Central neuraxial anaesthesia presenting with spinal myoclonus in the perioperative period: a case series.

INTRODUCTION: Perioperative spinal myoclonus is extremely rare. Many anaesthetists and perioperative practitioners may not diagnose or manage this complication appropriately when it occurs. This case report of unusual acute spinal myoclonus following regional anaesthesia highlights certain aspects of this rare complication that have not previously been published. CASE PRESENTATIONS: A series of four consecutive patients who developed acute lower-limb myoclonus following spinal or epidural anaesthesia are described. The case series occurred at three different hospitals and involved four anaest

Clonidine suppresses plasma and cerebrospinal fluid concentrations of TNF-alpha during the perioperative period.

UNLABELLED: The analgesic properties of alpha(2)-agonists are well known. In experimental models, tumor necrosis factor (TNF)-alpha regulates adrenergic responses in the brain. Constitutive TNF-alpha, in brain regions involved in pain perception, is decreased after the administration of clonidine. We investigated patients undergoing lower-extremity revascularization. Seven patients were treated with clonidine 0.2 mg per os (low), and three patients received 0.4 mg per os clonidine (high) before surgery. Eight patients received placebo and served as controls. Continuous spinal anesthesia was pr

[Role of non-steroidal anti-inflammatory agents in the perioperative period. Usefulness and limitations].

Nonsteroidal antiinflammatory drugs (NSAIDs), including various chemical families of drugs, inhibit prostaglandin synthesis and act on the central nervous system. Prostaglandins are involved in regulation of regional circulations, cell turn-over in the gastrointestinal tract, and in primary haemostasis. The patterns of action of NSAIDs result in analgesic properties, but also in adverse effects. NSAIDs are increasingly used perioperatively, alone or associated with opioids or local anaesthetics, because of their analgesic and opioid sparing properties. Some of their adverse effects, especially

Preventing delayed graft function after kidney transplantation: The emerging role of the perioperative period.

The perioperative period involves anesthesiologists, nephrologists and transplant surgeons as key contributors to patients undergoing kidney transplantation (KT). While KT remains the preferred treatment of end-stage renal disease, postoperative complications are common. Delayed graft function occurs in up to 40% of patients undergoing deceased donor KT and is predictive of poor graft and overall survival. Emerging evidence points to several strategies during the perioperative period that may contribute towards optimizing immediate graft function. This review summarizes the available evidence

DAMPs and PAMPs in the Perioperative Period: Danger Signaling, Immune Dysfunction, and Oncologic Implications.

Background and Objectives: The perioperative period is characterized by marked biological stress responses that extend beyond direct tissue injury. Innate immune activation during surgery is largely driven by molecular danger signals. Surgical trauma, ischemia-reperfusion injury, blood transfusions, mechanical ventilation, and perioperative infections are critical events that result in the release of danger signals: damage-associated molecular patterns (DAMPs) and pathogen-associated molecular patterns (PAMPs). This narrative review examines perioperative DAMP and PAMP sources, their molecular

How Would I Treat My Own Chronic Thromboembolic Pulmonary Hypertension in the Perioperative Period?

Chronic thromboembolic pulmonary hypertension (CTEPH) results from an incomplete resolution of acute pulmonary embolism, leading to occlusive organized thrombi, vascular remodeling, and associated microvasculopathy with pulmonary hypertension (PH). A definitive CTEPH diagnosis requires PH confirmation by right-heart catheterization and evidence of chronic thromboembolic pulmonary disease on imaging studies. Surgical removal of the organized fibrotic material by pulmonary endarterectomy (PEA) under deep hypothermic circulatory arrest represents the treatment of choice. One-third of patients wit

[Consensus of Chinese experts on defecation management during perioperative period of benign anal diseases].

The significant increase in the incidence of benign anal diseases is related to the fast-paced life style, the change of dietary structure, the increase of work pressure and social psychological factors. Surgery is one of the most important treatments for benign anal diseases, while perioperative defecation management is closely related to the efficacy of surgery. In current clinical practice, there is no consensus on the management of perioperative defecation for benign anal diseases. Hence a discussion was held by the Professional Committee of Colorectal Diseases of Chinese Society of Integr

Pulmonary and Ear-Nose-Throat Considerations in the Perioperative Period for Children with Medical Complexity.

Children with medical complexity (CMC) represent a unique and growing population with multidisciplinary needs that require comprehensive management. CMC often require surgical procedures that need careful planning. They have variable baseline respiratory needs, including noninvasive ventilation, invasive mechanical ventilation, or tracheostomies. Additionally, they may have multiple coexistent conditions such as asthma or airway reactivity, oropharyngeal dysphagia, excessive oral secretions, aspiration, sleep-disordered breathing, and difficult airway scenarios. A thorough assessment by the Pu

The updated recommendations for medical nutrition in the perioperative period: a focus on early oral intake.

PURPOSE OF REVIEW: Early oral feeding after gastrointestinal surgery is safe, promotes recovery, preserves gut integrity and aligns with enhanced recovery after surgery (ERAS) pathways. However, individualization and adaption of the pathway may be necessary postoperatively particularly after upper gastrointestinal procedures, where energy and protein requirements often remain unmet. The 2025 ESPEN guideline update advocates early oral/tube feeding while addressing supplementation strategies. The purpose of this review is to discuss the updated recommendations regarding the very recent literatu
