Various lesions can cause conductive hearing loss in a patient with a normal tympanic membrane. These include congenital ossicular anomaly, otosclerosis, and congenital or acquired ossicular fixation and discontinuity. We had an experience with a patient who presented with a conductive hearing loss in both ears, in which small pieces of the long process of the incus were absent and had been replaced with fibrous tissues in both ears. No other abnormalities, such as postinflammatory changes or fixation of the ossicles, were found. Because the long process of the incus undergoes remodeling throu

OBJECTIVE: To describe and compare 2 different surgical techniques used in treatment of malleus fixation syndrome.
STUDY DESIGN: A retrospective study including 24 patients (25 ears). In 11 of them, the attical fixation was removed via a transmastoid approach without disruption of the ossicular chain. In 14 patients, an ossiculoplasty was performed via a transcanal approach; the malleus head was removed with the incus, and the ossicular chain was reconstructed either by incus interposition (9 cases) or by using a partial ossicular replacement prosthesis (5 cases). The hearing results were ana

Erosive middle ear disease (EMED) involves a spectrum of otologic conditions that involve chronic inflammatory processes, including chronic otitis media with or without cholesteatoma, that contribute to osseous erosion of the middle ear and ossicular chain. Treatment of EMED involves the eradication of the disease followed by ossicular chain reconstruction to optimize hearing outcomes. There are many challenges regarding ossiculoplasty in EMED, including the potential for microscopic residual disease, variability of ossicular chain status following disease eradication, and concern for recurren

OBJECTIVE: Compare hearing benefit of incus preservation in primary cholesteatoma surgery versus cartilage-myringostapediopexy.
METHODS: Prospective cohort study in a tertiary referral center. Tympanoplasty utilizing cartilage or other grafts, with or without intact incus was performed in 195 ears (187 children) with intact stapes. Outcome measures were pre and post-operative four-tone air conduction (AC) threshold (0.5, 1, 2, 4 kHz) and proportion with normal hearing (AC ≤ 30 dB HL) at 12 months.
RESULTS: Ears with intact ossicles had better post-operative AC thresholds than those with incu

CONCLUSION: After initial contact of the active middle ear implant (AMEI) on the incus, significant increases in device performance can be achieved intraoperatively without affecting residual hearing by additional static loading of the incus with 62 μm (quarter turn) to 125 μm (half turn) increments via an adjustment screw.
OBJECTIVES: To assess the performance gains of driving the incus with an AMEI under increasing static loads in cadaveric temporal bones.
METHODS: Incus drive efficacy was assessed using laser Doppler velocimetry measurements of stapes velocities over a frequency range of

Temporalis fascia remains the most widespread material for reconstructing the tympanic membrane in tympanomastoid surgery. If total or partial ossicular replacement prostheses are needed or pathology of the eustachian tube causes ventilation impairment, a more rigid transplant material is required. Between 1989 and 1994 perichondrium-cartilage composite grafts were used in a series of 597 cases of tympanomastoid surgery performed at the University ENT Clinic of Würzburg. The graft was taken from the tragus and prepared as a cartilage island with perichondrium attached to one side, the perichon

OBJECTIVE: The results of a recently published micro-CT study suggested a correlation of the distance between long incus process and stapes footplate and the required prosthesis length in malleostapedotomy. The goal of this study was to test the reliability of that assumption.
METHODS: Rectangular and bent prostheses were tested in 11 cadaveric human temporal bone specimens; 1 of them showed a stapedial artery. Prosthesis length was calculated based on the distance between long incus process and stapes footplate. The rate of acceptable prosthesis insertion into the vestibule was investigated.

Successful replacement of the sound conducting mechanism of the ear has been hampered by infection, perforation, prosthesis instability and extrusion. In this animal study on thirty rats a silicone prosthesis was inserted between two layers of fascia as a total replacement of the tympanic membrane and ossicular chain. After four months the graft was found to be intact without evidence of infection or extrusion in all but one of the 30 rats. These results justify a trial of a similar silicone prosthesis in humans.

Titanium middle ear (ME) prostheses are widely used in surgical practice due to their acoustic properties. However, they present a significant drawback shared by all synthetic materials currently in use for ME reconstruction: they can be rejected by the organism of the host. In this study, we aim to review the current literature on titanium partial ossicular replacement prostheses (PORPs) and total ossicular replacement prostheses (TORPs) extrusion and dislocation. Eighty articles were analysed after a full article review based on the inclusion and exclusion criteria. The most common indicatio

In order to improve the hearing results of tympanoplasty, we performed the operation on 113 cases (124 ears) by using ossicular replacement prostheses made of Titanium. The results demonstrated that among all the operated ears, 73/124 (58.9%) had obtained a narrow gap (< 20 dB HL) between BC and AC, 89/124 (71.8%) had regained socially acceptable hearing ability. The total effective rate of hearing gain (> 10 dB HL) was 115/124 (92.7%). No reject reaction and recurrent cholesteatoma were found. This operation method, based on animal experiments and applied anatomic study of human suprastapedia

We report the first case of unilateral incus agenesis in the literature. Preoperative diagnosis by virtual endoscopy further enhances the significance of this case. We present thin-section temporal bone computed tomographic images and 3-dimensional reconstructed views which were all correlated with intraoperative findings. The patient was successfully treated with an ossicular prosthesis replacement.

A Plastipore (porous polyethylene) Total Ossicular Replacement Prosthesis gave an excellent initial hearing result which was maintained for 14 years. Hearing then began to deteriorate and revision surgery showed disintegration of the prosthesis and a defect in the stapes footplate. Histological examination confirmed previous findings in porous polyethylene with multinucleated foreign body giant cells and breakdown of the material.

The round window vibroplasty is a feasible option for the treatment of conductive, sensorineural and mixed hearing loss. Although clinical data suggest a satisfying clinical outcome with various coupling methods, the most efficient coupling technique of the floating mass transducer to the round window is still a matter of debate. For this, a soft silicone-made coupler has been developed recently that aims to ease and optimize the stimulation of the round window membrane of this middle ear implant. We performed a temporal bone study evaluating the performance of the soft coupler compared to the
