Human factors in the clinical implementation of deep learning-based automated contouring of pelvic organs at risk for MRI-guided radiotherapy.

PURPOSE: Deep neural nets have revolutionized the science of auto-segmentation and present great promise for treatment planning automation. However, little data exists regarding clinical implementation and human factors. We evaluated the performance and clinical implementation of a novel deep learning-based auto-contouring workflow for 0.35T magnetic resonance imaging (MRI)-guided pelvic radiotherapy, focusing on automation bias and objective measures of workflow savings. METHODS: An auto-contouring model was developed using a UNet-derived architecture for the femoral heads, bladder, and rect

MFFUNet: A hybrid model with cross-attention-guided multi-feature fusion for automated segmentation of organs at risk in cervical cancer brachytherapy.

Brachytherapy is a common treatment option for cervical cancer. An important step involved in brachytherapy is the delineation of organs at risk (OARs) based on computed tomography (CT) images. Automating OARs segmentation in brachytherapy has the benefit of both reducing the time and improving the quality of radiation therapy planning. This paper introduces a novel segmentation model named MFFUNet for the automatic contour delineation of OARs in cervical cancer brachytherapy. The proposed model employs a staged encoder-decoder structure, integrating the self-attention mechanism of Transformer

Shoot-through proton FLASH irradiation lowers linear energy transfer in organs at risk for neurological tumors and is robust against density variations.

Objective. The goal of the study was to test the hypothesis that shoot-through FLASH proton beams would lead to lower dose-averaged LET (LETD) values in critical organs, while providing at least equal normal tissue sparing as clinical proton therapy plans.Approach. For five neurological tumor patients, pencil beam scanning (PBS) shoot-through plans were made, using the maximum energy of 227 MeV and assuming a hypothetical FLASH protective factor (FPF) of 1.5. The effect of different FPF ranging from 1.2 to 1.8 on the clinical goals were also considered. LETDwas calculated for the clinical plan

Dosimetric comparison of deformable image registration and synthetic CT generation based on CBCT images for organs at risk in cervical cancer radiotherapy.

OBJECTIVE: Anatomical variations existing in cervical cancer radiotherapy treatment can be monitored by cone-beam computed tomography (CBCT) images. Deformable image registration (DIR) from planning CT (pCT) to CBCT images and synthetic CT (sCT) image generation based on CBCT are two methods for improving the quality of CBCT images. This study aims to compare the accuracy of these two approaches geometrically and dosimetrically in cervical cancer radiotherapy. METHODS: In this study, 40 paired pCT-CBCT images were collected to evaluate the accuracy of DIR and sCT generation. The DIR method wa

Assessment of Radiation Doses Delivered to Organs at Risk Among Patients With Early-Stage Favorable Hodgkin Lymphoma Treated With Contemporary Radiation Therapy.

IMPORTANCE: Response-adapted randomized trials have used positron emission tomography-computed tomography to attempt to identify patients with early-stage favorable Hodgkin lymphoma (ESFHL) who could be treated with doxorubicin, bleomycin, vinblastine, and dacarbazine (ABVD) without radiation therapy (RT). While maximal efficacy is demonstrated with combined modality therapy, RT is often omitted in fear of late adverse effects; however, the application of modern RT could limit these toxic effects. OBJECTIVE: To determine the radiation doses delivered to organs at risk with modern involved-sit

Study of the dose-volume parameters variation in tumor target volumes and organs at risk during nasopharyngeal carcinoma radiotherapy applying deformation registration.

BACKGROUND: During intensity-modulated radiotherapy (IMRT) for nasopharyngeal carcinoma (NPC), the volume of the target volume and the organs at risk (OARs) will change constantly, which may lead to differences between the actual dose received and the initial planned dose. In this study, the cumulative dose of the two plans was obtained by deformable registration. This study provides an approach to evaluate the dose volume of IMRT for the NPC objective. METHODS: From July 2014 to May 2018, eighteen NPC patients who accepted simultaneous integrated boost IMRT were enrolled. All patients underw

Artificial ascites for organs at risk sparing in intrapelvic brachytherapy: a case report of recurrent uterine cervical carcinoma adjacent to the bowel.

Artificial ascites has been reported as an effective technique to reduce the risk of thermal injury in radiofrequency ablation of liver tumors by increasing the distance of collateral organs located next to the ablated sites. In this case report we share our experience with artificial ascites in an attempt to reduce the toxicity of collateral adjacent organs in the setting of re-irradiation for recurrent cervical cancer. A 52-year-old female who developed local recurrence after definitive radiation therapy was treated with interstitial re-irradiation by means of image-guided, (single-implant/m
