BACKGROUND: Defects sustained at the little finger and the ulnar aspect of the hand are common and pedicled perforator flaps have unique advantages in resurfacing it. The purpose of this study is to reappraise the anatomy of the septocutaneous perforator in the postero-medial aspect of the hand and present our clinical experience in using perforator flaps based on it.
METHODS: This study was divided into anatomical study and clinical application. In the anatomical study, 30 preserved upper limbs were used. Clinically, 16 patients with defects at the little finger or the ulnar aspect of the ha

Traumatic digit defects of high complexity and with inadequate local tissue represent challenging surgical problems. Recently, perforator flaps have been proposed for reconstructing large defects of the hand because of their thinness and pliability and minimal donor site morbidity. Here, we illustrate the use of thin latissimus dorsi perforator flaps for resurfacing multiple defects of distal digits. We describe the cases of seven patients with large defects, including digits, circumferential defects and multiple-digit defects, who underwent reconstruction with thin latissimus dorsi perforator

Major scrotal defects may result from infection due to Fournier's gangrene, excision of scrotal skin diseases, traumatic avulsion of scrotal and penile skin, and genital burns. The wide spectrum of bacterial flora of the perineum, difficulty in providing immobilisation, and obtaining a natural contour of the testes make testicular cover very difficult. Various methods have been reported to cover the penoscrotal area, including skin grafting, transposing them to medial thigh skin, and use of local fasciocutaneous or musculocutaneous flaps. In this report, reconstruction using six local medial c

We raised thenar island flaps that were supplied by perforators that originated in the superficial palmar arch or the superficial palmar branch of the radial artery for the reconstruction of fingertip defects in eight patients. The flap was so well-vascularised that a large flap with increased mobility could be raised. The donor site was covered with well-vascularised thick tissue, and skin grafting of the donor site was avoided in all cases. No patient developed a complication, and all flaps survived. Functional and cosmetic results of both fingertips and donor sites were excellent. A perfora

The peroneal artery perforator flap and its modifications have been widely used for coverage of soft tissue defects of ankle and foot in the past decade. In this article, we report on a series of upper knee, ankle, and foot reconstructions with a proximally or distally based extended peroneal artery perforator (EPAP) flap supplied by distinct perforating branches off the proximal or distal peroneal artery. Total pedicle length obtained ranged from 6 to 12 cm. Twelve patients with soft tissue defects of the lower extremity underwent reconstruction using the EPAP flap. The flaps were designed wi

Free flaps are usually required rather than local flaps for large plantar defects, due to a lack of locally available tissue. The medial sural artery perforator free flap, recently introduced clinically by several authors, is a noticeable option for soft tissue coverage, but it has still not been widely used for the reconstruction of various large plantar defects. Between 2005 and 2007, medial sural artery perforator free flaps were used to reconstruct soft tissue defects in plantar areas in 11 patients at our institute. Patient ages ranged from 10 to 68 years (mean, 43 years), and follow-up p

The hands and forearms are frequent sites of burn injury, and due to the high concentration of superficial tendons, bones, and joints, flaps are frequently needed to cover defects in these areas. We present a patient who suffered remote electrical injury to the right forearm which later developed into an open wound with exposed tendon. Successful coverage of this defect was accomplished with an islanded fasciocutaneous flap based on perforating arterial branches of the radial artery, which was harvested from an area of burn injury. Surgical treatment of hand and forearm wounds with axial pedic

We report our experience of using the medial sural artery perforator flap in the reconstruction of soft tissue defects in the hand in seven cases with 1 to 2 year follow-up. The flap is harvested from the posteromedial aspect of the leg, just below the knee and superficial to the medial head of the gastrocnemius muscle. It is based on the perforator arteries and veins supplied by the medial sural artery. The flaps ranged in size from 14 x 10 cm to 8 x 6 cm. The donor area was closed directly or by a skin graft. All but one flap survived. The cosmetic results were satisfactory and without appar

The need for thin flap coverage has increased, especially for contouring or covering shallow defects of extremities. The free thoracodorsal artery perforator flap harvested from the upper lateral back can be useful for this purpose. The thoracodorsal artery supplies the latissimus dorsi muscle and supplies perforating branches to the overlying skin. The flap is based upon the proximal perforator of the thoracodorsal artery, which usually emerges in an area approximately 8-10 cm below the posterior axillary fold and 2-3 cm posterior to the lateral border of the latissimus dorsi muscle. Between

Necrobiosis lipoidica (NL) is a granulomatous disease presenting as plaques with potential for progression to chronic ulcerating lesions. Breast involvement of the disease is rare. There are currently no FDA-approved medical therapies, and surgical treatment is controversial due to concerns for disease recurrence. Limited historical cases of surgical management demonstrate success when excision is taken to deep fascia. No prior reconstruction of NL manifesting on the breast has been documented in the literature. In this report, we present a patient with isolated ulcerated NL of the right breas

The deep inferior epigastric perforator (DIEP) flap is considered the gold standard in breast reconstruction. Robotic harvest of the DIEP flap aims to further minimize abdominal morbidity by reducing anterior fascial damage. Most descriptions of robotic DIEP flaps describe transabdominal approaches that require transecting and then repairing the posterior sheath. We describe a unilateral robotic totally extraperitoneal approach using multiple ports that avoids transecting the posterior sheath and entry into the abdomen.
