[Post-traumatic cholesteatoma of temporal bone].

Posttraumatic cholesteatoma of the middle ear is a rare condition that may present years after temporal bone fracture. We present the 36 years old woman with acquired cholesteatoma of the middle ear and perilymphatic fistula six years after a temporal bone fracture. CT scan was extremely helpful in determining the sites of fracture and fasioning the surgical approach. The middle ear was cleaned with cholesteatoma, perilymphatic fistula was sealed with 3 layers of tissues and the ossicular chain was reconstructed in one stage. The presence of fracture lines in temporal bone years after trauma m

Early computed tomography findings of the inner ear after stapes surgery and its clinical correlations.

OBJECTIVE: Pneumolabyrinth resulting from temporal bone trauma and stapes luxation has been associated with sensorineural hearing loss (SNHL). The principal purpose of this study was to determine the incidence and volume of pneumolabyrinth after stapedotomy in which iatrogenic perilymphatic fistula is created and to also correlate this with possible hearing loss and vertigo. STUDY DESIGN: Prospective study. SETTING: Tertiary referral center. PATIENTS AND METHODS: Fifty stapedotomy patients were operated on for otosclerosis, and of those 50, 20 underwent high-resolution computed tomography (

[Preoperative evaluation of translabyrinthine cholesteatomas by MRI].

PURPOSE: Hearing preservation is one of the major goals of surgical resection of invasive cholesteatomas. Patients were prospectively evaluated using a 3D-CISS MR acquisition in order to improve the detection of perilymphatic fistulae. MATERIALS AND METHODS: 16 patients (10 M, 6 F) presenting with a primary (1 case) or a secondary (post-otitis) middle ear cholesteatoma extending to the osseous labyrinth (as defined by HR-CT) were evaluated at MR (1.5 T Vision, Siemens). The 3D-CISS sequence (TR: 12.25 ms, TE: 5.90 ms, flip angle: 70 degrees, slice thickness: 1 mm, matrix: 307 x 512, FOV: 200)

Surgical management of common peripheral vestibular diseases.

PURPOSE OF REVIEW: To review the latest developments in the surgical management of common peripheral vestibular disorders. RECENT FINDINGS: Although surgery is indicated mainly for patients with Meniere's disease, other less common disorders such as benign paroxysmal positional vertigo, semicircular canal dehiscence, perilymphatic fistulae, fistulae of semicircular canals, VIII nerve vascular compression, chronic vestibular neuronitis, and dizziness following sudden sensorineural hearing loss may require surgical intervention when conservative management has failed. SUMMARY: Surgery for vest

Middle ear damages.

Middle ear damages. The eardrum and middle ear are often exposed to blunt and penetrating trauma, blasts, thermal or caustic injuries. These injuries may result in tympanic membrane perforation, middle ear haemorrhage, dislocation and fracture of the ossicular chain, perilymphatic fistula and damage to the chorda tympani and/or facial nerve. In case of life-threatening injuries and/or mass casualty incidents, middle ear trauma obviously does not take highest priority. However, middle ear lesions should be suspected and recognized as early as possible. After meticulous history taking, physical

Imaging of the windows of the temporal bone.

Imaging of the window of the temporal bone has became an important tool in the analysis of hearing loss, vertigo, tinnitus in a context of trauma, malformation, otosclerosis, and chronic otitis media. A good knowledge of the anatomy and a good technical procedure are necessary for making an efficient diagnosis. The increased thickness of the footplate may be delineated in otosclerosis, chronic otitis media, malformation, when it is measured at 0.7 mm or more in horizontal computed tomography (CT) sections. The traumatic displacement of the stapes, particularly within the labyrinths, is easily

Delayed meningitis following stapes surgery.

Perilymphatic fistulae, developing even years after stapedectomy, open a communication between the perilymph and middle ear. These fistulae constitute a potential pathway for flora of the middle ear to invade the subarachnoid space by way of the cochlear aqueduct. Our patient developed pneumococcal meningitis 14 months after stapedectomy. The history of stapedectomy in a patient with meningitis is an indication for reexploration+ion of the operative site. The appearance of fistula symptoms in patients who have had stapedectomy indicates a high risk for development of intracranial infection.

Posttraumatic endolymphatic hydrops.

A case of posttraumatic endolymphatic hydrops that was confirmed by a positive glycerin test is reported. Although head trauma has been implicated as a cause of hydrops, few such cases are reported in the literature. Careful consideration of perilymphatic fistula must be accorded before a diagnosis of posttraumatic hydrops is made.

Audiovestibular impairment presenting as a portent of vertebrobasilar infarction.

Acute audiovestibular symptoms (hearing loss and vertigo) can be caused by labyrinthine disorders such as viral labyrinthitis, cerebrovascular diseases, perilymphatic fistula and acoustic neuroma. If neurological symptoms are not present, it is more likely that viral labyrinthitis is the cause, especially in the absence of abnormal findings on imaging (Lee et al, 2002).

Hair-clip type piston artificial stapes and the safety auger.

A hair-clip artificial stapes prosthesis was used in the surgical treatment of otosclerosis in 22 ears. In nine patients followed an average of 15 months the air-bone gap stabilized at 6-7 dB. Further advantages of the prosthesis design and technique are that they minimize the risk of ischemic necrosis and perilymphatic fistula formation.

Invisible saccule in 3T MRI: insights into inner ear vulnerability and fluid dynamics.

BACKGROUND: The trabecular mesh of the inner ear plays a critical role in maintaining the structure of the vestibular organs, particularly within the pars superior. Cases have been reported in which the saccule cannot be visualized in the pars inferior lacking this structure. OBJECTIVES: To investigate the clinical features and endolymphatic hydrops (EH) and perilymphatic enhancement (PE) on MRI in ears with an invisible saccule (IS) and to clarify the mechanism of IS. MATERIALS AND METHODS: Retrospective study of 89 IS ears and 71 contralateral visible saccule (VS) ears from 406 patients wi

Perilymph fistula resulting from microfissures.

Perilymphatic fistulas are well documented in the otolaryngologic literature as a cause for both auditory and vestibular symptoms. The anatomic location of fistulas has been clinically confirmed in both the oval and round windows. Furthermore, membranous tears within the cochlea have been described post mortem and may be the cause of the remaining fistulas which are suspected but not confirmed surgically. A fourth source of perilymph fistula has previously been suggested by Okano and Harada. During the past several years, surgical confirmation of perilymph leakage from two microfissure areas h

From thunder to silence: Cochlear implantation for bilateral profound sensorineural hearing loss after lightning strike.

INTRODUCTION: Lightning strikes can cause diverse neuro-otologic injuries. Tympanic membrane rupture is common, whereas bilateral profound sensorineural hearing loss with vestibular dysfunction and perilymphatic fistula is rare. Cochlear implantation for lightning-induced deafness has been reported only once. A farm worker struck by lightning sustained second-degree burns and intracranial hemorrhages requiring emergency neurosurgical management and cervical wound closure. During intensive care, clear left-sided otorrhea prompted ENT evaluation, revealing bilateral tympanic membrane perforation

[Peripheral vertigo classification. Consensus document. Otoneurology committee of the Spanish otorhinolaryngology society (2003-2006)].

There are many different vertigo classifications and different denominations are frequently used for the same clinical processes. The Otoneurology Committee of the Spanish Society for Otorhinolaryngology and Head and Neck Pathology proposes an eminently practical classification of peripheral vertigo to facilitate a common terminology that can be easily used by the general ENT practitioners. The methodology used has been by consensus within our Society and especially among the most outstanding work groups in the area of otoneurology in Spain. Initially vertigo is divided into single-episode ver

Early alterations of cochlear function in experimental perilymph fistulas.

Alterations of cochlear function secondary to perforation of the round window membrane in guinea pigs were expected to reveal sensitive and/or specific audiometric parameters in perilymph fistulas. A perforation of the round window membrane either did not affect acoustic compound action potential thresholds, or induced a delayed, possibly conductive, inner ear hearing loss. Also, evoked suprathreshold responses deteriorated progressively and independently of the increase of compound action potential thresholds. A sudden (conductive) hearing loss and a poor response to suprathreshold stimulatio

[Labyrinthine fistulas: surgical management].

The authors presented management rules and the results of surgical treatment in perilymphatic fistulas. Materials comprise 7 cases of cholesteatoma and 6 cases after previously performed so called conservative modified operation of the ear. In 3 cases cholesteatomas were destroying the membranous structures or were being adherent to them. In these cases labyrinthectomy was performed with partial conservation of hearing in one patient. In the remained 10 cases the fistulas were cleaned and sealed with 3 layers of tissues--fascia, cartilage and fascia. After the ossicular chain was reconstructed

Ear trauma caused by a yucca plant leaf spine.

Three uncommon cases of ear trauma caused by a yucca plant leaf spine are presented. One patient presented with tympanic perforation and the second with mixed hearing loss after spontaneous closure. The third patient probably had a perilymphatic fistula with subsequent labyrinthitis and hearing loss. Although the yucca is a ubiquitous plant, to the best of our knowledge, such incidents have not been previously reported.

Surgical treatment of long-term sensorineural hearing loss due to labyrinthine fistula.

Perilymphatic fistula, usually in the round or oval window, causes a variety of symptoms, including sensorineural hearing loss. Surgical repair of these fistulas has resulted in restoration of hearing in some cases. It has been suggested that surgery must be performed within 2 months of the trauma if improvement in hearing is to occur. This paper presents the case of a patient whose fistula and resulting hearing loss had persisted ten years before surgical repair. Restoration of normal hearing and discrimination of speech occurred in the ear which had previously presented a severe sensorineura
