Legalism, countertransference, and clinical moral perception.

This target article focuses on dynamics that arise in three typical ethically complex cases in which psychiatric consultations are requested by physicians: a dying patient refuses life-prolonging treatment, an uncooperative patient demands to be allowed to go outside and smoke, and an angry patient demands to be admitted to the hospital. The discussion canvasses what is at stake morally and clinically in each of these cases and explores clinician-patient interactions, dynamics in relationships between consulting physicians and consultant psychiatrists, patient transference, and physician count

Cues for Haptic Perception of Compliance.

For the perception of the hardness of compliant materials, several cues are available. In this paper, the relative roles of force/displacement and surface deformation cues are investigated. We have measured discrimination thresholds with silicone rubber stimuli of differing thickness and compliance. Also, the influence of the finger span is assessed. When compliance is expressed as the Young's modulus, the thresholds in the different conditions follow Weber's law with a Weber fraction of 15 percent. When the surface deformation cue was removed, thresholds more than trebled. Under the assumptio

Aesthetic perception after root coverage procedure.

AIM: To investigate the aesthetic perception of the clinical outcomes of a simulated root coverage procedure in three different groups: patients, dentists, and periodontists. MATERIAL AND METHODS: 100 patients, 107 general dentists affiliated with the Dental Association of Prato, Italy, and 81 active members of the Italian Society of Periodontology were recruited for this study. The following variables: age, gender, level of education, place of residence, and years of practice (only for dentists and periodontists) were recorded by means of a questionnaire administered to each subject within t

Amygdalar enlargement associated with unique perception.

Interference by amygdalar activity in perceptual processes has been reported in many previous studies. Consistent with these reports, previous clinical studies have shown amygdalar volume change in multiple types of psychotic disease presenting with unusual perception. However, the relationship between variation in amygdalar volume in the normal population and the tendency toward unusual or unique perception has never been investigated. To address this issue, we defined an index to represent the tendency toward unique perception using ambiguous stimuli: subjects were instructed to state what t

Gender in voice perception in autism.

Deficits in the perception of social stimuli may contribute to the characteristic impairments in social interaction in high functioning autism (HFA). Although the cortical processing of voice is abnormal in HFA, it is unclear whether this gives rise to impairments in the perception of voice gender. About 20 children with HFA and 20 matched controls were presented with voice fragments that were parametrically morphed in gender. No differences were found in the perception of gender between the two groups of participants, but response times differed significantly. The results suggest that the per

Somatosensory processes subserving perception and action.

The functions of the somatosensory system are multiple. We use tactile input to localize and experience the various qualities of touch, and proprioceptive information to determine the position of different parts of the body with respect to each other, which provides fundamental information for action. Further, tactile exploration of the characteristics of external objects can result in conscious perceptual experience and stimulus or object recognition. Neuroanatomical studies suggest parallel processing as well as serial processing within the cerebral somatosensory system that reflect these se

Fundamental requirements for primary visual perception.

A novel perspective for the necessary and sufficient conditions for primary visual perception is proposed based upon relating recent discoveries from the visual sciences to relevant foundational neurological observations. This analysis suggests that a fundamental requirement for the emergence of normal primary visual perception is the coupling between the early visual cortices in the occipital lobe subserving image content with specific areas in the parietal lobe subserving selective attention, representations of extrapersonal space, the body schema, and the initiation of perceptual ownership.

Consumer perception of household hazardous materials.

UNLABELLED: We evaluated consumer perception of household hazardous materials (HHM) to identify links between storage of HHMs and consumer perception. METHODS: 357 telephone surveys were conducted within one county to determine home storage location (high, low, unknown) of 10 substances common to pediatric poisoning. Questions addressed look-alikes, poison information resources, disposal/recycling practices, and the transfer of cleaning products to other containers. RESULTS: Prescription medications were stored in lower elevations than vitamins with iron and OTC ibuprofen or acetaminophen. P

Incomplete figure perception and invisible masking.

The Gollin test (measuring recognition thresholds for fragmented line drawings of everyday objects and animals) has traditionally been regarded as a test of incomplete figure perception or 'closure', though there is a debate about how such closure is achieved. Here, figural incompleteness is considered to be the result of masking, such that absence of contour elements of a fragmented figure is the result of the influence of an 'invisible' mask. It is as though the figure is partly obscured by a mask having parameters identical to those of the background. This mask is 'invisible' only conscious

Pain perception in irritable bowel syndrome.

Irritable bowel syndrome (IBS) is a functional gastrointestinal disorder characterized by chronic abdominal discomfort or pain in the absence of detectable organic disease. IBS is common and is associated with a significant impairment in health-related quality of life. Enhanced perception of visceral stimuli ("visceral hypersensitivity") appears to be an important pathophysiological mechanism. Early IBS studies using functional brain imaging techniques suggest an alteration in central pain modulation circuits, rather than an increased sensitivity of peripheral visceral pain pathways. The frequ

Validation of the urgency perception scale.

OBJECTIVE: To ascertain the validity and responsiveness of the Urgency Perception Scale (UPS) as an indicator of perceived urinary urgency. PATIENTS AND METHODS: The UPS was developed to assess perceived urinary urgency in clinical studies evaluating the efficacy of the antimuscarinic drug, tolterodine, for treating the overactive bladder (OAB) syndrome. Secondary analyses of clinical and patient assessment data from three clinical studies of tolterodine were conducted to evaluate the UPS. Construct validity was assessed by correlations between the UPS and patient voiding diary variables and

Epileptic kinetopsia: ictal illusory motion perception.

A 38-year-old woman with a right posterior temporo-occipital brain tumor developed partial seizures with illusory motion perception of environmental objects going from the center to the periphery within her left visual field. Subdural EEG recordings during visual seizures revealed onsets in the right temporo-parieto-occipital junction. Her ictal visual distortion was probably caused by activation of V5, an area involved in motion perception. Given that the tumor location corresponds with the ictal onset in the V5 area, and the semiology of her seizures, this case supports that epileptic dysfun

Nonveridical visual perception in human amblyopia.

PURPOSE: Amblyopia is a developmental disorder of spatial vision. There is evidence to suggest that some amblyopes misperceive spatial structure when viewing with the affected eye. However, there are few examples of these perceptual errors in the literature. This study was an investigation of the prevalence and nature of misperceptions in human amblyopia. METHODS: Thirty amblyopes with strabismus and/or anisometropia participated in the study. Subjects viewed sinusoidal gratings of various spatial frequencies, orientations, and contrasts. After interocular comparison, subjects sketched the su

Disorders of face perception and recognition.

Face recognition is one of the most complex visual tasks performed by the human brain. Data from monkeys suggest that area IT may play a key role in identifying faces, and functional imaging research suggests that the human homologue of IT may be located in the medial occipitotemporal cortex, where a FFA has been located. Damage to medial occipitotemporal structures on the right or bilaterally leads to prosopagnosia, the failure to recognize facial identity. Prosopagnosia is not a single functional disorder but a family of dysfunctions, with different patients having different degrees of impai

Temporal envelope perception in dyslexic children.

Speech intelligibility depends heavily on the accurate perception of auditory temporal envelope cues, that is the slower amplitude modulations present in the speech waveform. In a previous study, McAnally and Stein demonstrated that dyslexics may show impaired audibility (i.e. detectability) of these envelope cues. In the present psychophysical study, the ability to process temporal envelope cues was further investigated in dyslexic children by measuring detection thresholds of sinusoidal amplitude-modulation (SAM) and discrimination thresholds of SAM depth and SAM rate. Each threshold was mea

Auditory perception in vestibular neurectomy subjects.

The auditory efferent nerve is a feedback pathway that originates in the brainstem and projects to the inner ear. Although the anatomy and physiology of efferents have been rather thoroughly described, their functional roles in auditory perception are still not clear. Here, we report data in six human subjects who had undergone vestibular neurectomy, during which their efferent nerves were also presumably severed. The surgery had alleviated these subjects' vertigo but also resulted in mild to moderate hearing loss. We designed our experiments with a focus on the possible role of efferents in a

Taste perception after lingual nerve repair.

PURPOSE: This retrospective study evaluates patients' perception of taste after lingual nerve injury and repair. It is hypothesized that return of taste is a distinct neurophysiologic phenomenon related to specialized taste physiology and it does not correlate with specific, objective sensory return. PATIENTS AND METHODS: During 1995 to 1996, 30 patients underwent microsurgical repair of lingual nerve injuries. Of these, 22 patients met the inclusion criteria for this study. Chart review documented the date and cause of injury, sensory examination, injury classification, procedures, operative

Breast cancer risk: perception versus reality.

Evidence that breast cancer is hormonally mediated has fueled women's concern that use of oral contraceptives (OC) will increase their risk of developing the disease. A recent reanalysis of combined worldwide epidemiologic evidence regarding the relationship between breast cancer risk and use of combination OC provides reassurance that there is little or no association between OC use and breast cancer. Ten or more years after discontinuation of OC use, there is no difference in cumulative risk of breast cancer among OC ever-users and never-users. The risk of breast cancer diagnosis is slightly

Perception of spatial orientation in microgravity.

Experiments during space and parabolic flights have shown that human spatial orientation in microgravity differs to a significant extent from its performance on earth. Due to the missing reference of gravitational force, unusual perceptual phenomena are observed, from inversion illusions to errors of perceived motion and position with respect to the spacecraft. This article gives an overview of results collected from space missions and parabolic flight campaigns, and proposes new lines of research about the perceptual phenomena of spatial orientation in microgravity. It is shown that most of t

Categorical speech perception in cerebellar disorders.

Keele and Ivry (1991) considered the cerebellum an "internal clock" responsible for temporal computations both in the motor and in the perceptual domain. These authors, therefore, expected that the processing of durational parameters of the perceived acoustic speech signal such as voice onset time (VOT) depends upon the cerebellum as well. However, a preliminary investigation of Ivry and Gopal (1992) revealed unimpaired phoneme-boundary effects in cerebellar patients along a continuum of monosyllabic stimuli with systematically varied VOT (/ba/-/pa/). Since the energy of the aspiration noise p
