Chronic pain. Clinical management of common causes of geriatric pain.

Effective management of chronic pain in older persons is attainable, although unnecessarily elusive. A host of factors can impede assessment and drug management, including impaired cognitive function, multiple potential causes of pain, pharmacokinetics and pharmacodynamics unique to the geriatric population, and clinician anxiety regarding analgesic addiction. In the geriatric population, some of the most prevalent chronic pain management cases involve osteoarthritis, low back pain, and neuropathy. Effective pain management is achieved with analgesics (acetaminophen and nonsteroidal anti-infla

Understanding psychological aspects of chronic pain in interventional pain management.

There is no doubt that chronic pain is recognized as a biopsychosocial phenomenon in which biological, psychological, and social factors dynamically interact with each other. Thus, the role of psychological factors and understanding chronic, persistent disabling pain has been well recognized, but poorly understood. Approximately 1/2 to 2/3 of all patients diagnosed with chronic pain manifest to various levels of psychological distress. Chronic pain and psychological disorders are the two most common elements in the United States. Statistics show that, approximately 22% of Americans suffer from

Farmers' Perspectives on Chronic Pain and Chronic Pain Management Strategies.

OBJECTIVE: This study aimed to explore farmers' perspectives on: 1) the meaning of chronic pain, 2) its impact, 3) their preferences for chronic pain management in the future, and 4) views on specific evidence-based chronic pain management strategies. METHODS: In-depth, semi-structured interviews were conducted with Australian farmers with a history of chronic pain within the last 12 months. A positivist, descriptive approach to Thematic Analysis was employed for data analysis. RESULTS: Sixteen Australian farmers, aged 36-75 years, participated. Five themes and 11 sub-themes were identified:

Progress in pain mechanisms and management.

Pain is a multifaceted sensory and emotional experience that profoundly affects human health and quality of life. It functions as a double-edged sword: while acute pain serves as a protective warning signal, chronic pain compromises physiological and psychological well-being. Pain is now widely regarded as the "fifth vital sign," and given its rising prevalence and substantial global burden, chronic pain may also be considered a major chronic disease entity. Accordingly, elucidating the neural and molecular underpinnings of pain and developing effective analgesic strategies remain urgent prior

[Management of pain in chronic pancreatitis].

Pain in chronic pancreatitis remains difficult to treat, due to a lack of understanding of the underlying nociceptive process. Therefore, treatments are often based on anatomical changes, although morphological features are not correlated with pain intensity and severity. It is well established that the cause of pain in chronic pancreatitis involves multiple mechanisms. Traditional theories focused solely on the role of morphological abnormalities of the pancreas in pain are flawed; there is now clear evidence showing that the resulting pain resembles that observed in neuropathic pain. Therefo

Pain management in victims of disasters.

Pain is widely studied and is considered a major clinical, social, and economic problem worldwide, although it remains poorly understood. For disaster victims, the complex picture, biologically, psychologically, and socially, only makes the situation even more complicated. This narrative review aims to describe specific aspects of pain and pain management in disaster victims. We reviewed relevant literature, both on pain management and selected specific data related to conflict victims. We discuss the complexity of the picture, its different aspects, and mechanisms. We discuss the limitations

An overview of pain assessment and management.

Pain is a distressing, subjective and complex phenomenon that all nurses will encounter in their clinical practice. Effective pain management requires nurses to undertake a structured assessment to identify the probable causes of pain and guide management. Interventions used to reduce pain can be varied and multimodal. This article provides an overview of pain including its definition, classifications, assessment and management. It emphasises the importance of a person-centred approach to care which reflects Margo McCaffery's seminal quote that pain is 'whatever the experiencing person says it

tVNS in the management of headache and pain.

First clinical observations of the therapeutic effect of vagus nerve stimulation were of patients who were treated for refractory epilepsy with a fully implanted vagus nerve stimulator, who also reported an improvement of their migraine and cluster headache. With the development of non-invasive vagus nerve stimulation, first clinical studies concerning a possible therapeutic effect in migraine and cluster headache were performed. In a controlled study, transcutaneous cervical vagus nerve stimulation (tcVNS) showed a significant but limited effect in acute treatment of a migraine attack. There

Management of Pain Associated with Fractures.

PURPOSE OF REVIEW: The purpose is to review current literature on pain management strategies from initial presentation to postoperative care on common fracture types. RECENT FINDINGS: - Hip fractures benefit from use of multimodal pain control for early mobility and decreased narcotic requirement. - Distal radius fracture pain during reduction can be managed with hematoma block. Postoperatively, a soft dressing is adequate, and use of a compression glove may improve pain control and edema. - Ankle fractures can be reduced with hematoma block, though use of procedural sedation may reduce reduc

Assessment and management of chronic pain.

Chronic pain can have significant physical, psychological and social effects on a person's life, as well as on their families and friends. However, it is often not well-recognised or understood, which can lead to further harm. Therefore, an individualised, person-centred approach to chronic pain is essential to accurately assess pain and to develop an appropriate treatment plan. This article outlines the biomedical and psychosocial factors that can influence an individual's pain experience that should be considered as part of the assessment and management of chronic pain, and explores the asse

Optimal Management of Endometriosis and Pain.

The pathophysiology of endometriosis-associated pain involves inflammatory and hormonal alterations and changes in brain signaling pathways. Although medical treatment can provide temporary relief, most patients can achieve long-term sustained pain relief when it is combined with surgical intervention. Owing to its complexity, there is an ongoing debate about how to optimally manage endometriosis-associated pain. We believe optimal management for this condition requires: 1) possible egg preservation in affected young patients with and without endometriomas; 2) preoperative medical suppression

CE: Managing Postoperative Pain.

: Acute postoperative pain remains undertreated despite the dramatic increase in opioid prescribing in the United States over the past 20 years. Inadequately relieved postoperative pain may be a risk factor for persistent postoperative pain, chronic pain, and disability. In an effort to promote evidence-based strategies for optimal postoperative pain management, the American Pain Society published a new postoperative pain management guideline in 2016. Its 32 recommendations for interdisciplinary and multimodal postoperative pain management are stratified according to risks and benefits, based

Management of abdominal pain in children.

Abdominal pain (AP) is a very common complaint caused by a variety of conditions. Mild or moderate AP affects practically all children of all ages. The pain usually settles spontaneously without medical intervention. AP severe enough to require medical intervention has both surgical and non-surgical causes. It is responsible for considerable morbidity, missed school days, and significant use of health resources. Children usually present either with an acute or recurrent AP. In comparison, chronic AP with persistent symptoms, lasting days or weeks, is rare in children. Surgical conditions may b
