Of 9,000 patients with bronchus carcinoma observed during 1958-1980, 3.041 (33%) underwent resection. Although in principle the standard operations are lobectomy for the peripheral and bilobectomy or pneumonectomy for the central carcinoma, parenchyma-saving operations such as clamping or segmental resection or main bronchus resection are justified under certain conditions; however, here all regional lymph nodes have to be removed again. Oncological pain in palliative care is common. This article presents a 61-year-old man with a Pancoast tumor which caused severe pain and impaired his quality of life. Multimodal analgesia was not effective due to a limited analgesic effect and side effects. After one year the patient underwent a percutaneous cordotomy which resulted in a complete and long-term pain relief and an improvement of social and physical functioning. A 74-year-old man presented with a left superior sulcus tumour invading the subclavian artery. After induction chemoradiotherapy, he underwent a vertebral artery reconstruction in addition to the subclavian artery reconstruction via a transmanubrial approach and video-assisted thoracoscopic left upper lobectomy. The final pathology was ypT4N0M0 adenocarcinoma. The technical aspects and indication for a vertebral artery reconstruction are discussed in this article.

BACKGROUND: The evolution of therapy in 105 patients with superior sulcus (Pancoast) tumor over the past 42 years was reviewed.
METHODS: There were 82 men and 23 women aged 30 to 75 years. The Pierre-Marie-Bamberger syndrome is a rare paraneoplastic syndrome caused by bronchial carcinoma. Typical signs are symmetric periostoses on the diaphyses of the long tubular bones, clubbed fingers and toes with eye-glass shape of the nails, neuro-vegetative disturbances and dysproteinemia. We report a 37-year-old patient with long-term nicotine abusus, who attracted attention by symptoms of a Pierre-Marie-Bamberger syndrome. Further diagnostics revealed a tumor in the apex of the left lung. After lobectomy of the upper lobe of the left lung the symptoms are completly disappeared.

Twelve patients with non-small cell lung cancer had tumors that were adherent to the vertebral column and clinically suspected of invading the bone. All were free of mediastinal node involvement as assessed by pretreatment mediastinoscopy. All received 3000 rads of preoperative radiation followed by en bloc resection of the lung and a tangential portion of the involved vertebral bodies.
