Validity, Reliability, and Responsiveness of a New Device for Measuring Pressure Pain Thresholds.

Background/Objectives The evaluation of treatment effectiveness largely depends on the outcome measures used in the study. Patient-reported outcome measures (PROMs), such as the visual analog scale (VAS), and more objective measures, such as pressure pain thresholds (PPT), are commonly employed to capture pain-sensory profiles and hypoalgesic effects. The NOD is a novel digital algometer designed to assess the PPT. This study aimed to evaluate the validity, reliability, and responsiveness of the NOD algometer using a conventional algometer as a reference instrument for PPT assessment. Methods:

Impact of respiration on abdominal pain thresholds in healthy subjects - A pilot study.

OBJECTIVES: Pressure algometry is a validated method for objectively quantifying pain sensitivity in musculoskeletal disorders, particularly useful for diagnosis and monitoring of treatment responses. However, its application to abdominal pain sensitivity remains insufficiently standardized, especially regarding the influence of physiological factors such as respiration. This study investigated the impact of respiratory phase (inhalation vs exhalation) on abdominal pressure pain thresholds (PT) to support the development of standardized assessment protocols. METHODS: Thirty-one healthy medica

Knee extensor strength is associated with pressure pain thresholds in adults with fibromyalgia.

OBJECTIVE: Individuals with fibromyalgia (FM) have lower muscle strength and lower pressure pain thresholds (PPT). The primary aim of this study was to determine the associations between muscle strength and PPT in adults with FM to test the hypothesis that greater measures of muscle strength would be associated with greater values of PPT. Secondary aims included determining the effects of pain severity and the peak uptake of oxygen (VO2) on the associations between muscle strength and PPT. METHODS: Knee extensor and flexor strength (N = 69) was measured in the dominant leg using a dynamometer

Relationships between pain thresholds, catastrophizing and gender in acute whiplash injury.

The mechanisms underlying sensory hypersensitivity (SH) in acute whiplash associated disorders (WAD) are not well understood. We examined the extent of the relationships between the sensory measures of pressure pain threshold (PPT) and cold pain threshold (CPT), catastrophizing, pain and disability levels and gender in acute WAD. Thirty-seven subjects reporting neck pain following a motor vehicle accident were examined within five weeks post-injury. Measures of neck pain and disability (Neck Disability Index, NDI) and catastrophizing (Pain Catastrophizing Scale, PCS) were taken. CPT was assess

Experimental pain thresholds and plasma beta-endorphin levels during exercise.

Experimental pain thresholds (electrical intracutaneous finger and dental pulp stimulation) and plasma hormone levels (beta-endorphin, cortisol, and catecholamines) were measured in ten healthy sportive men before, during, and after progressively more strenuous physical exercise. In a double-blind study conducted on two different days, 20 mg of the opioid-antagonist naloxone or placebo was administered prior to exercise. A significant pain threshold elevation was found during exercise for finger (ANOVA, P less than 0.004) and dental pulp stimulation (P less than 0.01). Pain threshold elevation

Reliability and standard error of measurement of pressure pain thresholds at articular landmarks.

BACKGROUND: Pressure pain threshold (PPT) testing is a widely used quantitative sensory testing method to assess mechanical pain sensitivity. While reference values for PPT at articular-/joint-related and osseous landmarks are available, data on measurement reliability remain limited. This study aimed to determine the intra- and inter-rater reliability of PPT measurements and to report the standard error of measurement (SEM) for joint PPT. METHODS: Healthy participants aged 18-40 years were included in a two-part study. In Part A (N = 40), intra-rater reliability was assessed through three re

Respiratory movement and pain thresholds in airway environmental sensitivity, asthma and COPD.

INTRODUCTION: Patients with "sensory hyperreactivity" (SHR) have airway environmental sensitivity, chronic cough and dyspnoea. Cough, chest discomfort and sense of difficulties getting air are some of the symptoms these patients seek medical attendance for. The patients have increased cough sensitivity to inhaled capsaicin, mediated by ion channel receptors on sensory nerves also known to react to pain stimuli. Whether a link exists between capsaicin airway sensitivity and pain sensitivity has not yet been evaluated. The aim was to investigate chest mobility, respiratory movement and pain sens

A preliminary report on pain thresholds in bulimia nervosa during a bulimic episode.

Subjects with bulimia nervosa (BN) have been shown to exhibit abnormal satiety responses. Short-term satiety is largely mediated by afferent vagal activity. Activation of afferent vagal fibers has also been found to stimulate a descending pain inhibitory pathway that leads to elevation in somatosensory pain thresholds. Therefore, the study of pain thresholds in BN subjects may lead to a better understanding of afferent vagal function in this disorder. In this preliminary study, pressure pain thresholds were assessed in nine subjects with BN on 3 consecutive days during a binge-eating and vomit

The reproducibility of cephalic pain pressure thresholds in control subjects and headache patients.

Pain pressure thresholds (PPT) were measured at 13 cephalic points bilaterally in 30 headache patients (10 with tension-type headache, 10 with migraine and 10 with cervicogenic headache) and 10 control subjects on three different days. During the sessions, the subjects reported their pain intensity on the right and left sides of the head on a visual analogue scale (VAS). The variability between days was estimated as a coefficient of repeatability (CR = 2 standard deviations of intraindividual differences). The mean CR across all 13 locations was larger in headache patients (2.0 kg/cm2) than in

Cephalic and extracephalic pressure pain thresholds in chronic tension-type headache.

Pressure pain thresholds were assessed with an algometer (Somedic Inc.), over the forehead, temple and suboccipital region as well as over the Achilles tendon. A group of 32 patients suffering from chronic tension-type headache was compared to 20 healthy controls and to 10 migraineurs without aura. Although individual values were widely scattered, pressure pain thresholds were on average significantly lower in chronic tension-type headache, not only at pericranial sites but also over the Achilles tendon. Only 50% of these patients had one or more pericranial thresholds 1.5 S.D. below the mean

The measurement of pain threshold in man by means of electrical stimuli. A critical appraisal.

The problem of pain threshold measurement in man is critically considered. The advantages of electrical stimuli in determining the cutaneous pain threshold are discussed. The conducting properties of the skin to electric current are carefully examined: in fact, such properties must be taken into account when electrical stimuli are applied to the skin. A series of investigations on pain threshold measurement by means of electrical stimuli are reported. The different sensations induced by stimulating the skin with different kinds of electrodes and different types of stimuli were analysed: trains

Pregabalin and hyperbaric oxygen therapy on pain thresholds and anxio-depressive behaviors in a preclinical fibromyalgia pain model.

Fibromyalgia (FM) is a chronic, widespread pain disorder generally of a non-inflammatory nature with many known affective and cognitive comorbidities. There is promise in the implementation of hyperbaric oxygen therapy (HBO2) for alleviating FM pain and comorbidities, despite no work investigating the efficacy of this treatment in prominent preclinical FM models. This project aimed to investigate the affective components, specifically anhedonia and anxiety, associated with an acidic saline model of FM in rats. We investigated the acidic saline model's ability to produce the sensory component o

Tenderness as measured by pressure pain thresholds extends beyond the pelvis in chronic pelvic pain syndrome in men.

INTRODUCTION: Urological Chronic Pelvic Pain Syndrome (UCPPS) in men is a common complaint, and causes significant impairment in quality of life. Until recently, the focus of research has primarily been on pain symptoms or underlying prostate pathology. However, new clinical phenotyping demonstrates pelvic tenderness to be an important component of UCPPS. Unfortunately, mechanisms underlying tenderness remain to be understood, and tenderness itself has not been well quantified. AIM: To validate the use of pressure pain thresholds as a method of measuring tenderness in UCPPS and to demonstrate

Assessment of pain threshold and pain tolerance in women in labour and in the early post-partum period by pressure algometry.

The changes in the pressure pain threshold (PPThr) and pressure pain tolerance (PPTol) in 41 parturients have been studied during the active phase of labour and in the early post-partum period. The sensitivity to pressure stimuli was examined with an electronic pressure algometer placed on the sternum during the interval between painful contractions, after extradural analgesia and 24 h after childbirth. Prior to extradural analgesia, mean (+/- SD), PPThr and PPTol were 4.9 +/- 1.6 kg 0.25 cm-2 and 6.9 +/- 1.8 kg 0.25 cm-2, respectively. Similar values were recorded 1 h after induction of the e

Pressure pain threshold and pain tolerance in episodic tension-type headache do not depend on the presence of headache.

Pressure pain thresholds and responses to painful mechanical stimuli were obtained from 20 subjects with episodic tension-type headaches (TTH). Tender points in the temporalis and trapezius muscles were studied, along with two nontender points, one in the temporalis and one on the Achilles tendon. Two examinations were performed, one during and one without a headache, and results were compared. No significant differences were found in either variable for any tested points. The data demonstrate that the sensitivity level of these points does not differ based on the presence or absence of TTH. T
