Chronic Obstructive Pulmonary Disease: A Palliative Medicine Review of the Disease, Its Therapies, and Drug Interactions.

Despite significant advances in treatment, chronic obstructive pulmonary disease (COPD) remains a chronic and progressive disease that frequently leads to premature mortality. COPD is associated with a constellation of significant symptoms, including dyspnea, cough, wheezing, pain, fatigue, anxiety, depression, and insomnia, and is associated with increased morbidity. Palliative care is appropriate to support these patients. However, historically, palliative care has focused on supporting patients with malignant disease, rather than progressive chronic diseases such as COPD. Therapies for COPD

[Nutrition and fluid management in palliative medicine: do food and drink keep body and soul together?].

Induction, implementation and continuation of an invasive nutrition or fluid administration in patients with advanced, life-limiting illnesses is an often controversial but also very emotionally discussed topic. This article summarizes the current state of knowledge based mainly on the European Society for Clinical Nutrition and Metabolism (ESPEN) guidelines and is intended as a simple guide for clinical practice. In the early phase of disease the induction of an invasive food and fluid administration may be indicated in order to prevent undernutrition and cachexia, to enhance compliance with

Point-of-care ultrasound in palliative medicine residency programs: report of a national survey and local workshop.

PURPOSE: Point-of-care ultrasound (POCUS) is used in various clinical specialties to improve diagnostic and procedural accuracy and has many potential applications in palliative care. However, it is unclear what is the current state of training and use of POCUS in palliative care in Canada. Our objectives were to understand the current state of training and use of POCUS in Canadian Palliative Medicine subspecialty residency programs and to develop and implement a local training workshop for POCUS in palliative care. METHODS: We surveyed residency program leaders regarding formal training and

Ambient Artificial Intelligence Scribes: A Pilot Survey of Perspectives on the Utility and Documentation Burden in Palliative Medicine.

BACKGROUND/OBJECTIVES: There is growing evidence to support ambient artificial intelligence (AI) scribes in healthcare to improve medical documentation by generating timely and comprehensive notes. Using the Plan-Do-Study-Act (PDSA) methodology, this study evaluated the utility and potential time savings of an ambient AI scribe, Scribeberry, (V2), in a palliative medicine outpatient setting, comparing it to the standard practice of dictation. METHODS: This prospective quality improvement study was conducted at an academic medical center by two palliative medicine resident physicians. Resident

Use of Artificial Hydration at the End of Life: A Survey of Australian and New Zealand Palliative Medicine Doctors.

BACKGROUND: Dying is ubiquitous, yet the optimal management of hydration in the terminal phase is undetermined. Palliative care (PC) doctors' practices may act as a de facto measure of the benefits and burdens of artificial hydration (AH) use. OBJECTIVE: To identify PC doctors' AH prescribing practices for imminently dying patients and possible influencing factors. METHODS: An online survey of doctors belonging to the Australian and New Zealand Society of Palliative Medicine. RESULTS: One hundred and thirty-six surveys were completed (30% response rate). AH use for patients in the prognosti

Prescription Monitoring Program Review Among Patients with Cancer Receiving Opioids at a Safety-Net Palliative Medicine Clinic.

INTRODUCTION: Prescription monitoring programs (PMPs) are commonly used to monitor non-medical opioid use (NMOU); however, the effectiveness of PMPs for identifying cancer patients with risk factors is not well known. METHODS: This study assessed the frequency and predictors of concerning PMP findings among cancer patients in a palliative care clinic and examined the ability of PMPs, clinical review, and urine drug testing to identify NMOU behaviors. This was a retrospective analysis of consecutive cancer patients seen by palliative care at a safety-net hospital over four years. Demographic,

Off-label drug use in palliative medicine: Delphi study for the consensus of evidence-based treatment recommendations.

BACKGROUND: Off-label use of drugs is an integral part of everyday clinical practice in palliative medicine. However, it is associated with many uncertainties, that is, drug therapy safety or legal issues including cost coverage. Healthcare professionals often lack time and resources for comprehensive literature search and patient-specific risk-benefit analyses. AIM: The aim of this project is to develop, evaluate and rate agreement/disagreement on treatment recommendations for off-label use in adult palliative medicine. DESIGN: Online Delphi study with two rounds each to rate agreement/disa

Treatment Recommendations for Urological Symptoms in Cancer Patients: Clinical Guidelines from the Japanese Society for Palliative Medicine.

BACKGROUND: Urological symptoms such as gross hematuria, lower and upper urinary tract symptoms, and bladder pain are common in and distressing for patients with advanced cancer. Although palliation of urological symptoms is important to improve the quality of life of cancer patients and their families and caregivers, clinical guidelines for managing urological symptoms in patients with cancer have not been published. METHODS: Following the formal guideline development process, the Japanese Society for Palliative Medicine (JSPM) developed comprehensive clinical guidelines for the management o

Responsive emergency assessment and community team: an acute palliative medicine virtual ward with emergency department in-reach.

OBJECTIVES: More than 50% of people who die in England and Wales use an ambulance at least once in their last 3 months of life, and around 50% attend the emergency department (ED). In Bradford, an estimated 1000 patients a year are not recognised as being within the last year of life and do not access palliative care services. We aimed to improve access to palliative care and reduce hospital admissions and days spent in hospital during the last year of life. METHODS: We created the Responsive Emergency Assessment and Community Team (REACT), comprised of both ED palliative medicine in-reach an

Managing Medical Emergencies With Palliative Medicine in a Patient With Metastatic Breast Carcinoma Receiving Only Supportive Care.

We report the case of a 56-year-old female patient with synchronous breast carcinoma on supportive care alone, who presented to the emergency department with acute medical emergencies such as altered sensorium, anasarca, vesicular skin lesions, and hemodynamic instability. Initial investigations revealed a myriad of life-threatening conditions, including suspected brain metastasis, diabetic ketoacidosis, acute kidney injury and sepsis. Balancing symptom control with appropriate medical interventions while respecting the goals of care remains a critical challenge. Emergency management involved

The influence of the hidden curriculum on the risk of burnout in junior doctors in a palliative medicine rotation - a qualitative exploratory study.

BACKGROUND: Palliative Care (PC) provides person-centred care for patients with life-limiting diseases and their families. Studies have shown that healthcare professionals delivering PC are predisposed to moral distress and burnout due to constant exposure to death and dying and aspects of the hidden curriculum (HC) through which culture and values are transmitted implicitly. However, there are limited studies focusing on the latter through the lens of junior doctors. Using the Ring Theory of Personhood (RToP) and the Krishna-Pisupati Model (KPM), which categorize and map conflicts between per
