The restoration of the soft palate presents a challenge completely different from that of the hard palate. The mobility of the soft palate tends to interfere with velar extensions. The reduction in size of the soft palate extension to prevent impingement upon the mobile margins of the defect will lead to insufficient oronasal separation during functional activities. The solution is to construct a specially designed prosthesis to attain the maximum utilization of the remaining structures and their motility. Although each pharyngeal extension is different in shape, they give the patient an effec

Cleft lip and palate is a severe birth defect occurring approximately one in 800-1000 newborn infants. The incidence varies widely among races. Cleft lip and palate together account for approximately 50% of all cases whereas isolated cleft lip and isolated cleft palate occur in about 25% of cases. Many of these congenital anomalies appear to be genetically determined though the majority are of unknown causes or teratogenic influences. Presented here is a 3 day old infant with bilateral cleft lip and palate and phocomelia for whom a feeding obturator was made and delivered to facilitate feeding

This article aims to discuss the use of short dental implants in cleft patients, the construction of an implant-retained obturator and the use of a Createch milled titanium bar with three Locator overdenture attachments incorporated within the bar. CPD/Clinical Relevance: Implant-retained obturators offer a possible solution for treating patients with isolated cleft palate who are struggling with dentures due to the unfavourable soft and hard tissue profile. This paper demonstrates how to manage such patients and shows all the clinical and laboratory stages involved.

The rehabilitation of the cleft requires a multidisciplinary approach involving a pediatrician, surgeon, otolaryngologist, speech therapist, orthodontist, prosthodontist, and psychologist. The present case report illustrates the rehabilitation of a 12-day-old neonate with a cleft palate. Since the palatal arch of the neonate was very small, a feeding spoon was innovatively modified to obtain the impression. The obturator was fabricated on the same day and delivered in one appointment.

STATEMENT OF PROBLEM: When surgical treatment is not considered an option, prosthetic management of velopharyngeal insufficiency is carried out by means of a speech-aid prosthesis, whereas velopharyngeal incompetence is traditionally managed by a palatal lift prosthesis. Varying degrees of treatment success have been attributed to palatal lift prostheses.
PURPOSE: This study introduces the use of nasopharyngeal obturation instead of palatal elevation for the management of velopharyngeal incompetence.
METHODS: Seven patients afflicted by neurogenic velopharyngeal incompetence were treated wit

STATEMENT OF PROBLEM: Infants with a cleft palate often experience middle ear disease, a condition of great significance, and early prosthetic management of these infants is essential. However, any correlation between prosthetic palatal obturation and middle ear function is unclear.
PURPOSE: The purpose of this clinical trial was to assess whether prosthetic palatal obturation with a feeding appliance prevented or improved middle ear problems in infants with a cleft lip and palate.
MATERIAL AND METHODS: Ten infants with congenital cleft lip and palate (20 ears) were referred to the Prosthodo

It remains a significant challenge in prosthetic rehabilitation for combined hard and soft palate defects on account of two primary reasons. At first, conventional impressions can hardly get an accurate analogue and usually bring about a terrible experience for the patients. Secondly, conventional hard denture base resins used in obturator prostheses exhibit limitations in marginal sealing, undercut retention, and elastic buffering when in contact with the soft palate. This article presents a case where combined hard and soft palate defects were successfully and rapidly reconstructed by using

The fabrication of a Suersen obturator prosthesis with titanium framework is described in the case of a 43-year-old man with congenital absence of the soft palate. The patient revealed an open soft palate, bilateral cleft lip and palate, Angle Class III dental malocclusion, and anterior and posterior crossbite. He was treated with a multidisciplinary approach. A Dolder bar was fabricated between the maxillary canines, and telescopic crowns were prepared for maxillary premolars. Angle Class III tooth relation was readjusted to tooth-tooth relation; closure of the nasopharynx and oropharynx was

While dealing with a grave second wave of ongoing pandemic COVID-19, India also saw a surge in cases of COVID-19-associated mucormycosis, a systematic fungal infection caused by the Mucorales species. Mucormycosis is a highly angioinvasive, rapidly spreading fungal infection. In numerous cases of mucormycosis, bilateral subtotal maxillectomy was performed due to unpredictable and indefinable advancement of fungus clinically. Effective obturation of bilateral maxillectomy defect is a difficult task and as this is a relatively uncommon surgical problem, insufficient data are available on the con

Feeding a neonate baby with a complete cleft lip and palate is a problematic pursuit because of the communication between the oral and the nasal cavity and associated problems. This present case is of a 6-day-old underweight neonate with feeding difficulties due to the cleft palate. In this case report, simple, uncomplicated steps for the fabrication of a feeding obturator are explained to aid in the proper nourishment of neonates for definite corrective procedures in the future with overall growth.

The objective was to describe the use of a maxillary obturator supported by dental implants and ball abutments in an adult edentulous unilateral cleft patient. Adult edentulous cleft patients who have had no corrective surgeries or failed attempts at cleft repair in childhood suffer from oronasal communication resulting in hypernasal speech, dysphagia, and compromised masticatory ability. Tissue-supported obturator prostheses in such patients provide for compromised function due to instability and lack of retention. The treatment using fibromucosal-supported obturators is well established in t

Computer-aided design and computer-aided manufacturing technology has been applied to the fabrication of maxillary obturator frameworks, both directly and indirectly. However, with earlier techniques, it was not possible to accurately determine the position of the framework conforming to the palate, an issue that has been resolved in current fabrication methods. Using the patient's existing denture, prosthodontists can determine where the framework should be positioned in the defect area. This allows the obturator bulb to be hollowed, thereby reducing weight and making adjustment easier. The m

Nasal obturation using a removable medical device is a suitable treatment option for patients with velopharyngeal dysfunction. In the United Kingdom in recent years, the use of nasal obturation has increased because of the successful collaborations among the Speech and Language Therapy, Restorative Dentistry, and Maxillofacial departments. However, fabrication of the devices requires specialist skill and considerable time. A digital process which facilitates the rapid, cost effective production of a light, comfortable, unobtrusive nasal obturator is described.

BACKGROUND: The use of digital tools and 3D molding has become very common in dentistry today. However, there are few studies on the possibility of using 3D imaging tools for molding maxillofacial defects. In this review study, we examine articles that have used digital molding tools instead of conventional methods for molding maxillofacial defects.
MATERIALS AND METHODS: In this study, all articles related to keywords of "3D imaging tools", "CT scans", "Maxillofacial Prosthesis" were collected and reviewed by searching PubMed and ISI Web of Science until 2024. Then, the materials were classi

The treatment of patients with oral cancer is complex: a multidisciplinary approach needs to be taken and maxillofacial and oral surgeons, an oncologist, a prosthodontist should be included, and a psychologist is often needed. This case report describes the prosthetic rehabilitation of a patient after surgical removal of oral cancer with obturator prosthesis. Resection cavity was located in central part of the hard palate and the condition belonged to Aramany class 3 maxillary defects. The two-step impression technique of denture bearing area was used and the resection of cavity was performed.

Oronasal fistulas, a troublesome complication, often occur after cleft palate repair. Seventy-three patients in a series of 346 cases of cleft palate (21%) were found to have fistulas, most located at the junction of hard and soft palate (42%). Langenbeck's method of cleft palate repair resulted in more fistulas than Wardill's method. Fistulas occurred more frequently in bilateral clefts than in the unilateral type. Nasality was found to be the most common symptom in patients with oronasal fistulas. No treatment was needed for 17 patients, 10 were given obturators, and surgical repair was perf

A new technique for lining the superiorly based pharyngeal flap is described. After the soft palate in the oral mucosa is incised, the muscular velum is dissected laterally. A flap is then prepared on the nasal side of the palate with its pedicle on one side of the posterior edge of the palate. The pharyngeal flap is sutured into the donor defect of the palatal flap, which is reflected and used to line the pharyngeal flap. Both surface and bulk are added to the pharyngeal flap and the obturating effect is increased. This method is therefore especially indicated in patients with primary velopha

Solitary median maxillary central incisor (SMMCI) syndrome is a rare dental anomaly that affects 1:50,000 live births. SMMCI syndrome is characterized by the presence of a single central incisor located on the maxillary midline in both primary and permanent dentitions. It may occur as an isolated finding or in association with developmental defects and systemic involvement. Congenital anomalies associated with SMMCI syndrome can include short stature, mild forms of deviation in craniofacial morphology, mild to severe intellectual disability, congenital heart disease, and cleft lip and/or palat
