Nasolacrimal duct obstruction after maxillary orthognathic surgery.

PURPOSE: To report cases of nasolacrimal duct obstruction (NLDO) after maxillary orthognathic surgery. MATERIALS AND METHODS: The authors reviewed the clinical manifestations, dacryocystographic images, and orbital computed tomographic scans of 10 patients who were diagnosed with NLDO after undergoing maxillary orthognathic surgery. RESULTS: Six of the 10 patients (60%) complained of epiphora immediately after the surgery. Bilateral (n = 2, 20%) or unilateral (n = 8, 80%) NLDO occurred in all patients involved in the study. Twelve eyes of 10 patients were examined, and dacryocystography show

Submental intubation in orthognathic surgery: initial experience.

Submental intubation is a technique for use in maxillofacial trauma, which allows precise assessment of changes to the nasolabial complex, midlines, cants, and incisal display in patients having maxillary orthognathic surgery. We report our initial experience of it used as an adjunct in the management of orthognathic surgery in a series of 44 patients.

Orthognathic surgery in juvenile rheumatoid arthritis patients.

BACKGROUND: Mandible advancement osteotomy in juvenile rheumatoid arthritis patients for class II anterior open-bite malocclusion is controversial; however, there is no literature discussing outcomes of surgery in this group of patients. An evaluation of such outcomes was undertaken at The Hospital for Sick Children. METHODS: A retrospective chart review revealed that eight patients with juvenile arthritis underwent orthognathic surgery during a period of 5 years. All of the patients had a bilateral sagittal split osteotomy of the mandible, six had a simultaneous Le Fort I osteotomy, and four

Three-dimensional model simulation in orthognathic surgery.

Surgery simulation with a three-dimensional model constructed with stereolithography from digital image data has proven to be useful in the maxillofacial area. The simulation and surgery of Le Fort I and bilateral sagittal split osteotomies were carried out in two patients. The models replicating the facial skeleton with precise internal anatomy facilitated design of the osteotomy and preparation of osteosynthesis. Each sectioned segment of the maxilla and mandible was accurately repositioned by transferring the positional relationships of multiple reference points on the model to the bone sur

Soft tissue cephalometric analysis for orthognathic surgery.

A soft tissue cephalometric analysis designed for the patient who requires surgical-orthodontic car was developed to complement a previously reported dentoskeletal analysis. To make it clinically practical, the analysis has been reduced to its most relevant and significant measurements. Used along with other diagnostic aids, this soft tissue evaluation will enable the clinician to achieve good facial esthetics for his or her patients.

Benefits of the enhanced recovery after surgery pathway for orthognathic surgery.

The enhanced recovery after surgery (ERAS) protocol was designed to improve patient outcomes and decrease complications, opioid use, and postoperative nausea and vomiting (PONV). The aim of this retrospective cohort study was to examine the effectiveness of ERAS protocols implemented in orthognathic surgeries from 2017 to 2018 at the University of Alabama at Birmingham Hospital by measuring opioid use and PONV. Two groups were identified through chart review, a non-ERAS group (traditional) of patients who had surgery without a protocol and an ERAS group of patients who had surgery with the ERA

Prediction of surgery-first approach orthognathic surgery using deep learning models.

The surgery-first approach (SFA) orthognathic surgery can be beneficial due to reduced overall treatment time and earlier profile improvement. The objective of this study was to utilize deep learning to predict the treatment modality of SFA or the orthodontics-first approach (OFA) in orthognathic surgery patients and assess its clinical accuracy. A supervised deep learning model using three convolutional neural networks (CNNs) was trained based on lateral cephalograms and occlusal views of 3D dental model scans from 228 skeletal Class III malocclusion patients (114 treated by SFA and 114 by OF

Orthognathic surgery in COVID-19 times, is it safe?

COVID-19 has impacted the provision of orthognathic surgery globally. Uncertainty around its effects and transmission in aerosol generating procedures (AGP's) has led to disagreement within maxillofacial surgeons into the safety of orthognathic surgery during the pandemic. We present a local case series of orthognathic surgery undertaken during the COVID-19 pandemic. To our knowledge no such similar study has been reported worldwide. Data was collected from the 1st June to 30th November 2020 for all patients undergoing orthognathic surgery by a single consultant. All procedures and inpatient s

Current concept of the surgery-first orthognathic approach.

Traditional orthognathic surgery has long been performed after presurgical orthodontic treatment. Despite some concerns, the surgery-first orthognathic approach (SFOA) or surgery-first approach (SFA) without presurgical orthodontic treatment has gradually gained popularity. In recent years, several articles dealing with the concepts of the SFA have been published worldwide. However, the SFA has not yet been standardized, and many surgeons use slightly different protocols and concepts. This review article discusses the beginning and evolution of the SFA and its current concepts, including some

Assent for Orthognathic Surgery: Features, Challenges, and Benefits.

The ideal age to undergo orthognathic surgery approximates the age of majority, and as a result a considerable portion of the patient population undergoing orthognathic surgery are legal minors. When a patient cannot legally provide consent, assent is often sought. Assent for surgical orthodontics is complicated by particular changes in cognitive variables during adolescence, the multiphased and multi-provider treatment course, and the intervention's elective nature. Ultimately, inclusion of a high-quality assent process can help identify patients most likely to benefit from orthognathic surge

Treatment of the Patient With Crouzon Syndrome With Orthognathic Surgery.

Crouzon syndrome is a genetic disease that is transferred by autosomal dominant inheritance. Patients with this syndrome have craniosynostosis, hypertelorism, orbital proptosis, mid-face hypoplasia, mandibular prognathism, and concave facial profile. A number of surgical treatments are used to improve facial contours, and to correct dental and maxillary relationships. Orthognathic surgery is routinely used in the treatment of these patients. We present a female patient with Crouzon syndrome treated with Le Fort I and bilateral sagittal split osteotomy. Previously, surgically assisted rapid max

Orthognathic Surgery for Patients with Cleft Lip and Palate.

Cleft orthognathic surgery is an important component of a comprehensive cleft care plan. Applying combined orthodontic and orthognathic treatment principles to a cohort of patients with cleft lip and palate raises many challenges not encountered in conventional orthognathic care. Cleft patients share a commonality in their midfacial anatomy that is characterized by a 3-dimensionally deficient maxilla. The residual sequelae of multiple previous surgeries along with dental differences and unhealed fistulae are considerations when embarking on treatment. This article describes many of these chall

Simultaneous Glossectomy with Orthognathic Surgery for Mandibular Prognathism.

Macroglossia can create dental and skeletal instability after orthodontic treatment or orthognathic surgery for mandibular prognathism. In relevant literature, partial glossectomy is suggested for a good post-treatment prognosis. Most of the published partial glossectomy cases are two-staged surgery, because of concern about postoperative airway obstruction. As orthognathic surgical techniques and fixation method develop, however, concerns about postoperative airway obstruction have lessened. In this case, mandibular setback surgery and partial glossectomy were performed simultaneously, leadin
