BACKGROUND: Intradialytic hypotension is a serious complication during renal replacement therapy in critically ill patients. Early prediction of intradialytic hypotension could allow adequate prophylactic measures. In this study we evaluated the ability of peripheral perfusion index (PPI) and heart rate variability (HRV) to predict intradialytic hypotension.
METHODS: A prospective observational study included 36 critically ill patients with acute kidney injury during their first session of intermittent hemodialysis. In addition to basic vital signs, PPI was measured using Radical-7 (Masimo) d

BACKGROUND: The perfusion index (PI) derived from pulse oximetry readings represents the ratio of pulsatile (arterial blood) and nonpulsatile contributors to infrared light absorption. PI has been shown to correlate with cardiac performance. In theory, PI is readily available on every pulse oximeter; therefore, no additional sensors or infant handling are required. Currently, reference values are lacking in (preterm) neonates and the association with common clinical conditions is unclear.
OBJECTIVES: To establish reference values for the PI in premature infants and at the same time determine

BACKGROUND: Hypotension during spinal anaesthesia for Caesarean delivery is a result of decreased vascular resistance due to sympathetic blockade and decreased cardiac output due to blood pooling in blocked areas of the body. Change in baseline peripheral vascular tone due to pregnancy may affect the degree of such hypotension. The perfusion index (PI) derived from a pulse oximeter has been used for assessing peripheral perfusion dynamics due to changes in peripheral vascular tone. The aim of this study was to examine whether baseline PI could predict the incidence of spinal anaesthesia-induce

INTRODUCTION: Pulse oximetry screening is a safe, feasible test, effective in identifying congenital heart diseases in otherwise well-appearing newborns. Uncertainties still persist on the most effective algorithm to be used and the timing of screening. The aim of this study was to evaluate the role of the pulse oximetry screening associated with the peripheral perfusion index performed in the first 24 hours of life for the early detection of congenital heart diseases and noncongenital heart diseases in the newborns.
MATERIALS AND METHODS: A prospective observational cohort study was conducte

AIM: A 12-lead electrocardiogram (ECG) after the return of spontaneous circulation (ROSC) is recommended to diagnose a ST-segment elevation myocardial infarction (STEMI). In the early post-ROSC phase, the ECG can show signs of ischemia not necessarily of coronary origin and post-ROSC hypoperfusion could affect ECG reliability. We sought for an association between peripheral perfusion index (PI) values after ROSC and the percentage of false-positive ECG for STEMI.
METHODS: We considered all the consecutive patients with sustained ROSC after OHCA, admitted to the Fondazione IRCCS Policlinico Sa

In areas of severe asynergy, the clinically important task is to identify functionally recoverable myocardium. Fourteen patients with asynergy were investigated by H2(15O) dynamic positron emission tomography imaging before revascularization. Regional myocardial blood flow (MBF) was determined and the water-perfusable tissue fraction (PTF) for each region of interest and the total anatomical tissue fraction (ATF) were estimated. The PTF/ATF was analyzed as the water perfusable tissue index (PTI). Asynergy was defined as segments with wall motion more than 2SD below than that of a normal popula

Background and Objectives: Hemodynamic instability during anesthesia induction and endotracheal intubation remains a clinically important concern. Low-dose ketamine and intravenous lidocaine may provide complementary effects that improve cardiovascular stability and peripheral perfusion. This pilot study evaluated effects of adding ketamine and lidocaine to a propofol-based induction regimen using mean arterial pressure (MAP) and perfusion index (PI) as monitoring parameters. Materials and Methods: In this prospective observational pilot study, 30 adult patients undergoing elective surgery req

BACKGROUND: Caudal block is a widely practiced regional anesthetic technique in pediatric patients undergoing infraumbilical and lower limb surgeries. However, assessing its effectiveness intraoperatively remains a challenge due to limitations in conventional clinical evaluation methods, especially under general anesthesia. The perfusion index (PI), a noninvasive indicator of peripheral perfusion, has emerged as a potential early marker of successful neuraxial block.
AIM: To evaluate the perfusion index as a primary predictor of successful caudal block in children undergoing elective infraumb

BACKGROUND: Postinduction hypotension (PIH) is a common complication of general anesthesia with potential clinical consequences. A recent meta-analysis suggested that the peripheral perfusion index (PPI) has a high predictive value for PIH. However, this was limited by small sample sizes and significant heterogeneity. We aimed to evaluate the predictive performance of preinduction PPI for PIH in a large surgical cohort.
METHODS: In this retrospective single-center study, we analyzed 6653 adult patients who had general anesthesia between May 2022 and May 2023. PIH was defined as mean arterial

BACKGROUND: To explore the relationship between Normalized Wall Index (NWI) and Magnetic Resonance Perfusion Imaging Parameters in Patients with Mild Carotid Artery Stenosis.
METHODS: Initially, an analysis was conducted on 40 patients from our institution, and we identified through ultrasonographic examinations conducted between July 2021 and August 2022. These patients exhibited carotid artery plaques with mild luminal narrowing (with stenosis rates ranging from 20 to 50%, following the criteria of the North American Symptomatic Carotid Endarterectomy Trial, NASCET). All cases underwent hig

OBJECTIVE: Supraclavicular brachial plexus block is widely used for upper-limb surgery, although conventional assessment of block onset is slow and patient dependent. The perfusion index (PI) has emerged as an objective marker of block success. We compared the PI and PI ratio (PIR) with conventional clinical assessments for early identification of block onset.
PATIENTS AND METHODS: This prospective observational study enrolled 70 patients scheduled for elective orthopedic upper-limb surgeries under ultrasound-guided supraclavicular nerve blocks. Block onset was assessed at 2-min intervals usi

INTRODUCTION: Accurate and rapid assessment of the coronary microcirculation has become an important medical challenge. However, reliable and non-invasive quantitative methods to diagnose coronary microvascular disease (CMVD), select treatments for coronary artery disease (CAD), and therefore improve coronary microcirculation are lacking. Current detection methods have limitations. Therefore, we will assess whether a new detection method, the non-invasive index of microcirculatory resistance (IMR), based on computed tomography (CT) perfusion and hydrodynamics (CT-IMR), can effectively evaluate

BACKGROUND: Post-induction hypotension (PIH) is a common complication during general anesthesia. Perfusion index (PI), a non-invasive indicator reflecting peripheral tissue perfusion and hemodynamic status, has shown potential predictive value for PIH. We hypothesized that the change of PI (ΔPI) between supine and 45° semi-recumbent positions can predict PIH in patients undergoing elective general anesthesia, and a cut-off value of ΔPI for identifying a higher risk of PIH can be determined. METHODS: This prospective observational study enrolled eligible adults with American Society of Anesthes

BACKGROUND: Hemodynamic derangement during general anesthesia is associated with postoperative morbidity and mortality. Current paradigms for hemodynamic management focus on optimizing macrocirculatory parameters, which may not adequately reflect or improve microcirculatory function. The peripheral perfusion index, a noninvasive measure derived from pulse oximetry, may provide an indication of tissue perfusion status. In smaller studies, the intraoperative perfusion index has been linked to postoperative complications and mortality. This retrospective cohort study will evaluate the association

INTRODUCTION: Providing sufficient organ perfusion during cardiopulmonary bypass (CPB) is a common research topic among extracorporeal technology researchers. The application of an elevated cardiac index (CI) to enhance CPB flow, coupled with the adoption of pulsatile flow, constitutes a novel approach designed to improve organ perfusion and oxygen delivery (DO2). this study aims to assess the effects of increased CI and pulsatile flow on organ perfusion during CPB.
MATERIAL AND METHODS: In this pilot study, thirty patients scheduled for on-pump coronary artery bypass graft (CABG) surgery wit

The peripheral perfusion index (PPI) is a non-invasive indicator of peripheral tissue perfusion in neonates. However, reference values for the PPI at different postnatal time points in healthy neonates residing at high altitudes, as well as early perfusion characteristics in infants with congenital heart disease (CHD) in these settings, remain inadequately defined. The primary aim was to determine the dynamic changes in and percentile-based reference values of the PPI in healthy neonates during the first 6-72 h after birth in the high-altitude region of Tibet. A secondary, exploratory aim was
