OBJECTIVE: This study aimed to investigate the clinical and biomechanical effect of medial fixed-bearing unicompartmental knee arthroplasty (UKA) on the patellofemoral joint (PFJ). However, whether patellofemoral joint (PFJ) degeneration should be considered a contraindication for UKA remains controversial. In addition, the biomechanical effects of UKA on PFJ stress distribution have not been fully elucidated.
METHOD: Thirty-two patients (35 knees) who underwent medial fixed-bearing UKA were included with a mean of 4.5 years follow-up. The clinical parameters including the Oxford Knee Score (

BACKGROUND: Recent studies have shown that gait retraining can significantly affect the biomechanical load on the patellofemoral joint. It is of great significance to understand the effects of these running technique characteristics on patellofemoral joint load for developing effective prevention and rehabilitation strategies for injury.
METHODS: A systematic review was conducted on the relevant studies from PubMed, Web of Science, EBSCO, CNKI, and Wanfang database, and the data were synthesized. Data including patellofemoral joint contact force (PFCF) and patellofemoral joint stress (PFJS) w

BACKGROUND: Traditional running shoes with heel-to-toe drops is thought to be a contributor to increased patellofemoral joint stress, which is proposed as a mechanism of patellofemoral pain.
RESEARCH QUESTION: Is there an increase in patellofemoral joint stress when running in shoes with drops compared to running in shoes without a drop?
METHODS: Lower limbs kinematics and ground reaction force were collected from eighteen healthy runners during over-ground running in shoes with 15 mm, 10 mm, 5 mm drops, and without a drop. Patellofemoral joint force and stress were calculated from the kinem

Patellofemoral joint instability is a common clinical problem. However, little quantitative data are available describing the stability characteristics of this joint. We measured the stability of the patella against both lateral and medial displacements across a range of knee flexion angles while the quadriceps were loaded physiologically. For eight fresh-frozen knee specimens a materials testing machine was used to displace the patella 10 mm laterally and 10 mm medially while measuring the required force, with 175 N quadriceps tension. The patella was connected via a ball-bearing patellar mou

This was a quantitative study of the major anatomical structures associated with instability of the patellofemoral joint: the quadriceps muscles and the femoral trochlear groove. The attachments of the muscles to the patella, their lines of action, and their relative sizes (physiological cross-sectional areas) were found. On the basis of the physiological cross-sectional areas, it was estimated that the central muscles-the rectus femoris and vastus intermedius-contributed 35% of the quadriceps strength, with 40% from the vastus lateralis and 25% from the vastus medialis. The vastus lateralis h

The association between different knee exertion injuries and lower limb malalignments was studied in 121 male athletes with knee exertion injuries and in 20 male athletes without knee symptoms. The associations between different malalignments was also analyzed. Leg length inequality (LLI), knee valgus, and different indexes of patellofemoral joint congruence were measured using radiological methods. Male athletes with patellar apicitis (n = 59) had significantly more LLI (P less than 0.001) and patella alta (P less than 0.05) than athletes without knee symptoms. The knee valgus was of the same

BACKGROUND: Patellar instability is a common injury among young, active individuals resulting in increased risk for future patellofemoral osteoarthritis. The purpose of this study is to quantify the effect of gait pattern on patellofemoral joint stress in those with patellar instability.
METHODS: Individuals with patellar instability (n = 17) completed an instrumented gait analysis. A mathematical model, with sagittal plane knee angle and knee moment as inputs, was used to estimate patellofemoral joint stress during stance. Individuals were grouped by presenting with an extension dominant kne

Most knee osteoarthritis and meniscectomy studies focus on osteoarthritis in the tibiofemoral joint and ignore the patellofemoral joint. This study aims to assess the long-term effects of total meniscectomy on the patellofemoral joint. To our knowledge, this is the only study of osteoarthritis in the patellofemoral joint following meniscectomy that extends to a 40-year follow-up period. Twenty-two patients with osteoarthritis were evaluated at a mean of 40 years post-meniscectomy using standardised weight-bearing radiographs of the operated and non-operated knees. Patellofemoral joint osteoart

The patellofemoral joint constitutes a complex anatomical and functional entity. The tensile force of the quadriceps femoris muscle is transmitted through the patella and patellar ligament onto the tibial tuberosity. This particular three-dimensional arrangement increases the torsional moment acting on the knee joint. Dynamic alignment of the patella is determined by trochlear geometry and is supported by active muscular and passive connective tissue stabilizers. In addition to the retinaculum of the patella, the medial patellofemoral ligament is attracting increasing clinical attention. Multi

Traditional conservative treatment for patellar disorders is successful in about 80 percents of cases. We introduced two new conservative treatment protocols for patellar pathology in order to further improve the success rate. The first protocol consisted of high load/low repetition quadriceps femoris training (10 patients) while the second enclosed selective electrostimulation of vastus medialis muscle (7 patients). Results were evaluated clinically and neurophysiologically. High load/low repetition training resulted in significant increase of maximal voluntary contraction of quadriceps muscl

Estimating internal forces at common injury locations may identify injury-prone runners but requires in-lab gait analysis. Consumer-grade wearable-sensors track key gait metrics, which may enable prediction of internal forces during real-world running. This study aimed to use gait metrics from consumer-grade wearable-sensors to predict Achilles tendon (AT) and patellofemoral joint (PFJ) forces during running. Fifty-five runners completed a 38-minute run at 70-125% of preferred speed on an instrumented treadmill while equipped with a consumer-grade foot-pod and chest strap. Motion capture data

PURPOSE: Generalized joint hypermobility (GJH) has frequently been described as a risk factor for patellofemoral instability; however, only a few primary research studies have demonstrated any evidence of a relationship. The primary purpose of this study was to determine if isolated proximal soft tissue stabilization for patellofemoral instability is as successful in patients with GJH compared those without, as measured by disease-specific quality-of-life (QOL), symptom scores and functional outcomes. The secondary purpose was to determine if clinical and patient-reported outcomes were influen

Selective strengthening of the vastus medialis (VM) muscle is a conservative treatment used to address some patellofemoral joint (PFJ) problems. The objective of this study was to examine the effects of varying VM strength on PFJ kinematics and contact pressures and areas. We tested five fresh-frozen cadaveric knees using a custom knee jig, which permits the simulation of physiologic quadriceps loading while also allowing the VM force to be varied. PFJ kinematics were measured with a magnetic tracking device. PFJ contact pressures and areas were measured with Fuji pressure-sensitive film. For
