Although the ileal conduit is currently the most widely used method for urinary diversion in patients undergoing pelvic exenteration, reports continue to accumulate indicating the frequency of long-term complications. Four patients with failure of ileal urinary diversion after pelvic exenteration for invasive pelvic malignancy are presented. All four patients had either multiple pelvic surgical procedures and/or pelvic radiation. Interposition of jejunum between the proximal ureters or renal pelvises and skin was performed with satisfactory results in all four. In patients with pelvic exentera

BACKGROUND: Pelvic exenteration (PE) performed for locally advanced or recurrent pelvic malignancies often required urinary reconstruction but associated with high postoperative morbidity (POM). The influence of urinary diversion type on outcomes remains uncertain.
METHODS: This retrospective cohort study included 90 patients who underwent PE with urinary reconstruction between 2016 and 2023. Urinary diversions were classified as ileal conduit and alternative reconstructions (vesico-urethral anastomosis, Mitrofanoff procedure, ureteral implantation and enterocystoplasty). POM, quality of life

OBJECTIVE: To examine changes in performance and outcomes of pelvic exenteration for gynecologic malignancies.
METHODS: This is a population-based retrospective study examining the Nationwide Inpatient Sample between 2001 and 2015. Women with cervical, uterine, vaginal, and vulvar malignancies who underwent pelvic exenteration were examined. Comorbidity, perioperative complications, total charges, length of stay, and mortality were assessed.
RESULTS: There were 2647 cases included. Cervical cancer was the most common malignancy (45.1%), followed by vaginal cancer (27.6%). 26.9% of women had

OBJECTIVE: To determine the effect of surgeon experience on intraoperative, postoperative and long-term outcomes among patients undergoing pelvic exenteration for gynecologic cancer.
METHODS: This was a retrospective analysis of all women who underwent exenteration for a gynecologic malignancy at MD Anderson Cancer Center, between January 1993 and June 2013. A logistic regression was used to model the relationship between surgeon experience (measured as the number of exenteration cases performed by the surgeon prior to a given exenteration) and operative outcomes and postoperative complicatio

In locally advanced or recurrent tumors of the female genital tract anterior or total exenteration may be mandatory in case of tumor invasion into the lower urinary tract or if a second course of radiation therapy is not feasible. The management of resection and reconstruction of the affected lower urinary tract has to be well integrated into the gynecological therapeutic concept. In 11/32 patients the reconstruction of the partially resected lower urinary tract was feasible with preservation of a functionally intact urinary bladder. Urinary diversion following pelvic exenteration was achieved

BACKGROUND: Total pelvic exenteration (TPE) creates a massive pelvic dead space associated with a high risk of severe postoperative complications. We evaluated the clinical outcomes of pelvic floor and perineal reconstruction using a modified posterior thigh flap including the gluteus maximus muscle in patients undergoing TPE for anal canal cancer.
PATIENTS AND METHODS: We retrospectively reviewed 4 male patients who underwent this reconstruction at our institution between April 2020 and March 2025. Clinical outcomes, including operative details, postoperative complications, and ambulatory fu

OBJECTIVE: To evaluate postoperative and oncologic outcomes associated with pelvic exenteration for non-ovarian gynecologic malignancies.
METHODS: This was a retrospective review of patients who underwent pelvic exenteration for non-ovarian gynecologic malignancies at our institution from 1/1/2010-12/31/2019. Palliative exenteration cases were excluded from survival analysis. Postoperative complications were early (≤30 days) or late (31-180 days). Complications were graded using a validated institutional scale. Major complications were considered grade ≥ 3. Categorical variables were compared

Application of laparoscopic surgery(Lap)for colorectal cancer has expanded, and laparoscopic total pelvic exenteration (TPE)of locally advanced rectal cancer(LARC)invading the urogenital organs has been introduced in some institutions. In our institute, we have performed Lap TPE and posterior-TPE(PPE)in a total of 6 LARC patients so far. Here, we report the surgical technique of Lap TPE, and the associated surgical and short-term outcomes. We performed Lap TPE and Lap PPE in 3 patients each. Operation time was approximately 562 min, and the blood loss was 310 mL on an average. No patient devel

Pelvic exenteration is used in the treatment of several pelvic cancers, including those of the rectum, uterus, and bladder. We report the first case of robotic pelvic exenteration for the treatment of symptomatic prostate cancer involving the rectum and bladder. A six-port transperitoneal robotic approach was used. Bilateral extended lymphadenectomy up to the inferior mesenteric artery was performed. The rectum and bladder were removed en bloc, and a double-barrel anastomosis was then performed with both ureters being connected to the lower opening of the colostomy. Operative time was 249 minu

A 64-year-old woman with recurrence of carcinoma of the vulva in an irradiated area received an en-bloc total pelvic exenteration. Reconstruction of the pelvic defect was performed with an anterolateral thigh (ALT) flap and a rectus abdominis muscle (RAM) flap (PM/RAM). This combination of flaps is unique, with excellent results. In a large defect, often irradiated in advance, well-vascularised tissue should be placed. Multiple flaps can be used to reconstruct these large pelvic defects, each with their own advantages and disadvantages. The combination of flaps used in this case uses the good

OBJECTIVE: To determine the effect of distance to closest negative margin on survival after pelvic exenteration (PE).
METHODS: In this retrospective analysis of PE at Moffitt Cancer Center from 2000 to 2019, baseline characteristics, clinical details, and outcomes were ascertained. Distance to closest negative margin was measured. Close and distant negative margins were defined as <3 mm and ≥3 mm from malignancy to nearest surgical margin, respectively. Overall survival (OS) and progression-free survival (PFS) were determined, and Kaplan-Meier curves were compared. Cox proportional hazards re

Pelvic exenterations are known to be a last resort therapeutic option for advanced or recurrent gynecologic malignancies, which are known to have poor prognosis. All women treated with anterior (APE) or total (TPE) pelvic exenteration at our University hospital within a five-year period were identified and their data retrospectively analysed. Parameters such as demographic information, tumor type and stage, previous therapy as well as complication rate and overall survival were evaluated. 47 women were enrolled in this study. Most common indication for PE was cervical cancer (51.1%) followed b

PURPOSE: The main aim of this study was to show the interest of pelvic posterior exenteration to obtain complete resection of the tumor in case of invasion of the rectum by contiguity in advanced-stage ovarian cancer. The secondary aim was to determine the morbidity of this surgery.
METHODS: It is a multicentric, retrospective study of a series of 41 patients, who underwent posterior pelvectomy for advanced-stage ovarian cancer, over a period of 18 years, from July 1989 to July 2007.
RESULTS: The surgery resulted in macroscopically complete resection in 19 patients (46.34%), a residual tumor

BACKGROUND: Pelvic exenterations (PE) in solid organ gynaecological malignancies have historically been confined to the central pelvic compartment but an expansion of resection extent to include the lateral compartment is now technically feasible. There remains uncertainty about its influence on outcomes.
METHODS: Retrospective study of consecutive patients undergoing PE for non-ovarian gynaecological malignancies in a tertiary referral unit. Two groups were assessed and compared: standard resections including but not beyond the pelvic sidewall (SR); and extended resections beyond the pelvic

BACKGROUND: Total pelvic exenteration (TPE) is a rare operation in which the pelvic contents are removed entirely. Several options for pelvic floor and vaginal reconstruction have been described including transverse rectus abdominis musculocutaneous (TRAM) or deep inferior epigastric perforator (DIEP) flaps. The transverse musculocutaneous gracilis (TMG) flap has been introduced for breast reconstruction as a free flap. We adopted the pedicled TMG flap for reconstructions after TPE. To the best of our knowledge, this is the first report of this method in the literature.

PURPOSE: No standard exists for reconstruction after extralevator abdominoperineal excision (ELAPE) and pelvic exenteration. We propose a tailored concept with the use of bilateral gluteal V-Y advancement flaps in non-extended ELAPE and with vertical myocutaneous rectus abdominis muscle (VRAM) flaps in extended procedures. This retrospective study analyzes the feasibility of this concept.
PATIENTS AND METHODS: We retrieved all consecutive patients after ELAPE or pelvic exenteration for rectal, anal, or vulva cancer with flap repair from a prospective database. Perineal wound complications wer

Entero-articular fistulas are rare but serious complications following total hip arthroplasty (THA), often associated with prior abdominal or pelvic surgery, radiation therapy, or chronic steroid use. We report a case of a 65-year-old woman with metastatic ovarian cancer previously treated with pelvic exenteration and heated intra-peritoneal chemotherapy (HIPEC), who developed an entero-articular fistula following THA for avascular necrosis (AVN). Post-operatively, she presented with systemic signs of infection and polymicrobial joint cultures containing enteric organisms, raising suspicion fo
