BACKGROUND: In spite of antituberculosis chemotherapy, tuberculous (TB) pericarditis causes death or disability in nearly half of those affected. Attenuation of the inflammatory response in TB pericarditis may improve outcome by reducing cardiac tamponade and pericardial constriction, but there is uncertainty as to whether adjunctive immunomodulation with corticosteroids and Mycobacterium w (M. w) can safely reduce mortality and morbidity.
OBJECTIVES: The primary objective of the IMPI Trial is to assess the effectiveness and safety of prednisolone and M. w immunotherapy in reducing the compos

Tuberculous pericarditis (TBP) is a rare but potentially life-threatening manifestation of tuberculosis, often presenting with nonspecific symptoms and varied clinical features. The disease is characterised by inflammation of the pericardium due to mycobacterium tuberculosis, leading to complications such as effusion, tamponade, and, in chronic cases, constrictive pericarditis. TBP is associated with high mortality, particularly if not promptly diagnosed and treated. 18F-fluorodeoxyglucose positron emission tomography-computed tomography (18FDG PET-CT) has proven invaluable in diagnosing and m

Tuberculosis (TB) is a major public health challenge, primarily affecting the lungs. However, it can disseminate to any organ system, including the cardiovascular system. Among cardiac structures, the pericardium is the most commonly affected, often leading to constrictive pericarditis, whereas myocarditis remains rare. The clinical presentation of cardiac TB is heterogeneous and is frequently associated with extra cardiac manifestations, complicating the diagnostic process, particularly in the absence of definitive microbiological confirmation. In this context, advanced multimodal imaging is

BACKGROUND: In areas where Mycobacterium tuberculosis is endemic, tuberculosis is known to be the most common cause of pericarditis. However, the difficulty in diagnosis may lead to late complications such as constrictive pericarditis and increased mortality. Therefore, identification of patients at a high risk for poor prognosis, and prompt initiation of treatment are important in the outcome of TB pericarditis. The aim of this study is to identify the predictive factors for unfavorable outcomes of TB pericarditis in HIV-uninfected persons in an intermediate tuberculosis burden country.

BACKGROUND: The relationship between pericardial inflammation and subsequent constriction in tuberculous (TB) pericarditis remains poorly understood. Late gadolinium enhancement (LGE) has shown conflicting prognostic associations, suggesting that inflammation alone may not adequately reflect disease stage or reversibility. We hypothesised that inflammatory burden and oedema provide complementary information regarding subsequent pericardial tethering, and that segments without oedema would be more likely to demonstrate tethering at follow-up than oedematous segments.
METHODS: Participants with

Medical considerations for early diagnosis of tuberculous pericarditis (TBP) include Xpert MTB/RIF Ultra and TB lipoarabinomannan (LAM) antigen (Ag) tests, with immunological status influencing the performance of the latter. An evaluation of the efficiency of Xpert MTB/RIF Ultra and TB LAM Ag in detecting TBP was conducted using pericardial fluid samples from 46 patients with suspected TBP. Fifteen patients (34.1%) were diagnosed with TBP according to culture results. TB LAM Ag's sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive likelihood rat

BACKGROUND: Uganda ranks among the countries with the highest burden of TB the world and tuberculous pericarditis (TBP) affects up to 2% of people diagnosed with pulmonary tuberculosis worldwide. In Africa, it represents the most common cause of pericardial disease. Here, we present the case of a 21-year-old male patient who was diagnosed of cardiac tamponade due to tuberculous pericarditis with a positive urine LF-LAM.
CASE REPORT: We report a case of a 21-year-old male living in Oyam district, Uganda, who presented to the emergency department with difficulty in breathing, easy fatigability,

The diagnosis of tuberculous pericarditis (TBP) remains challenging. This prospective study evaluated the diagnostic value of Xpert MTB/RIF (Xpert) and T-SPOT.TB and adenosine deaminase (ADA) for TBP in a high burden setting. A total of 123 HIV-negative patients with suspected TBP were enrolled at a tertiary referral hospital in China. Pericardial fluids were collected and subjected to the three rapid tests, and the results were compared with the final confirmed diagnosis. Of 105 patients in the final analysis, 39 (37.1%) were microbiologically, histopathologically or clinically diagnosed with

We report a rapidly progressive case of tuberculous pleurisy and pericarditis. A 59-year-old, male patient with non-small-cell lung cancer commenced pembrolizumab monotherapy one month before but soon thereafter had fevers and dyspnea. Radiography revealed increased right pleural effusion, novel left pleural effusion and cardiomegaly, which had been absent 10 days earlier when a reduction in the target lesion was confirmed. Computed tomography revealed the presence of pericardial fluid. Analysis of the pleural effusion didn't detect malignancy; however, the culture was positive for tuberculosi

BACKGROUND: Tuberculous pericarditis (TBP) is associated with high morbidity and mortality, and is an important treatable cause of heart failure in developing countries. Tuberculous aetiology of pericarditis is difficult to diagnose promptly. The utility of the new quantitative PCR test (Xpert MTB/RIF) for the diagnosis of TBP is unknown. This study sought to evaluate the diagnostic accuracy of the Xpert MTB/RIF test compared to pericardial adenosine deaminase (ADA) and unstimulated interferon-gamma (uIFNγ) in suspected TBP.

BACKGROUND: Elevated adenosine deaminase (ADA) and normal tumor markers in pericardial or pleural effusion are usually considered to be a specific manifestation of benign pericardial or pleural effusion. Here we report a case of lung adenocarcinoma with pericardial metastasis with elevated ADA and normal tumor markers in pericardial effusion.
METHODS: Pericardiocentesis and lung puncture combined laboratory indexes and pathology were performed for diagnosis.
RESULTS: Analysis of pericardial fluid revealed a white blood cell (WBC) count of 2,000 x 106/L (70% for lymphocytes) with an ADA level

OBJECTIVE: To determine whether tuberculous pericardial effusion is associated with HIV infection.
DESIGN: Retrospective cohort study.
SETTING: Harare, Zimbabwe.
SUBJECTS: 61 consecutive patients presenting with tuberculous pericarditis over an 18 month period.
RESULTS: Tuberculous pericarditis was significantly associated with HIV seropositivity (odds ratio [OR] 2.46, 95% confidence interval [CI] 1.4-4.3, P = 0.001). The combination of tuberculous pericarditis and tuberculosis at another clinically obvious site (disseminated disease) was more common than pericarditis alone, and was signif

BACKGROUND: Tuberculous pericarditis begins with fibrinous and hemorrhagic pericarditis, followed by pericardial effusion, then pericardial hypertrophy, which may turn into subacute or chronic stage, and partly develop into pericarditis. Early diagnosis and treatment have very important clinical significance.
CASE SUMMARY: We present a case of an 82-year-old man with a known history of hypertension who was admitted for pleural effusion. CT scan of the chest showed findings of pleural effusion. An echocardiographic study during admission revealed a small amount of pericardial effusion (~1.2 cm

Heart failure in sub-Saharan Africans is mainly due to non-ischaemic causes, such as hypertension, rheumatic heart disease, cardiomyopathy and pericarditis. The two endemic diseases that are major contributors to the clinical syndrome of heart failure in Africa are cardiomyopathy and pericarditis. The major forms of endemic cardiomyopathy are idiopathic dilated cardiomyopathy, peripartum cardiomyopathy and endomyocardial fibrosis. Endomyocardial fibrosis, which affects children, has the worst prognosis. Other cardiomyopathies have similar epidemiological characteristics to those of other popul

Tuberculosis is a common cause of pericarditis worldwide and has been associated with pericardial masses. Non-tuberculous mycobacteria are uncommonly associated with cardiac disease, having primarily been described in cases of endocarditis. Here we describe a case of an immunocompetent patient with Mycobacterium paragordonae infection causing pericarditis with a large effusion containing pericardial masses. The patient presented with chest pain, hypoxia and biochemical evidence of inflammation (CRP 216.1 mg/L). This report illustrates a rare case of pericarditis with pericardial masses associa
