Coherence Tomographic Angiography in Neuro-Ophthalmic Practice.

BACKGROUND: Noninvasive ophthalmic imaging and quantification of the optic nerve and retina using near-infrared optical coherence tomography (OCT) have become an integral part of neuro-ophthalmic practice. Advances in OCT have allowed 3D mapping of the optic nerve and retinal microvascular using OCT angiography (OCTA) at micrometer precision. In this review, we discuss the utility of OCTA in neuro-ophthalmic and neurologic conditions with particular focus on current applications, needed progress, and integration into neuro-ophthalmic practice. Recurrent ophthalmic arteries supplying posterior meningiomas: embryologic background and 3D anatomical visualization.

PURPOSE: Recurrent branches of the ophthalmic artery rarely function as feeding arteries to posteriorly located meningiomas. This study describes two such cases and examines the embryologic basis and collateral pathways that enable this atypical supply. The utility of three-dimensional (3D) simulation for delineating skull base vascular relationships is also assessed. METHODS: Two patients with posteriorly located meningiomas supplied by recurrent ophthalmic artery branches underwent detailed angiographic evaluation. The embryogenesis of the superficial recurrent ophthalmic artery (SROA) and

Neural connections in and around the cavernous sinus in rat, with special reference to cerebrovascular innervation.

There is a confluence in and around the cavernous sinus of neural pathways innervating the intracranial structures. To determine the patterns of innervation, particularly of the cerebral arteries, we stained whole-mount preparations of the cavernous sinus and adjacent regions of the rat for acetylcholinesterase. The cavernous nerve plexus, with several small ganglia, mainly occupied the lateral wall of the sinus and extended laterally above the ophthalmic and maxillary divisions of the trigeminal nerve, in relation to the oculomotor and trochlear nerves. The cavernous plexus was connected to t

Pituitary apoplexy presenting atypical time course of ophthalmic symptoms.

Pituitary apoplexy is defined as a sudden loss of blood supply to the pituitary gland, leading to tissue necrosis and hemorrhage. Its clinical symptoms are characterized by sudden onset of headache, nausea, vomiting, ophthalmic symptoms and hormonal dysfunction. A 65-year-old woman presented with left-sided ptosis and blurred vision. These ophthalmic symptoms gradually worsened for one month without headache, visual acuity and field deficit. Neuro-ophthalmic examination revealed left oculomotor nerve palsy. Magnetic resonance imaging (MRI) revealed a round mass lesion in the left cavernous sin
