Relationship between patient activation and utilisation of health care and harm reduction services among people who inject drugs in Baltimore, Maryland.

INTRODUCTION: Given structural barriers, access to services is key for preventing drug-related harms and managing chronic disease among people who inject drugs (PWID). The Patient Activation Measure (PAM), a validated scale to assess self-efficacy in navigating one's own health care, was operationalised to improve service utilisation and outcomes but has not been assessed among PWID. We characterised PAM and its association with healthcare and harm reduction utilisation among PWID in the AIDS Linked to IntraVenous Experience cohort in Baltimore. METHODS: From 2019 to 2020, participants comple

First do no harm: A patient-reported survey of split skin graft donor site morbidities following thin and super-thin graft harvest.

The split-thickness skin graft (STSG) donor site is the commonest used during burn surgery which has its own complications and as such the focus should be on minimizing it. Modifications to practice in our unit which we believe aid this include limiting the amount of STSG taken and the harvest of super-thin STSGs, with 0.003-0.005 in. (0.08-0.13 mm) being the commonest dermatome settings used. A patient-reported survey via a mobile phone link to a questionnaire was sent to 250 patients who had a STSG for an acute burn between 1st August 2020 and 31st July 2021. Patient demographics were collec

E-cigarettes for harm reduction among COPD patients who smoke: A pilot investigation.

OBJECTIVE: COPD patients who smoke struggle to quit and have higher rates of lung cancer relative to adults who smoke without COPD. E-cigarettes may be a potential harm reduction tool. This secondary analysis examined the impact of e-cigarette switching on smoking and associated health impacts among COPD patients who smoke. METHODS: COPD patients who smoke and were not motivated to quit (n = 46) completed a 12-week pilot e-cigarette switching trial. At week 12, participants were categorized by use category into complete/predominant switchers (past 7-day e-cigarette use and ≥50% reduction in c

Does longer brace wear harm quality of life in patients with adolescent idiopathic scoliosis?

BACKGROUND CONTEXT: Rigid bracing is a key treatment in patients with adolescent idiopathic scoliosis (AIS) but requires long-duration wearing. Although the impact of brace wear and its psychological stress raises concerns in adolescents, its contributions to health-related quality of life (HRQOL) during brace treatment remain unclear. PURPOSE: To compare the associations of objectively recorded brace wear time and early brace-related stress with HRQOL at 1 year after brace initiation, and to test whether stress mediates the association between wear time and HRQOL. DESIGN: Retrospective sing

Iatrogenic harm in paediatric and adolescent populations of patients with migraine: A systematic review with meta-analysis.

BackgroundMigraine is a common condition that causes a high burden of disability even at a young age, significantly affecting various aspects of quality of life. Despite this significant burden, the pharmacological treatment of migraine is still a subject of debate, with controversial results that do not provide sufficient evidence of its effectiveness. Furthermore, the safety profile in this inherently fragile population leaves some doubts about pharmacological prophylaxis use. This study aims to provide an overview of the safety profile of the main drugs used in populations of children and a

Oral iron supplementation in patients with chronic kidney disease: Can it be harmful to the gut microbiota?

Patients with chronic kidney disease (CKD) have several pathophysiological alterations, including anemia, one of the first changes in CKD patients. More recently, researchers have observed that the intestinal microbiota alterations are also another complication in these patients. The most common treatment for anemia is oral (mainly ferrous sulfate) or intravenous iron supplementation. Despite being a necessary treatment, recent studies have reported that supplementation with oral iron may increase its availability in the intestine, leading to disturbance in the gut microbiota and also to oxida

Management of hypertension in the very old: aggressive reduction of blood pressure is harmful in most patients.

Arterial hypertension is particularly prevalent in the elderly and treatment is required according to the literature. Treatment in subjects between 65 and 80 years of age resembles by characteristics and results that obtained in younger population. The treatment of arterial hypertension in the very elderly differs in a relevant percentage of them. It is quite similar in those with a good physical fitness with or without cardiovascular disease. A series of factors as the presence of real sustained hypertension, orthostatic hypotension, frailty and disability, and the boundary of safety recommen

Generalizability of harm and pain expectations after exposure in chronic low back pain patients.

BACKGROUND: Exposure treatments are shown to be effective in reducing pain-related fear and the perceived harmfulness of physical activities. However, due to the fragility of extinction its stability is questionable. We investigated the generalizability of exposure effects in chronic low back pain (CLBP) patients by integrating a behavioral test in the context of an intervention study. METHODS: The study is an additional analysis of a randomized controlled trial investigating the efficacy of exposure in vivo. A total of 67 CLBP patients were randomly assigned to one of the three groups: Expos

Dry eye disease in strabismus patients: Does eye deviation harm ocular surface?

INTRODUCTION: Dry eye disease is a multifactorial disease which affects the ocular surface system, and determines ocular discomfort symptoms and visual disturbance. Various types of ocular surgery, including strabismus one, represents a recognized cause for iatrogenic dry eye. However, it is not uncommon that strabismus patients, even not undergone to surgery, report symptoms of ocular discomfort and dryness. Hypothesis We hypothesize that two possible mechanisms may determine dry eye in strabismus patients by setting up a specific vicious circle: firstly, the increased exposure of the area of
