We describe a patient with persistent papular acantholytic dermatosis provoked by phototoxic reaction to his usual perfumed soap and shaving foam. Therapy with systemic and topical corticosteroids failed to improve the patient. Treatment with cyclosporine was found to be effective.

BACKGROUND: Fragrance allergy is the most common cause of cosmetic contact dermatitis. Many occult sources of fragrance exist. Those which cause the most concern are some "fragrance-free" products that contain fragrance raw ingredients. Thus, the very patients requiring fragrance-free items may be exposed to potential perfume allergens or cross-reactors in seemingly safe products. Additionally, medications dermatologists recommend, both prescription and over-the-counter, sometimes contain fragrance.

Most people in modern society are exposed daily to fragrance ingredients from one or more sources. Fragrance ingredients are also one of the most frequent causes of contact allergic reactions. The diagnosis is made by patch testing with a mixture of fragrance ingredients, the fragrance mix. This gives a positive patch-test reaction in about 10% of tested patients with eczema, and the most recent estimates show that 1.7-4.1% of the general population are sensitized to ingredients of the fragrance mix. This study reports a photoallergic contact dermatitis to musk ambrette in two male patients. Musk ambrette is a widely used perfume ingredient, but it is also included in food and drugs. Both patients reacted in the long ultraviolet light (UV-A) to musk ambrette and to the after shave lotions containing musk ambrette. The histopathologic finding in the photopatch test site revealed acute spongiotic dermatitis.

In the present study, we have investigated 42 cosmetic products based on natural ingredients for content of 11 fragrance substances: geraniol, hydroxycitronellal, eugenol, isoeugenol, cinnamic aldehyde, cinnamic alcohol, alpha-amylcinnamic aldehyde, citral, coumarin, dihydrocoumarin and alpha-hexylcinnamic aldehyde. Since chemical anti-allergic compounds have adverse effects, many investigators pay attention to relatively safe natural products. Twenty-four newly crossbred rosebuds were extracted with 80% ethanol and analyzed for polyphenols, flavonoids, tannins, proanthocyanidins, and pyrogallol (1,2,3-benzenetriol). A 73-year-old patient, diagnosed as having seborrheic dermatitis, was patch tested with his hair preparations. The hair stick gave a positive reaction. Among its ingredients, D & C Yellow No. 11, from 0.0001% to 0.1%, and perfume showed positive reactions. D & C Yellow No. 11 was found to consist only of quinophthalone by chemical analyses. The concentration of quinophthalone in the hair stick was determined as 9.41 ppm w/w by high-performance liquid chromatography.

Patch testing data indicate that the 5 most prevalent contact allergens out of more than 3700 that are known are: nickel (14.3% of patients tested), fragrance mix (14%), the topical antibiotic neomycin (11.6%), balsam of Peru (used in some perfumes, toiletries, and pharmaceutical items) (10.4%), and the mercury-based vaccine preservative thimerosal (10.4%).

Sexually induced dermatoses (SID) are conditions resulting from sexual activity. Sometimes this can be modified by bacterial infection or systemic disease. Many materials such as medications, cosmetics, perfumes, pads, hygiene, sprays, condoms, etc, may cause sexually induced dermatoses in sexual partners. An additional set for testing of patients with eczema was prepared. This set comprised 38 compounds, including 12 components of perfumes and food flavours, 12 components of rubber or plastic materials, 9 drugs for external use and 5 other agents. Foreign bodies (FBs) in the lower gastrointestinal tract are sporadically described in children. The therapeutic approach is individualized, depending on the type of FB, the location, time since insertion and the severity of bowel injuries. These FBs can be frequently removed manually or endoscopically. However, exploratory laparotomy is inevitable in unsuccessful and complicated cases. Herein we present a teenager boy with a self-inserted large perfume bottle in the sigma, and we performed the review of the published literature.

A 49-year-old woman presented with asymptomatic progressive erythema, hyperpigmentation, atrophy, and telangiectasia on her neck. The clinical appearance and histopathology of the skin lesion was consistent with poikiloderma of Civatte. Photopatch-testing with our standard photoallergens yielded a positive reaction to 6-methylcoumarin, which was found in the patient's perfume. This report describes the first case in which photoallergic contact sensitization to 6-methylcoumarin may play an important role in the pathogenesis of poikiloderma of Civatte.

Out of 545 consecutive contact dermatitis clinic patients, 73 (13%) had dermatitis of the neck of whom 18 (25%) were nickel positive. 7 had clothing dermatitis and 3 of them reacted to chromate in military uniforms. 5 patients were sensitised to perfume and 4 to medicaments used to treat existing dermatitis. 3 patients were sensitised to hair dyes. Nickel dermatitis, was clinically overdiagnosed, mainly due (for various reasons) to inability to make a firm diagnosis of atopy.

Contact allergens produce three types of reactions. The commonest reaction is allergic contact dermatitis (ACD).
