Arteriovenous malformations (AVMs) of the perineum are rare and can pose significant challenges during pregnancy. Literature suggests that neither surgical nor radiological interventions offer a definitive cure, making management particularly complex. We present the case of a primigravida in her mid-20s who, at 28 weeks of gestation, developed severe pain and swelling in the perineal and left gluteal regions. Imaging confirmed an AVM. OBJECTIVES: Genital talc use is associated with increased risk for ovarian carcinoma in epidemiologic studies. Finding talc in pelvic tissues in women with ovarian carcinoma who have used talc is important in documenting exposure and assessing talc's biologic potential, but tissue-based morphology studies have been rarely reported.
METHODS: We report five patient cases with documented perineal talc use, each of whom had talc (by both polarized light and scanning electron microscopy) in multiple pelvic sites distant from the perineum. Six negative-exposure control patients were also analyzed.

Extramammary Paget's disease (EMPD) is a rare intraepithelial neoplasm arising in apocrine rich area of the skin. It is divided into primary and secondary EMPD based on whether there is an underlying malignancy, either local apocrine cancers or distant neoplasms. Many cases of superinfected hidradenitis suppurativa (HS) involve multiple species of bacteria, but gas-producing infections are rare and can complicate the clinical picture. Fournier gangrene is a rare and severe necrotizing fasciitis. It is burdened with high morbi-mortality, requiring early and massive medical and surgical management. Initial treatment is based on patient's resuscitation associated with surgical debridement. Subsequently, the main challenge is the healing process and its possible sequelae. Aggressive angiomyxoma (AA) is a rare mesenchymal tumor that usually occurs in the pelvis and perineum of young females. AA can simulate Bartholin's gland cyst, abscess, lipoma, simple labial cyst, or other pelvic soft tissue tumors. Cross-sectional imaging, particularly magnetic resonance imaging (MRI), plays an essential role in the diagnosis and management of AA. We report a case of AA in a 38-year-old woman with typical MRI features. We also present the findings of a literature review on the radiological features of this disease.

A 17-year-girl presenting with features of intestinal obstruction and managed with colostomy was referred for continuing to pass feces per anus despite a functioning colostomy. She was diagnosed with a rare congenital anomaly with duplication of urethra, bladder, vagina, uterus, anus, and distal colon; all openings close together in the perineum. Excision of the obstructed duplicated colon was done. The anomaly and its features are discussed with review of literature.
