Change in levator veli palatini muscle activity of normal speakers in association with elevation of the velum using an experimental palatal lift prosthesis.

OBJECTIVE: The purpose of this study was to examine whether mechanical elevation of the velum can directly affect the levator veli palatini muscle (LVP) activity using normal speakers as subjects. DESIGN: Each subject was instructed to produce /mu/, /u/, /pu/, /su/, and /tsu/ in a speech task and to blow with maximum possible effort. Smoothed electromyographic activity of the LVP was recorded with an experimental palatal lift prosthesis (PLP) both in place and removed. PARTICIPANTS: Four normal speakers were used as subjects. RESULTS: LVP activity for all tasks was significantly smaller (p

Outcome of Palate Re-repair with Radical Repositioning of the Levator Muscle Sling as a First-Line Strategy in Postpalatoplasty Velopharyngeal Incompetence Management Protocol.

BACKGROUND: Palatal re-repair aims to improve velar function by retropositioning the levator muscles. Although it has become a popular procedure, very few studies document its efficacy. To date, this is the largest series reported to clarify its indications and efficacy. METHODS: One hundred eighty-three consecutive cleft patients presenting with velopharyngeal incompetence and evidence of abnormally oriented levator muscles underwent palate re-repair (regardless of the gap size) performed by a single surgeon from 2000 to 2015. Perceptual speech assessment was performed using the Pittsburgh W

Full-thickness reconstruction of the eyelid with rotation flap based on orbicularis oculi muscle and palatal mucosal graft: long-term results in 12 cases.

BACKGROUND: Numerous techniques have been proposed for full-thickness eyelid reconstruction. Previously, we reported full-thickness eyelid reconstruction with a rotation flap based on the orbicularis oculi muscle and palatal mucosal graft. Here, we report long-term results in 12 cases. METHODS: After confirmation of defect size, the mucosal defect was covered with a split-thickness palatal mucosal graft. The rotation flap was elevated at the lateral orbital region and the skin defect was covered. Seven cases were reconstructions after tumour excision and five cases were for lagophthalmos afte

The influence of the muscles of facial expression on the development of the midface and the nose in cleft lip and palate patients. A reflection of functional anatomy, facial esthetics and physiology of the nose.

The further improvement of well-established techniques in primary and secondary cleft surgery requires a detailed and interdisciplinary knowledge and observation of anatomical, functional and developmental problems. An investigation into the macroscopic and microscopic anatomy of the perinasal and perioral muscles and parts of the human nasal septum, as well as into the pathomorphology of ancient skulls with untreated clefts is presented. On this basis an interpretation of clinical findings in untreated newborns compared with surgically treated CLP-patients has been undertaken. The 3D-CT, supe

Assessment of levator veli palatini muscle morphology and its correlation with velopharyngeal function using dynamic magnetic resonance imaging in children with repaired cleft palate.

Understanding the underlying anatomic variability of the levator veli palatini (LVP) and relationships among velopharyngeal structures is important for effective treatment of velopharyngeal insufficiency (VPI) following cleft palate surgery. This study was designed to reveal potential correlations between LVP muscle characteristics and velopharyngeal competence using dynamic evaluation in pediatric patients with repaired cleft palates. LVP muscle variations were further examined according to sex, cleft type, timing of cleft palate repair, and surgical techniques. A retrospective analysis was c

[One-stage reconstruction of orbital floor combine hard palate defect with the temporalis muscle flap after malignant tumor of maxillary sinus opreation: a case report].

An elderly male case of olfactory neuroblastoma in the left maxillary sinus, who underwent complete resection of the tumor and total maxillectomy. Ipsilateral temporalis muscle flap was used to repair orbital floor and hard palate defects. During the 3-month follow-up, the patient recovered well. It is suggested that one-stage reconstruction with temporalis muscle flap is a good choice for the repair of maxilla defect after resection of maxillary sinus carcinoma, especially in case of invasion of orbital floor and hard palate.

Correction of incomplete cleft palate by u-shaped flap palatoplasty.

OBJECTIVES: During cleft palate repair, velopharyngeal sphincter reconstruction is still a challenge to plastic surgeons. To improve results of surgical treatment of cleft palate and secondary velopharyngeal incompetence, a carefully designed modified procedure for palatoplasty is presented. MATERIALS AND METHODS: Thirty patients with incomplete cleft palate corrected by this procedure from April 2003 to October 2007 were included. A u-shaped incision was made in the anterior palate to separate bipedicle flap based on the greater palatine arteries of both sides. After complete dissection of t

Symptomatic and essential palatal tremor. 2. Differences of palatal movements.

Palatal tremor, a rhythmic movement disorder of the soft palate, may be described as two separate entities: symptomatic palatal tremor (SPT) and essential palatal tremor (EPT). The symptomatic form is associated with brain stem or cerebellar disease, whereas the essential form has no known etiology. A cardinal symptom of EPT is the presence of ear clicks, which do not occur in SPT. Visual observation of the movements in the two disorders suggests that the difference in symptoms is due to the activation of different palatal muscles, the levator veli palatini in SPT and the tensor veli palatini

Electromyographic Evaluation of Superior Orbicularis Oris Muscle Activity in Patients With Complete Cleft Lip and Cleft Palate Before and After Vestibuloplasty Using Carbon Dioxide Laser Therapy.

Introduction: Cleft lip and cleft palate are the most typical congenital craniofacial anomalies. Post-surgical scars in cleft lip patients can cause shallow upper labial sulcus and reduced upper lip length. The aim of the present study was to evaluate the impact of CO2 laser vestibuloplasty on the electromyographical (EMG) activity of patients' superior orbicularis oris muscle, depth of labial sulcus, and upper lip length, and their satisfaction with the procedure and its results. The CO2 laser is an effective vestibuloplasty procedure, is safe and highly acceptable for patients, and has littl

Palatal anatomy for sleep apnea surgery.

UNLABELLED: The goal of this review is to advance the understanding of the muscular and soft tissue palatal anatomy as it relates to palatal surgery for sleep apnea and the phenotypic variations that generate the shape and collapsibility of the retropalatal airway. Anatomically, the soft palate has both a proximal and distal segments separated by the palatal genu. The proximal palatal segment has a variable angle from the hard palate (ie, alpha angle) determined by the position and length of the levator veli palatini muscle. The palatopharyngeus muscle (PP) is a major defining element of the p

Secondary Palatal Elongation: Improvement in Speech Quality.

Cleft palate is one of the challenging problems in the field of craniofacial surgery. In particular, the conventional methods of bilateral and severe cleft palate repairs have failed to achieve normal speech. In most instances, secondary procedures such as pharyngoplasty and pharyngeal flap surgery are performed to improve speech. This study introduces secondary palatal elongation (SPE) as a new approach to cleft palate repair. The patients included usually had a short palate and unrepaired palatal muscles. The authors' procedure involved dissecting the previously repaired palatal mucosa and pu

Treatment of palatal myoclonus with botulinum toxin injection.

Palatal myoclonus is a rare cause of pulsatile tinnitus in patients presenting to the otolaryngology office. Rhythmic involuntary contractions of the palatal muscles produce the pulsatile tinnitus in these patients. Treatment of this benign but distressing condition with anxiolytics, anticonvulsants, and surgery has been largely unsuccessful. A few investigators have obtained promising results with botulinum toxin injection into the palatal muscles. We present a patient with palatal myoclonus who failed conservative treatment with anxiolytics. Unilateral injection of botulinum toxin into her t

Reconstruction of the palatal aponeurosis with autogenous fascia lata in secondary radical intravelar veloplasty: a new method.

Velopharyngeal insufficiency in cleft patients with muscular insufficiency detected by nasendoscopy is commonly treated by secondary radical intravelar veloplasty, in which the palatal muscles are reoriented and positioned backwards. The dead space between the retro-displaced musculature and the posterior borders of the palatal bone remains problematic. Postoperatively, the surgically achieved lengthening of the soft palate often diminishes due to scar tissue formation in the dead space, leading to reattachment of the reoriented muscles to the palatal bone and to decreased mobility of the soft

Botulinum toxin injection for objective tinnitus from palatal myoclonus: a case report.

Objective tinnitus may be caused by many etiologies-palatal myoclonus being one of them. We report one patient of voluntary palatal myoclonus presenting with objective tinnitus treated with botulinum toxin injection. Five units of botulinum toxin A were injected into each side of the soft palate at the palatal muscles (levator veli palatini and tensor veli palatini muscle). The tinnitus disappeared within two days of injection and no side effect was observed.

Soft-palate myogenesis: a developmental field paradigm.

Surgical correction of clefts of the soft palate leads to varying degrees of normal function although the repair itself is successful. Explanations for this include structural abnormalities of the muscles. Previous studies have focused primarily on gross anatomical features of late fetal and postnatal cleft palate musculature; however, infrequent reference has been made to early prenatal morphologic patterns of soft-palate development, beginning with the embryo. Thus we evaluated the chronology of prenatal myogenesis of the soft palate from its early mesenchymal phase through the appearance of

Palatal myoclonus--its remote influence.

Four patients with palatal myoclonus were examined electromyographically. Surface EMG was recorded simultaneously from several myoclonic muscles and the correlation between their rhythmic discharges was shown. The possible existence of a remote influence of palatal myoclonus was tested by recording soleus and anterior tibial H-reflexes and seeking a correlation of their amplitudes with the rhythm of palatal myoclonus. Two cases, both with additional severe spasticity in the legs, showed rhythmic variation of the soleus H-reflex amplitude corresponding with that of their palatal myoclonus. This

Oscillatory activity in different motor subsystems in palatal myoclonus. A case report.

In a patient with palatal myoclonus the mutual interaction between voluntary movements and the myoclonic activity was analysed. Amplitude and frequency of myoclonic activity in hand muscles were modulated by flexions and extensions. A 1:1 relationship was found between EMG-bursts in hand muscles and palatal movements. A 1:2 relationship was found between eye and finger movements. Resetting of myoclonus in the abductor digiti minimi muscle occurred after cutaneous ulnar nerve stimulation. It is suggested that feedback plays an important role in the generation of the oscillatory activity in PM i
