[From reconstruction to function : Hands-on training in tympanoplasty using real-time feedback].

BACKGROUND: Surgery of the middle ear requires a very high level of fine motor skills. Due to the increased potential for complications in middle ear operations, it is appropriate to acquire these skills beforehand by operating on a model. OBJECTIVE: How satisfactory is the training on suitable models? Are the skills acquired from working on a model transferable to intraoperative situations? Will the type of model and its use for training influence ear surgery in the future? MATERIAL AND METHODS: Available publications and own experiences with the Dresden tympanoplasty model (DTM) are analyz

[Basis of endoscopic dissection of the ear with analysis of endoscopic anatomy. Part 1].

Today, endoscopic transcanal ear surgery with confident steps is becoming a practice. It will be used by the patient in repeated practice by the surgeon before being used in the patient. It is so important and popular in connection with this dissection work. Working on biomaterial sets itself several tasks: training manual skills to work with an endoscope and an instrument in the middle ear, stage-by-stage training for performing specific interventions, detailed analysis and understanding of structures anatomy and topography. This article presents a step-by-step dissection algorithm with an an

Steerable Robot-assisted Micromanipulation in the Middle Ear: Preliminary Feasibility Evaluation.

HYPOTHESIS: The use of a robotic manipulator with a dexterously orientable gripper will expand the ability of middle ear surgeons to perform precise tasks and access otherwise challenging anatomic regions. BACKGROUND: Middle ear surgery presents unique challenges because of the constrained operative space and limited access to certain anatomic regions. METHODS: A custom-designed robot with a sideways-reaching gripper was used to evaluate feasibility of manipulation tasks in different middle-ear anatomical zones. Reachable workspace within the middle ear, accuracy of free-space path following

Transtympanic dilatation of the eustachian tube during chronic ear surgery.

CONCLUSION: Evaluation of the protympanic segment of the eustachian tube is feasible during chronic ear surgery. Balloon dilatation of that segment yields a bigger aperture. OBJECTIVE: To evaluate the feasibility of visualization and balloon dilatation of the protympanic segment of the eustachian tube during chronic ear surgery. METHODS: This study was carried out on a consecutive case series. All patients undergoing surgical treatment for cholesteatoma or tympanic membrane perforation over a 6-month period of time at a tertiary hospital were evaluated intraoperatively for the ability to vis

Results of endoscopic middle ear surgery for cholesteatoma treatment: a systematic review.

Traditional surgery for cholesteatoma of the middle ear is performed by microscopic approaches. However, in recent years endoscopic instrumentation, techniques and knowledge have greatly improved, and in our opinion endoscopic surgical techniques will gain increasing importance in otologic surgery in the future. The aim of this study was to focus on outcomes obtained using endoscopic surgery for the treatment of middle ear cholesteatoma. A systematic review of the literature was performed. A total of 7 articles comprising 515 patients treated exclusively with endoscope or with a combined techn

Surgery of the semicircular canals.

INTRODUCTION: Surgery of the semicircular canals is seeing a revival as recently we have witnessed the development of specialized surgeries for each canal. OBJECTIVES: The aim of this work is through a review of the literature to describe these different surgeries while stressing on certain surgical aspects, their respective indications, results and their risks. DISCUSSION: 1: The surgery of the posterior canal relates to the benign paroxysmal positional vertigo resistant to the medical treatments. The results are very good but the indications have become rarer since the introduction of the

Complications and their management in tympanomastoid surgery.

Chronic diseases of the ear and their surgical treatment can endanger the delicate structures of the temporal bone in close relationship with the middle ear. The experience and expertise of the otologic surgeon, based on perfect knowledge of the complex anatomy of the whole temporal bone, are unfortunately built up partly through the management of complications encountered in tympanomastoid surgery and may require knowledge of basic neuro-otologic procedures. Paralysis and paresis of the facial nerve and labyrinthine, and dural and vascular injuries may have consequences not only in the final

Stapes surgery: how precisely do different prostheses attach to the long process of the incus with different instruments and different surgeons?

HYPOTHESIS: The goal of this study was to compare the attachment of stapes prostheses with differently shaped loops to the long process of the incus. BACKGROUND: In stapes surgery, the attachment of the prosthesis to the long process of the incus plays an important role concerning the gain in hearing and the development of late complications such as incus erosion and necrosis. Band-shaped and spiral loops have been developed to achieve a broad, firm attachment to the long process of the incus. During stapes surgery, the view at the prosthesis is restricted, making it impossible to evaluate th

The combined posterior temporoparietal and galeal fascial flap: a new flap in the elevation of the constructed auricle (second stage of microtia correction).

Over the past few years, the technique of elevating the buried ear framework in the second stage of microtia correction has shifted from skin grafting to the use of flaps and cartilage blocks in the retroauricular sulcus. While the temporoparietal fascial flap should be reserved for secondary procedures and the treatment of complications, the mastoid fascial flap is inadequate by itself and needs an additional cartilage graft. Here, we describe a new flap, the combined posterior temporoparietal and galeal fascial flap, for the elevation of the buried ear cartilage. The flap is robust, with a d

Current Applications and Emerging Horizons of Artificial Intelligence in Temporal Bone Computed Tomography.

The temporal bone has intricate anatomy. CT examinations can clearly depict fine anatomical structures and subtle lesions, providing indispensable imaging evidence for diagnosing otologic diseases and planning surgical procedures. However, image interpretation has several inherent limitations, including subjectivity, substantial interobserver variability, and heavy reliance on physician experience, all of which can compromise clinical decision-making and lead to inconsistent patient management. In recent years, AI-driven approaches have made significant progress in this field, particularly in

New surgical frontiers for 4K 3D-exoscope in paediatric head and neck surgery.

PURPOSE: To define the interest, advantages, and disadvantages of the use of a 3D-exoscope in paediatric ENT surgery. METHODS: Four surgeons with experience in paediatric surgery completed a questionnaire following each surgery performed under 3D-exoscope to evaluate the contribution of the tool compared to the usual practice (microscope or magnifying loupes). Surgeries were separated into three groups: otology, transoral and cleft palate surgery, and open head and neck surgery. RESULTS: Between June 2021 and June 2022, 151 paediatric surgeries were included in this study. Among them, 93 (62

Microtia and congenital aural atresia.

Microtia and congenital aural atresia (CAA) are congenital anomalies that are so common that every otolaryngologist should be familiar with the initial evaluation and care of the patient. When one ear hears normally, speech and language development should be normal. The gross and fine motor development of the baby or child is not expected to be affected in isolated cases of microtia and CAA. Current technologies allow for reconstruction or habilitation of the microtic ear when the child is several years of age. The hope is that tissue engineering can eliminate donor site morbidity. Management of acoustic neuromas, 1978-1983.

The surgical aspects of 162 consecutive procedures for removal of acoustic neuromas, performed from 1978 through 1983, are reviewed. Nearly all of the procedures were done through a retrosigmoid suboccipital craniectomy. Most used the combined skills of a neurosurgeon and an otological surgeon. Total tumor removal was accomplished in 98% of cases. There have been two recurrences and one postoperative death. The facial nerve was preserved in 81% of procedures. Facial function returned in nearly all of these patients, but the degree of return was variable. The cochlear nerve was preserved in 55
