Long-term proton pump inhibitor use and risk of dementia: a focused review on pantoprazole.

BACKGROUND: Proton pump inhibitors (PPIs) are widely prescribed for acid-related disorders, yet concerns about their long-term effects on cognitive function persist. Emerging evidence suggests a possible association between prolonged PPI use and increased dementia risk, though findings remain inconclusive. This narrative review focuses on pantoprazole, a PPI with unique pharmacokinetics, to assess its potential role in dementia development. METHODS: A systematic search was conducted using PubMed and PubMed Central with keywords including "dementia," "proton pump inhibitors," "Alzheimer's dise

Pantoprazole suppresses carcinogenesis and growth of hepatocellular carcinoma by inhibiting glycolysis and Na+/H+ exchange.

Hepatocellular carcinoma (HCC) is one of the deadliest cancers. The prevention and therapy for this deadly disease remain a global medical challenge. In this study, we investigated the effect of pantoprazole (PPZ) on the carcinogenesis and growth of HCC. Both diethylnitrosamine (DEN) plus CCl4-induced and DEN plus high fat diet (HFD)-induced HCC models in mice were established. Cytokines and cell proliferation-associated gene in the liver tissues of mice and HCC cells were analyzed. Cellular glycolysis and Na+/H+ exchange activity were measured. The preventive administration of pantoprazole (P

Routine application of the proton-pump inhibitor pantoprazole in patients with gastric lymphoma undergoing chemotherapy.

OBJECTIVE: The stomach is the most common site of origin for extranodal lymphomas. While resection has played a major part in the management of such patients in the past, in recent years there has been a change towards organ-conserving therapies. However, the risk of perforation and bleeding in patients undergoing organ-conserving therapy has been used as an argument against primary application of chemotherapy. In this article, we present our experience with the prophylactic use of oral pantoprazole. MATERIAL AND METHODS: All patients undergoing chemotherapy for gastric lymphoma at our instit

The impact of Helicobacter pylori resistance on the efficacy of a short course pantoprazole based triple therapy.

BACKGROUND: Many of the currently used Helicobacter pylori eradication regimens fail to cure the infection due to either antimicrobial resistance or poor patient compliance. Those patients will remain at risk of developing potentially severe complications of peptic ulcer disease. AIM: We studied the impact of the antimicrobial resistance on the efficacy of a short course pantoprazole based triple therapy in a single-center pilot study. METHODS: Forty previously untreated adult patients (age range 20 to 75 years, 14 males) infected with Helicobacter pylori and with inactive or healing duodena

Pantoprazole: an update of its pharmacological properties and therapeutic use in the management of acid-related disorders.

Pantoprazole (Protonix) is an irreversible proton pump inhibitor (PPI) that reduces gastric acid secretion. In combination with two antimicrobial agents (most commonly metronidazole, clarithromycin or amoxicillin) for 6-14 days, pantoprazole 40 mg twice daily produced Helicobacter pylori eradication rates of 71-93.8% (intent-to-treat [ITT] or modified ITT analysis) in patients without known antibacterial resistance. Pantoprazole-containing triple therapy was at least as effective as omeprazole- and similar in efficacy to lansoprazole-containing triple therapy in large trials. In the treatment

Eradication of Helicobacter pylori with pantoprazole and two antibiotics: a comparison of two short-term regimens.

BACKGROUND: High rates of Helicobacter pylori eradication can be achieved by combining proton pump inhibitors with two antibiotics. However, in the search for an optimal therapy a direct comparison of different regimens is necessary. METHODS: For this open study, 331 patients with duodenal ulcer were screened and randomly allocated to either pantoprazole 40 mg b.d., clarithromycin 500 mg b.d., and metronidazole 500 mg b.d. (PCM) or pantoprazole 40 mg b.d., amoxycillin 1000 mg b.d., and clarithromycin 500 mg b.d. (PAC) for 7 days. Both combinations were followed by a 7-day therapy with pantopr

Pantoprazole-induced anaphylactic asthma and Type I Kounis syndrome: a case report and literature review.

BACKGROUND: Proton pump inhibitors (PPIs) are among the most frequently prescribed medications and are generally considered to have a favourable safety profile. Nevertheless, immediate hypersensitivity reactions may occur and, in exceptional circumstances, can precipitate Kounis syndrome (KS), an acute coronary syndrome resulting from the activation of inflammatory pathways. We describe an elderly patient who developed severe anaphylaxis with Type I KS shortly after intravenous pantoprazole administration in the presence of an unsuspected myocardial bridge, and discuss the potential clinical i

Severe symptomatic hypocalcemia with neuromuscular hyperexcitability associated with long-term pantoprazole use: a case report.

BACKGROUND: Hypocalcemia is a common electrolyte abnormality; however, severe symptomatic hypocalcemia presenting with neuromuscular hyperexcitability and cardiac manifestations is an uncommon clinical condition requiring prompt recognition and management. Long-term proton pump inhibitor (PPI) therapy has been associated with disturbances in mineral metabolism, particularly hypomagnesemia, which may contribute to impaired calcium homeostasis. CASE PRESENTATION: A 54-year-old woman from a remote rural area was referred to our hospital after developing progressive weakness, paresthesia, and pai

Pantoprazole reduces the size of postbanding ulcers after variceal band ligation: a randomized, controlled trial.

Elective esophageal variceal ligation (EVL) is performed to decrease the risk of variceal hemorrhage. Side effects of EVL include hemorrhage, chest pain, dysphagia, and odynophagia. Because gastric acid may exacerbate postbanding ulcers and delay healing, proton pump inhibition may decrease side effects associated with EVL. The aim of this study was to assess the efficacy of pantoprazole, a proton pump inhibitor, as an adjunct to elective EVL. We performed a double-blinded, randomized, placebo-controlled trial of pantoprazole after elective EVL. Subjects in the pantoprazole arm received 40 mg

Dose-range finding study with the proton pump inhibitor pantoprazole in acute duodenal ulcer patients.

Pantoprazole is a newly developed benzimidazole derivative with strong inhibitory actions on gastric acid secretion by blocking H(+)-K(+)-ATPase. This randomized double-blind multicenter trial investigated the efficacy of 20 mg, 40 mg and 80 mg pantoprazole o.m. on ulcer healing and symptomatic relief in 219 out-patients with endoscopically assessed acute duodenal ulcer. After 2 weeks complete ulcer healing was achieved in 58%, 89% and 82% of the patients with 20 mg, 40 mg and 80 mg pantoprazole o.m., respectively. After 4 weeks, corresponding figures were 93%, 99% and 100%; the difference of
