Cyclodextrin overcomes deficient lysosome-to-endoplasmic reticulum transport of cholesterol in Niemann-Pick type C cells.

A handoff model has been proposed to explain the egress from lysosomes of cholesterol derived from receptor-mediated endocytosis of LDL. Cholesterol is first bound by soluble Niemann-Pick C2 (NPC2) protein, which hands off the cholesterol to the N-terminal domain of membrane-bound NPC1. Cells lacking NPC1 or NPC2 accumulate LDL-derived cholesterol in lysosomes and fail to deliver LDL cholesterol to the endoplasmic reticulum (ER) for esterification by acyl-CoA acyltransferase (ACAT) and for inhibition of sterol regulatory element-binding protein cleavage. Here, we support this model by showing

Comparing methadone policy and practice in France and the US: Implications for US policy reform.

Despite being among the most effective treatments for opioid use disorder, methadone is largely unavailable in the United States, due primarily to federal and other policies that limit its availability and regulate clinical decisions about doses, visit frequency, and drug testing. There is unprecedented momentum to change decades-old US methadone policies. Yet uncertainty remains as to whether reforms will be adopted and how policies will be implemented. France has among the best methadone access and lowest overdose death rates worldwide. 87 % of French people with opioid use disorder receive

Critical Cardiopulmonary Event Series: Four Simulations for Pediatric ICU Fellows, Critical Care Nurses, and Pediatric Residents.

INTRODUCTION: Critical cardiopulmonary events arising from congenital or acquired heart diseases are infrequent in some pediatric critical care units but can be associated with significant morbidity and mortality when encountered. We developed four simulation cases for interprofessional pediatric critical care teams (fellows, residents, and nurses) to provide participants with high-acuity cardiopulmonary scenarios in safe learning environments. The included cases were coarctation of the aorta, Kawasaki disease, myocarditis, and tetralogy of Fallot. METHODS: The simulations were typically 15 m

The high yield of 1-year colonoscopy after resection: is it the handoff?

BACKGROUND: This study aimed to determine the diagnostic yield of colonoscopy 1 year after colorectal cancer (CRC) resection based on whether the index colonoscopy was performed by the operating surgeon. METHODS: All patients undergoing surgery for colorectal cancer by two colon and rectal surgeons at a university hospital from 1991 to 2005 were identified from the tumor registry. Those patients with a complete preoperative colonoscopy by any physician and a 1-year follow-up examination by the operating surgeon were selected for the study population. Family history of colorectal cancer, tumor

Screening for unhealthy alcohol and other drug use by health educators: do primary care clinicians document screening results?

OBJECTIVES: Health educators are increasingly being used to deliver preventive care including screening and brief intervention (SBI) for unhealthy substance use (SU) (alcohol or drug). There are few data, however, about the "handoff" of information from health educator to primary care clinician (PCC). Among patients identified with unhealthy SU and counseled by health educators, the objective of this study was to examine (1) the proportion of PCC notes with documentation of SBI and (2) the spectrum of SU not documented by PCCs. METHODS: Before the PCC-patient encounter, health educators scree

Generic Engine for Module Software Task Orchestration and Negotiation (GEMSTONe): A Multi-Stage Workflow System for Orchestrating Virtual Imaging Trials.

Virtual imaging trials rely on complex, multi-stage computational workflows spanning a large array of software systems for patient modeling, image simulation, and analysis. Integrating heterogeneous software modules into efficient, reproducible, and scalable workflows remains challenging: ad hoc conventions, manual handoffs between developers, and inconsistent file formats often lead to fragile, error-prone results. To address these challenges, we developed GEMSTONe, a Generic Engine for Modular Software Task Orchestration and Negotiation, which decouples domain-specific module computation fro

Rationale and development of a prehospital goal-directed bundle of care to prevent rearrest after return of spontaneous circulation.

In patients with out-of-hospital cardiac arrest (OHCA) who attain return of spontaneous circulation (ROSC), rearrest while in the prehospital setting represents a significant barrier to survival. To date, there are limited data to guide prehospital emergency medical services (EMS) management immediately following successful resuscitation resulting in ROSC and prior to handoff in the emergency department. Post-ROSC care encompasses a multifaceted approach including hemodynamic optimization, airway management, oxygenation, and ventilation. We sought to develop an evidenced-based, goal-directed b

Contributory factors and potentially avoidable neonatal encephalopathy associated with perinatal asphyxia.

BACKGROUND: The recently published monograph, Neonatal encephalopathy and neurologic outcome, from the American College of Obstetricians and Gynecologists calls for a root cause analysis to identify components of care that contributed to cases of neonatal encephalopathy to design better practices, surveillance mechanisms, and systems. All cases of infants born in New Zealand with moderate and severe neonatal encephalopathy were reported to the New Zealand Perinatal and Maternal Mortality Review Committee from 2010. A national clinical review of these individual cases has not previously been un

Impact of Comprehensive STEMI Protocol on Process Metrics and Clinical Outcomes in STEMI Patients With Nonsystem Delays.

Hospital-wide strategies improve outcomes in STEMI, yet their impact on patients with nonsystem delays (NSD) to primary PCI remains unknown. This study evaluated the effect of a 4-step comprehensive STEMI protocol (CSP) on outcomes in this high-risk population. This observational cohort analysis included STEMI patients with NSD as defined by the ACC National Cardiovascular Data Registry CathPCI Registry v5.0 criteria, which included difficult vascular access, difficulty crossing the culprit lesion, cardiac arrest and/or need for intubation before PCI, patient delays in providing consent for PC

A Novel Use of the "3-Day Rule": Post-discharge Methadone Dosing in the Emergency Department.

INTRODUCTION: Methadone is a medically necessary and lifesaving medication for many patients with opioid use disorder. To adequately address these patients' needs, methadone should be offered in the hospital, but barriers exist that limit its continuation upon discharge. The code of federal regulations allows for methadone dosing as an inpatient as well as outpatient dispensing for up to three days to facilitate linkage to treatment. As a quality initiative, we created a new workflow for discharging patients on methadone to return to the emergency department (ED) for uninterrupted dosing. MET

Getting on the Same Page: A Quality Improvement Project to Enhance Nurse-to-Resident Communications and Reduce Overnight Sleep Interruptions.

BACKGROUND: Many residency programs utilize "home call" residents who answer hospital communications and place orders from home. Often, residents are required to live nearby and arrive in-person if needed. Residents work normal daytime work hours while on home call, which can last several nights. This disrupts sleep and creates the potential for increased resident fatigue and patient safety issues. OBJECTIVE: To implement and evaluate a novel program to reduce non-urgent overnight pages from nurses to home call physical medicine and rehabilitation (PM&R) residents between 11 pm and 6 am. MET

Diagnostic Orphaning: The dermatologist and the uninsured patients: What happens after the biopsy.

Uninsured patients may receive an accurate dermatologic diagnosis and appropriate recommendation, yet remain unable to obtain treatment, because diagnostic and therapeutic services are often delivered by separate entities. We term this "diagnostic orphaning": the patient has a diagnosis, a clinician, and perhaps a referral, but no feasible route for care. This contribution proposes an adequate pathway, as an ethical standard for determining when efforts to address financial barriers constitute an adequate treatment plan.??? An adequate pathway preserves clinical integrity, is reasonably feasib

Integrating preexposure prophylaxis into gynecologic care: determinants and strategies.

BACKGROUND: Preexposure prophylaxis (PrEP) is highly effective for HIV prevention, yet cisgender women remain underrepresented among PrEP users relative to their HIV burden in the United States. Obstetrics and gynecology (OB/GYN) settings-where many women receive routine sexual and reproductive health care-represent a promising but underutilized venue for PrEP delivery. Despite professional guidance endorsing PrEP discussion and prescribing in OB/GYN practice, implementation remains limited. METHODS: We conducted a qualitative implementation study with two linked components in an academic OB/

Medical and nonmedical factors influencing utilization of delayed pushing in the second stage.

OBJECTIVE: To evaluate factors impacting selection to delayed pushing in the second stage of labor. STUDY DESIGN: This case-control study was a secondary analysis of a large retrospective cohort study. Cases included women who delayed pushing for 60 minutes or more in the second stage of labor. Controls began pushing prior to 60 minutes from the time of diagnosis of complete dilation. Demographic, labor, and nonmedical factors were compared among cases and controls. Logistic regression modeling was used to identify factors independently associated with delayed pushing. RESULTS: We identified

Identifying Discrepancies in Gestational Diabetes Mellitus Postpartum Follow-Up During Care Transitions.

BACKGROUND: Gestational diabetes mellitus (GDM) is a common complication of pregnancy that carries lasting risks. For women who have a history of GDM, the risk of developing type 2 diabetes mellitus (T2DM) increases by 50 to 60%. To monitor and prevent the development of T2DM, guidelines suggest screening those with a history of GDM at 6-weeks and 3-years post-delivery. For patients with abnormal lab results at the 6-week mark, it is advised to complete lab work sooner, at the 1-year mark. Data suggests that these guidelines are not being followed globally, and most do not receive adequate scr

Three kinds of proactive risk analyses for health care.

BACKGROUND: In health care, proactive risk assessment usually takes the form of Failure Mode and Effects Analysis (FMEA). An applied research firm, four community hospitals, and a community health care alliance in south-central Washington State--as members of the Tri-Cities Patient Safety Coalition (TCPSC)--used proactive risk assessment methods such as event tree analysis (ETA) and hazard identification to assess the risk of adverse events associated with a process or system. Object-Centric Process Mining of Laparoscopic Cholecystectomy Workflows: Identifying Bottlenecks and Optimization Opportunities.

This study applies object-centric process mining (OCPM) techniques to analyze laparoscopic cholecystectomy (LC) procedures. Traditional process mining techniques are limited in analyzing workflows involving multiple interacting entities (patients, surgeries, anesthesia physical status, etc.).This study aimed to construct an Object-Centric Event Log (OCEL) from LC procedures, discover multientity process patterns, identify workflow bottlenecks, and analyze how patient complexity affects perioperative dynamics. An OCEL representing 1,186 LC cases (1,108 performed, 78 cancelled) from the Universit
