Abnormal blood pressure and pediatric obesity phenotype: Red flags for clinical practice.

BACKGROUND AND AIMS: systolic and diastolic blood pressure values identify different subtypes of Primary Hypertension. In pediatric age, hypertension is often related to obesity. Characterization of an individual's Blood Pressure (BP) status needs multiple measurements in the course of time, but this is rarely the case, principally in everyday pediatrics. Thus, the finding of abnormal BP values even in a single routine check visit should need a special attention. The aim of this study was to evaluate if even a single abnormal measurement could be a marker of metabolic risk, according to clinic

Exploring sleep-related breathing disorders in pediatric obesity and Prader-Willi syndrome.

OBJECTIVE: To analyze the differences of clinical and diagnostic features of sleep related breathing disorders (SRBDs) between children with PWS and obese children, considering obesity as a unifying risk factor for sleep apnea. STUDY DESIGN: This retrospective cohort study included ≥2 years of age children who had obesity and genetically confirmed Prader-Willi syndrome (PWS) or were non-PWS obese children. Out of 267 children, 58 children met inclusion criteria. Clinical data and records of standard overnight polysomnography (PSG) were collected and compared between groups during the study.

Disordered Eating and Eating Disorders in Pediatric Obesity: Assessment and Next Steps.

While the exact prevalence of disordered eating in youth who are overweight and have obesity has not been determined, studies show that the odds of a young adult (18-24 years) with obesity engaging in disordered eating behaviors is 2.45 times more likely to occur than in young adults with Body Mass Indexes (BMI) in the normative range. The purpose of this review is to highlight the role that disordered eating and eating disorders may play in pediatric obesity and the importance of screening for these conditions. The ability to identify and assess disordered eating alters the course of treatmen

Evaluating the association between obesity and discharge functional status after pediatric injury.

BACKGROUND: Children with obesity frequently have functional impairment after critical illness. Although obesity increases morbidity risk after trauma, the association with functional outcomes in children is unknown. OBJECTIVE: To evaluate the association of weight with functional impairment at hospital discharge in children with serious injuries. METHODS: This secondary analysis of a multicenter prospective study included children <15 years old with a serious injury. Four weight groups, underweight, healthy weight, overweight, and obesity/severe obesity were defined by body mass index z-sco

Obesity, acanthosis nigricans, insulin resistance, and hyperandrogenemia: pediatric perspective and natural history.

We studied the syndrome of acanthosis nigricans, obesity, insulin resistance, and hyperandrogenemia in 22 patients. Although isolated case reports in adolescents have appeared, this syndrome has not received full recognition as a pediatric entity. Our patients (17 girls, five boys) had a mean weight 5.7 SD above the mean for age, although mean height was only 0.5 SD above the mean for age. All patients had acanthosis nigricans. Their insulin resistance was significantly greater than that in a control group with comparable obesity. Fasting insulin concentration was 5.25 microU/ml in lean contro

Use of obesity medications in a young pediatric patient with optic nerve hypoplasia and severe early-onset obesity: A case report.

INTRODUCTION: Pediatric obesity presents complex challenges in children with underlying endocrine disorders. In patients with optic nerve hypoplasia (ONH) and hypothalamic-pituitary dysfunction, lifestyle changes alone are often ineffective for obesity treatment. This case report describes targeted pharmacologic interventions for severe early-onset obesity in a child with ONH and multiple pituitary hormone deficiencies. CASE PRESENTATION: A Hispanic female was diagnosed at 45 days of life with ONH and central arginine vasopressin deficiency, central hypothyroidism, secondary adrenal insuffici

Obesity class versus the Edmonton Obesity Staging System for Pediatrics to define health risk in childhood obesity: results from the CANPWR cross-sectional study.

BACKGROUND: Disease severity in paediatric obesity is usually defined using the body-mass index (BMI). Although informative at the population level, its usefulness on an individual level has limitations. The use of a clinical staging system-Edmonton Obesity Staging System for Pediatrics (EOSS-P)-in identifying health risk has been proposed. This study aimed to examine the association between BMI class and EOSS-P stage. METHODS: This cross-sectional study was done in children with obesity aged 5-17 years who enrolled in the Canadian Pediatric Weight Management Registry (CANPWR) between May 31,

Preptin, PPARγ, and NLRP3 as inflammatory biomarkers in pediatric obesity - a pilot study.

BACKGROUND: Childhood obesity leads to an increased inflammatory response, which in the long term can lead to serious health problems such as metabolic syndrome, diabetes, cancer and cardiovascular disease, and may also increase the risk of obesity in adulthood. Our aim was to evaluate inflammatory markers associated with pediatric obesity in 30 healthy children (BMI between the 15th–85th percentiles) and 30 obese children (BMI > 95th percentile) and to determine the possible associations of these markers. METHODS: Anthropometric measurements of the children were obtained. The serum preptin, p

DUSP21 expression is associated with obstructive sleep apnea in pediatric patients with obesity.

BACKGROUND: Obesity is a well-known risk factor for developing obstructive sleep apnea (OSA) in children. Both OSA and obesity are independently associated with cardiovascular and metabolic comorbidities. These comorbidities are driven by shared pathophysiological pathways, making it difficult to distinguish the individual contributions of obesity and OSA. This study aimed to investigate the molecular mechanisms of OSA in children with obesity through whole blood mRNA sequencing. METHODOLOGY: Children with obesity, aged 8-18 years, were enrolled at the start of a multidisciplinary weight loss
