Study of Morphological Changes in the Inferior Turbinate After Le Fort I Osteotomy.

OBJECTIVE: There are cases in which patients complain of nasal obstruction after Le Fort I osteotomy, but the relationship with postoperative morphological changes in the nasal cavity, including the septum and inferior turbinate, is not clear. STUDY DESIGN: The authors evaluated the three-dimensional relationship of the morphological changes in the inferior turbinate before and after surgery in 84 patients who underwent Le Fort I osteotomy. Three classifications were made according to superior amount of maxillary movement at the base of nasal cavity. RESULTS: The high elevation group (4.0 mm

Factors Affecting Optic Nerve Damage in Le Fort III Osteotomy: A Retrospective Study.

The causes of visual impairment following Le Fort osteotomy for syndromic craniosynostosis have not been completely elucidated. The authors investigated the potential causes and means of prevention of optic nerve damage, with particular emphasis on intraoperative blood transfusion volume and operating time. This retrospective study evaluated patients who underwent Le Fort III osteotomy for syndromic craniosynostosis between 2000 and 2020. Data on pupillary reflex, pupil size, operating time, blood transfusion, age at time of surgery, sex, and syndrome type were obtained from medical records. U

A rare complication of hemolacria after Le fort I osteotomy: a case presentation.

BACKGROUND: Nasolacrimal duct obstruction (NDO) is a common pathology preventing the proper drainage of the tears, and its main symptom is epiphora. Secondary acquired nasolacrimal duct obstruction (SANDO) can be due to a variety of causes including infection, trauma, or neoplasms. It has been reported to occur with different forms of maxillofacial trauma, especially Le Fort II, Le Fort III, naso-orbital-ethmoidal, and orbital floor fractures. CASE PRESENTATION: A 20-year-old Egyptian female presented to correct a facial disharmony due to a cleft lip and palate defect. The patient reported a

Accelerated orthodontic tooth movement following le fort I osteotomy in a rodent model.

PURPOSE: In surgery-first accelerated orthognathic surgery, the clinical phenomenon of accelerated orthodontic tooth movement after osteotomy is a benefit compared with the conventional approach. However, because much of the literature on this phenomenon is based on empirical evidence and case reports, experimental animal-based studies are needed to verify and quantify this acceleration effect. The purpose of this prospective experimental study was to identify whether osteotomy procedures increase tooth movement. MATERIAL AND METHODS: Le Fort I osteotomies were performed on the left maxillas

Maxillary quadrangular Le Fort I osteotomy: long-term skeletal stability and clinical outcome.

The primary purpose of this study was to evaluate the long-term skeletal stability of the maxillary quadrangular Le Fort I osteotomy, and secondarily to determine patient overall experience and satisfaction with the surgical outcome. This retrospective cohort study evaluated a sample of patients with midface zygomatic-maxillary deficiency and Class III skeletal malocclusion. The primary outcome measure, on the basis of cephalometric analysis, was long-term vertical and horizontal skeletal stability in cleft and non-cleft patients, with and without interpositional autogenous iliac bone graft st

[Non-union of the maxilla: a rare complication after Le Fort I osteotomy].

INTRODUCTION: Maxillary non-union following Le Fort I osteotomy is a rare complication in orthognathic surgery. We report our experience and analyze the possible etiological factors. PATIENTS AND METHOD: We retrospectively analyzed the files of 150 patients having undergone Le Fort I osteotomy, between 1996 and 2006, screening for maxillary non-union. We documented patients' features of: sex, age, medical history, indication, orthodontics, osteotomy type(s) and displacement(s), osteosynthesis, quality of dental occlusion, orthodontics, clinical signs supporting a diagnosis of non-union, radio

Segmental Le Fort I osteotomy with bone grafting in unilateral severely atrophied maxilla.

This study describes the use of a segmental Le Fort I osteotomy for interpositional autogenous iliac bone grafts in 5 patients with severe unilateral atrophy of the maxilla. Unilateral pterygomaxillary disjunction and osteotomy of the lateral and medial walls of the maxillary sinus were performed. The mucosa of the maxillary sinus was preserved and lifted cranially. It was stabilized with autogenous cancellous bone grafts. One side of the maxilla was gently downfractured until it replaced the original morphology of the alveolar crest. The inter-arch space was therefore reduced to the ideal lev

Assessment of pterygomaxillary separation in Le Fort I Osteotomy in class III patients.

PURPOSE: To examine the separation of the pterygomaxillary region at the posterior nasal spine level after Le Fort I osteotomy in Class III patients. PATIENTS AND METHODS: The study group consisted of 37 Japanese patients with mandibular prognathism and asymmetry, with maxillary retrognathism or asymmetry. A total of 74 sides were examined. Le Fort I osteotomy was performed without a pterygoid osteotome, with an ultrasonic curette used to remove interference at the pterygomaxillary region. Postoperative computed tomography (CT) was analyzed for all patients. The separation of the pterygomaxil

Heparinization after Le Fort I osteotomy in a patient with chronic atrial fibrillation.

The middle-aged patient with a dentofacial deformity complicated by partial edentulism and underlying medical problems presents a challenge to the orthodontic-surgical team. The clinical management of a 51-year-old man with maxillary deficiency and chronic atrial fibrillation is presented. Preoperative evaluation and planning, and a scheme and rationale for perioperative anticoagulation, are crucial. The etiology and risks of chronic atrial fibrillation are reviewed along with anticoagulation therapy.

Le Fort III osteotomy to correct dish-face deformity resulting from facial trauma.

Untreated craniofacial fractures may result in a deformity similar to that which results from underdevelopment of the maxilla. Such a deformity can be corrected by osteotomy simulating Le Fort type III fracture lines. This operation necessitates careful pre-operative planning which incorporates a feasibility study with the combined use of dental study casts, cephalometric radiography and life-size photographic reproductions. A case history which describes the management of late complications of a malunited class III fracture is given.

Use of the alar base cinch suture in Le Fort I osteotomy: is it effective?

The alar base cinch suture is often used to mitigate the unfavourable increase in nasal width after Le Fort I impaction or advancement of the maxilla. Recently a number of studies have looked at the stability and clinical outcome of this intervention. We investigated the effect of an alar base cinch suture on the change in width of the alar base after Le Fort I osteotomy using a three-dimensional imaging system. Twenty-eight patients were prospectively randomised into an intervention group where a cinch suture was used, and a control group. The width of the alar base was measured before operat

Midface distraction following Le Fort III and monobloc osteotomies: problems and solutions.

Distraction osteogenesis has been used increasingly for midfacial advancement in patients with syndromic craniosynostosis and in severe developmental hypoplasia of the midface. In these patients, the degree of advancement required is often so great that restriction of the adjacent soft tissues may preclude stable advancement in one stage. Whereas distraction is an ideal solution by which to gradually lengthen both the bones and the soft tissues, potential problems remain in translating the distraction forces to the midface. In these patients, severe developmental hypoplasia may be associated w

Clinical anatomy of the posterior maxilla pertaining to Le Fort I osteotomy in Thais.

This article studies the anatomy of the posterior maxilla pertaining to bone-cut design of Le Fort I osteotomy to avoid the injury to the descending palatine artery in Thais. Fifty-five skulls (38 males, 17 females) were assessed for the anatomical landmarks by a combination of direct inspection, computerized imaging, and computed tomography scan analysis. The results showed that 27.28% of the pterygomaxillary junction (PMJ) became synostosis. The mean heights of the PMJ, posterior maxilla, and maxillary tuberosity were 15.14 +/- 2.46 mm, 22.51 +/- 3.50 mm, and 7.45 +/- 2.76 mm, respectively.

["Quadrangular" Le Fort II osteotomy for hypoplasia of the midface. Apropos of 28 cases].

In 1971, Kufner described the "quadrangular" osteotomy of the midface, which he used to correct asymmetry in patients with recession of the midface and a normal nasal skeleton. We have used this method in 28 patients, 5 women and 23 men. The surgical technique is described and the indications, the approach and the stability of the postoperative results are discussed. The conclusion is that the "quadrangular" osteotomy is a very satisfactory method for the aesthetic and functional rehabilitation of a certain group of midfacial anomalies.
