Dynamic transperineal ultrasound in the diagnosis of pelvic floor disorders: pilot study.

PURPOSE: Defecating proctography and more recently, magnetic resonance imaging have both been used for diagnosis in patients with pelvic floor dysfunction. This pilot study assessed the feasibility of dynamic transperineal ultrasound in a range of specific disorders affecting evacuation. METHODS: A protocol of sagittal and transverse transperineal imaging was established defining the infralevator viscera and soft tissues and the margins of the puborectalis muscle. Dynamic measurements were possible for the extent of puborectalis shortening, the anorectal angle, and the movement during straini

Characteristics Associated with Pelvic Floor Disorders among Female Canadian Armed Forces Members.

OBJECTIVES: Urinary incontinence (UI) and pelvic organ prolapse (POP) are prevalent pelvic floor disorders (PFDs) among the female population. In the military environment, being a non-commissioned member (NCM), and physically demanding occupations are factors associated with higher PFD risk. This study seeks to characterize the profile of female Canadian Armed Forces (CAF) members reporting symptoms of UI and/or POP. METHODS: Present CAF members (18-65 years) responded to an online survey. Only current members were included in the analysis. Symptoms of UI and POP were collected. Multivariate

A Systematic Review of Racial/Ethnic Disparities in Female Pelvic Floor Disorders.

Numerous studies have investigated the influence of health disparities among women with pelvic floor disorders with varied results. Racial/ethnic disparities, in particular, inconsistently indicate differences in prevalence of disease, disease severity, and treatment outcomes. We aim to review the body of literature examining racial/ethnic disparities in pelvic floor disorders, including overactive bladder, stress urinary incontinence, pelvic organ prolapse, and interstitial cystitis. A better understanding of these disparities may help guide clinicians, researchers, and advocates in providing

Predicting risk of pelvic floor disorders 12 and 20 years after delivery.

BACKGROUND: Little progress has been made in the prevention of pelvic floor disorders, despite their significant health and economic impact. The identification of women who are at risk remains a key element in targeting prevention and planning health resource allocation strategies. Although events around the time of childbirth are recognized clinically as important predictors, it is difficult to counsel women and to intervene around the time of childbirth because of an inability to convey a patient's risk accurately in the presence of multiple risk factors and the long time lapse, which is oft

Patient-selected goals: the fourth dimension in assessment of pelvic floor disorders.

The purpose of the study was to assess the relationship between self-expressed urogynecologic goals, symptoms, and treatment choice. Charts of women presenting for urogynecology consultation were reviewed. Demographics, diagnoses and responses to the pelvic floor distress inventory and medical, social, and epidemiologic aspects of aging questionnaires were recorded. Patients listed urogynecology goals before consultation. We categorized goals into five categories and then compared these categories by symptom type, severity, and treatment. Three hundred five women reported 635 goals (median 2,

Pelvic floor dyssynergia and psychiatric disorders. Does the snake bite its tail?

AIM: Psychological and/or psychiatric disorders (PSY) and functional gastrointestinal disorders (FGID) are often linked. Pelvic floor dyssynergia (PFD) is one of the most frequent FGID, but few studies have investigated its possible relationship with PSY. The aim of the present study was to evaluate whether an increased prevalence of PSY, and of what types, exist in patients affected with PFD. METHODS: Thirty-four female patients PFD and 34 age- and gender-matched control subjects were evaluated. The prevalence rates of axis I psychiatric disorders (DSM IV) and of pathological temperaments (S

Female Sexual Dysfunction Across the Female Lifespan: The Impact of Menopause, Pelvic Floor Disorders, and Pelvic Surgery.

Female sexual dysfunction is a multifactorial condition influenced by biological, psychological, relational, and sociocultural factors that can compromise sexual well-being. It is a prevalent condition, affecting nearly 40% of the global female population. Furthermore, problems related to desire, arousal and orgasm have an impact on 12% of women across the lifespan. Despite the high prevalence of this issue and its harmful impact on mental health and vitality, a considerable number of women do not seek medical care. Another issue is the lack of physicians specialising in sexual health problems

How to Best Measure and Characterize Symptoms of Patients with Pelvic Floor Disorders.

Pelvic floor disorders (PFDs) are common and can be associated with various symptoms, including fecal incontinence, constipation, urinary incontinence, lower urinary tract symptoms, sexual dysfunction, and pelvic pain. Characterizing symptoms can be challenging due to limitations in patient reporting, provider ability to collect data, and the instruments used to collect the data. Symptoms are assessed using patient-reported outcome measures (PROMs), instruments that can be generic or disease-specific. Here, we describe the frequently used and recommended instruments for assessing symptoms of P

Reflections on 30 Years of Taking Care of Patients with Pelvic Floor Disorders.

Over the past three decades care of patients with pelvic floor disorders has changed dramatically. Fecal incontinence surgery that initially was repair or encircling the anal sphincter now is treated as a first line with sacral nerve stimulation. Rectal prolapse surgery has benefited from minimally invasive approaches and an abdominal approach is considered even in older frail individuals. Medication for constipation has dramatically helped more patients avoid surgery. A multidisciplinary approach with surgeons operating during the same anesthesia on the middle/anterior pelvis and posterior pe
