Anti-SOX1 antibody-positive paraneoplastic neurological syndrome presenting with Lambert-Eaton myasthenic syndrome and small cell lung cancer: A case report.

Paraneoplastic neurological syndromes (PNS) are rare disorders affecting any part of the central, peripheral or autonomic nervous system that occur in association with cancer. Among cancer patients, less than 1% overall develop PNS. Anti-SOX1 antibodies' positive paraneoplastic neurological disorders are rare and are usually associated with small cell lung cancer (SCLC). Here, we report a case of a 61-year-old male patient who presented with an unusual anti-SOX1 positive PNS. The right tibialis anterior showed noticeable low-amplitude motor unit potentials and high amplitude motor potentials i

[Progressive dysautonomia as initial manifestation of anti-Hu antibody-related syndrome].

INTRODUCTION: The anti-Hu antibody are associated with central nervous system paraneoplastic syndromes such as sensory neuropathy, cerebellar ataxia or limbic encephalitis. The isolated autonomic dysfunction as the presenting manifestation of anti-Hu antibody-related paraneoplastic syndrome is extremely infrequent. CASE REPORT: A 68 year-old male presented with a two-month history of severe constipation following of orthostatic hypotension, impotence, urinary retention and intestinal pseudo-obstruction as manifestations of progressive dysautonomia. Four months after the onset of these symptom

Atypical Presentation of Gall Bladder Adenocarcinoma as Paraneoplastic Myeloneuropathy.

Paraneoplastic neurologic syndromes are a rare, heterogeneous group of complications associated with malignancy, unrelated to direct tumor invasion or metastasis. They are the remote effects of a malignancy owing to immune-mediated reaction toward the antigens that are common to both the malignant and normal cells. Few paraneoplastic syndromes are reported with gall bladder cancer. However, neurological symptoms are infrequent. Here we share an interesting case of adenocarcinoma of gall bladder with initial presentation as a paraneoplastic noncompressing myeloneuropathy.

Paraneoplastic syndromes associated with classic Hodgkin lymphoma, a systematic literature review.

PNS are uncommon manifestations of cancer. The current literature about these syndromes in the setting of cHL is disintegrated. A systematic literature review of all published literature was conducted. One hundred twenty-eight patients from 115 publications met the inclusion/exclusion criteria. Eight-five patients were of the NS subtype (66.4%). The most frequent clinical presentation of the PNS was CNS manifestation (25.8%). The majority of patients were diagnosed with the cHL and PNS simultaneously (42.2%). In 33.6% of patients, the lymphoma diagnosis preceded the PNS diagnosis. In 16.4% of

Autoimmune encephalitis in the elderly: who to test and what to test for.

The awareness and understanding of autoimmune encephalitis are blossoming in neurology, and patients are being diagnosed and successfully treated with immunotherapy. The diverse symptomatology associated with autoimmune encephalitis means that patients may present initially to mental health services, which are, as yet, less well equipped to identify and investigate such phenomena. Older adult mental health services are used to managing complexity, but the range of pathologies presenting with unusual symptoms that may mimic autoimmune encephalitis is wide and there is no clear guidance as to wh

Autonomic paraneoplastic neurological syndromes.

Autonomic paraneoplastic neurological syndromes (PNS) typically present as chronic gastrointestinal pseudo-obstruction or orthostatic hypotension and usually occur in association with other PNS rather than in isolation. Although rare, they are often debilitating, sometimes fatal, and probably seriously underdiagnosed. Here, we discuss the clinical, immunological and oncological features of these syndromes and review the molecular and cellular mechanism that may underlie the triggering and maintenance of their autoimmune pathogenesis.

Electrodiagnostic Assessment of Hyperexcitable Nerve Disorders.

Peripheral nerve hyperexcitability (PNH) typically presents with complaints of muscle twitching, cramps, and muscle stiffness. Symptoms and signs indicating central and/or autonomic nervous system dysfunction also may be reported. An electroclinical spectrum exists, spanning from the milder cramp-fasciculation syndrome to more severe syndromes characterized by continuous muscle fiber activity. It is important to recognize that PNH may be an autoimmune phenomenon associated with antibodies targeting proteins of the voltage-gated potassium channel-complex and, in some patients, a paraneoplastic

(18)F-FDG PET/CT and MRI findings in a patient with anti-GABA(B) receptor encephalitis.

Autoimmune synaptic encephalitis can occur as paraneoplastic neurological syndromes, which are dysfunctions of the nervous system in cancer patients. One such rare but treatable form is associated with GABAB (γ-aminobutyric acid-B) receptor antibody. We report a 55-year-old man with small cell lung cancer who presented with 3 weeks of progressive seizures, memory impairment, and behavioral disorder. His cerebrospinal fluid anti-GABAB receptor antibody titer was elevated. F-FDG PET/CT revealed pronounced medial temporal hypermetabolism with gross hypometabolism in the rest of the brain. There w

Double trouble: para-neoplastic anti-PCA-2 and CRMP-5-mediated small fibre neuropathy followed by chorea associated with small cell lung cancer and evolving radiological features.

Patients with Purkinje cell cytoplasmic autoantibody type 2 (PCA-2) and collapsin response-mediator protein-5 (CRMP-5) autoantibody can present with multifocal elements of encephalomyeloneuropathy. Except for an anecdotal report, case descriptions of paraneoplastic small fibre neuropathy are lacking. We report paraneoplastic small fibre neuropathy followed by chorea associated with small cell lung cancer. A man aged 57 years with a 35 pack-year smoking history presented with painless subacute paresthesia and weight fluctuation. A non-length-dependent small fibre neuropathy was confirmed by ski

Case Report: Anti-Yo antibody mediated paraneoplastic cerebellar degeneration in a patient with squamous cell lung carcinoma.

Paraneoplastic cerebellar degeneration (PCD) is a heterogeneous group of neurologic syndromes associated with primary tumors. It is postulated that the immune system targets a tumor antigen that is also expressed endogenously in the nervous system. The majority of these patients are diagnosed with breast cancer or gynecological cancer, while it is exceedingly rare in lung squamous cell carcinoma (LUSC) patients. Here we reported a rare case of anti-Yo antibody-positive PCD in a patient with LUSC and got successfully treated via immunotherapy and oncological treatment. The patient's ataxia symp

Neuromuscular symptoms in cancer patients.

The neuromuscular system is often affected in patients with cancer. This can be due to a direct effect of cancer, neurotoxic treatment, radiation therapy or paraneoplastic phenomena. This review focuses on the sites of involvement, such as cranial nerves, nerve roots, and plexus, mono- and polyneuropathies disorders at neuromuscular transmission, and muscle disease. At each site of the peripheral nervous system, the most common types of involvement and therapeutic issues are discussed. Knowledge of neuromuscular symptoms is important for medical oncologists, neurologists and neuro-oncologist a

[Paraneoplastic neuropathies].

RARE AND INAUGURAL: Paraneoplastic neuropathies (PN) are rare, affecting approximately 4-5% of patients with cancer. Their diagnosis is difficult because the clinical picture is nonspecific and because the neuropathy precedes the discovery of the cancer in a majority of patients. SUBACUTE SENSORY NEUROPATHY (SSN): Initially described by Denny-Brown, subacute sensory neuropathy is characterized by a severe inflammatory loss of the dorsal roots ganglia. The clinical hallmark is a severe, asymmetric sensory neuropathy progressing over a few weeks to months until the patient is bedridden. The unde

Myasthenia-like paraneoplastic syndrome with multiple cranial nerve tumor infiltration: A case report and literature review.

RATIONALE: Approximately 0.001% of patients with cancer have paraneoplastic nerve system syndrome, which can affect the central nervous system, neuromuscular junction, or peripheral nervous system. Although myasthenia gravis (MG) may exist as a thymic paraneoplastic syndrome (PNPS), its association with primary lung cancer remains unknown. PATIENT CONCERNS: A 55-year-old female presented with slurred speech, weakness in chewing, sporadic difficulty in swallowing, and weakness in both lower limbs for half a year. DIAGNOSES: Based on cerebrospinal fluid and electromyography findings, we presen

[Plasmapheresis in central nervous system disorders].

Therapeutic plasmapheresis (TPE) has an established role in disorders of the peripheral nervous system, but its use in disorders of the central nervous system (CNS) does not rely upon evidence-based data. Nevertheless, TPE is currently used in severe acute forms of demyelinating disease (multiple sclerosis/acute encephalomyelitis) unresponsive to corticosteroids. Recently, antibodies against the water channel aquaporin-4 have been detected in patients affected by neuromyelitis optica (Devic syndrome) and their pathogenetic role has been demonstrated, supporting the use of TPE in this disease.

[Paraneoplastic autoimmune disorders].

Paraneoplastic syndrome is defined as tumor-associated symptoms and signs not related to the physical effects of primary or metastatic tumors. The mechanisms of this syndrome include the production of bioactive soluble factors by tumor cells and autoimmune diseases elicited by the immune responses against tumors. Production of bioactive soluble factors causes endocrinologic symptoms. The paraneoplastic autoimmune process may affect the nervous system, cutaneous tissue, musculoskeletal system, hematopoietic cells or kidneys. Paraneoplastic rheumatic diseases show symptoms similar to inflammator

Paraneoplastic Intracellular Neuropathies: A Narrative Review.

Paraneoplastic neuropathies are rare, immune-mediated disorders caused by an antitumor immune response that cross-reacts with components of the peripheral nervous system, such as peripheral nerves and dorsal root ganglia. Unlike syndromes associated with antibodies targeting neuronal surface or extracellular antigens, in which auto-antibodies are considered directly pathogenic, neurological syndromes associated with intracellular onconeural antigens are predominantly mediated by cytotoxic T-cell responses. Accurate diagnosis of these syndromes relies on early identification of high-risk clinic
