[A better understanding of clinical pain. Experimental data on 3 animal models of pain].

For a better understanding of clinical pain, several groups involved in the study of basic pain mechanisms have proposed the use of various experimental models close to clinical situations. These models are based either on neurogenic or inflammatory process. Data obtained with three of these models will be developed in the paper: rats rendered arthritic by Freund's adjuvant injection into the tail, rats with an intraplantar injection of carrageenin in one hindpaw, rats with a moderate ligature of one common sciatic nerve. The various pharmacological approaches revealed dramatic changes of the

Engel's "Psychogenic Pain and the Pain-Prone Patient:" a retrospective, controlled clinical study.

Engel's hypothesis of pain-prone patients having a distinct pattern of developmental psychosocial experiences was tested in a controlled design including four groups of 20 patients each: A) psychogenic pain, B) organic pain, C) psychogenic bodily symptoms, and D) organic disease. On admission an open-ended interview, including childhood experiences, was tape-recorded. Measures were taken to minimize observer bias. Patients in group A had, compared to the other groups, significantly increased prevalence of: "Parents, verbally and/or physically abusive of each other," "parents, abusive of the ch

Changes in Pain Score Associated With Clinically Meaningful Outcomes in Children With Acute Pain.

BACKGROUND: Identifying changes in pain score associated with clinically meaningful outcomes is necessary when using self-report measures to assess pain in children. We aimed to determine the changes in pain score associated with a minimum clinically significant difference (MCSD), ideal clinically significant difference (ICSD), and patient-perceived adequate analgesia (PPAA) and to evaluate for differences based on initial pain intensity and patient characteristics. METHODS: This was a cross-sectional study of children 6 to 17 and 4 to 17 years old who were assessed using the Verbal Numerical

Ethnic Differences Identified by Pain Sensitivity Questionnaire Correlate With Clinical Pain Responses.

BACKGROUND AND OBJECTIVES: The Pain Sensitivity Questionnaire, English version (PSQ-E), is predictive of pain-related responses to experimental stimuli. Ethnic differences have been noted in experimental measures of pain sensation using quantitative sensory testing. The present study sought to determine if the PSQ-E also identified similar ethnic differences. METHODS: Fifty-seven subjects who self-identified as African Americans (AAs) and who were scheduled to undergo a low-back interventional procedure completed the PSQ-E and other questionnaires. Their data were compared with an age-, sex-,

Myofascial face pain. Clinical characteristics of those with regional vs. widespread pain.

BACKGROUND: The authors conducted a study to determine whether there are differences in salient clinical characteristics between patients who have both myofascial face pain, or MFP, and comorbid fibromyalgia, or FM, and patients who have MFP but not FM. METHODS: The authors enrolled in the study 162 female subjects who had histories of MFP. In physical examinations at the time of initial consultation, they recorded facial pain signs and symptoms. At the research interview follow-up (seven years post-consultation), participants were screened for a lifetime history of FM and other health proble

Application of a Clinical Approach to Diagnosing Primary Pain: Prevalence and Correlates of Primary Back and Neck Pain in a Community Physiatry Clinic.

Chronic back or neck pain (CBNP) can be primary (nociplastic or neuroplastic; without clear peripheral etiology) or secondary (to nociceptive or neuropathic causes). Expanding on available models of nociplastic pain, we developed a clinic-ready approach to diagnose primary/nociplastic pain: first, a standard physical exam and review of imaging to rule out secondary pain; and second, a detailed history of symptom presentation to rule in primary pain. We trained a physician who evaluated 222 patients (73.9% female, age M = 59.6) with CBNP; patients separately completed pain and psychosocial ques

Clinical Practice Guideline: Nonspecific Neck Pain.

BACKGROUND: Nearly half of the adults in Germany have neck pain at least once a year. Neck pain is distressing to those who suffer from it and costly to society at large. Its treatment requires a coordinated approach with the participation of general practitioners, physician specialists, and other health care professionals, such as physical therapists. The German Society for General Medicine and Family Medicine has now issued an upgraded (S1 to S3 level) set of recommendations for rational diagnosis and treatment. METHODS: The new recommendations were developed in an interdisciplinary and int

Clinical judgement in chest pain: a case report.

BACKGROUND: Cardiac stress testing is a validated diagnostic tool to assess symptomatic patients with intermediate pretest probability of coronary artery disease (CAD). However, in some cases, the cardiac stress test may provide inconclusive results and the decision for further workup typically depends on the clinical judgement of the physician. These decisions can greatly affect patient outcomes. CASE PRESENTATION: We present an interesting case of a 54-year-old Caucasian male with history of tobacco use and gastroesophageal reflux disease (GERD) who presented with atypical chest pain. He ha
