[Peripheral vascular disease: prevalence, mortality and relationship with inflammation in hemodialysis].

Peripheral vascular disease (PVD) is a common disease among patients undergoing hemodialysis leading to increase morbidity and mortality with a high risk of inflammation and sepsis. The aim of the present study was to determinate PVD prevalence in our hemodialysis population and association with inflammation. The study sample consisted of 220 patients prevalents in hemodialysis. A basal study was made in 2001 and a follow up for 47 months. Data were collected retrospectively. PVD diagnosis was made attending to limb pulses and doppler in revisions. Diagnosis was classified as rest pain, ischem

Revascularization for peripheral vascular disease in Aboriginal and non-Aboriginal patients.

BACKGROUND: Canadian Aboriginal subjects have a higher prevalence of diabetes, renal disease, and lower extremity amputation than non-Aboriginal subjects. However, limited information is available about patient outcomes for arterial bypass surgery in Canadian Aboriginal compared with non-Aboriginal subjects. METHODS: A retrospective study of all patients undergoing revascularization for peripheral vascular disease at a tertiary care referral center was performed. RESULTS: A total of 828 procedures were performed on 678 patients between 1995 and 2002: 108 (13%) procedures on 84 (12%) Aborigin

[Amputation due to chronic peripheral vascular disease. Risks and prognosis].

A series of 181 amputations in 156 patients treated between 1985 and 1996 because of chronic peripheral vascular disease was reviewed. 107 major amputations (above- and below-knee) and 74 minor amputations (foot and forefoot) were performed. 81% of the patient population had previous vascular operations. Complications, revisions and mortality of all patients were retrospectively analyzed. The operative mortality rate of major amputations was 20% and the operative mortality rate of minor amputations was 9%. By a vessel-reconstruction before amputation the complication- (wound healing) and revis

A brief history of Doppler ultrasound in the diagnosis of peripheral vascular disease.

The history of Doppler ultrasound in peripheral vascular diagnosis is considered in terms of basic developments, clinical applications and impact on medical practice. Many early developments occurred at Osaka University in Japan and the University of Washington in the United States. Through progressive steps, Doppler ultrasound technology has provided clinical applications in blood-flow sensing, waveform analysis, localizing blood flow and two-dimensional (2-D) mapping of blood flow. An important advance was the development of duplex and color Doppler scanning. Real time velocity measurements

Control of risk factors in peripheral vascular disease. Management of hypertension.

Hypertension is a highly prevalent disorder and a potent factor in promoting atherosclerosis. Identifying and controlling high blood pressure are major goals in caring for the vascular patient. A focused history and physical examination and selected laboratory studies will identify potential underlying (and remediable) causes of hypertension, target organ effects, and other risk factors for vascular disorders. Lifestyle changes and pharmacologic therapy tailored for this patient population are discussed, along with the importance of considering whether renovascular hypertension is present.

Prospective evaluation of peripheral vascular disease in heart transplant recipients.

PURPOSE: Retrospective reviews suggest that the progression of peripheral vascular disease (PVD) may be accelerated in heart transplant recipients. This study was undertaken to determine the incidence and to identify those risk factors that may be associated with the development or progression of PVD in these patients. METHODS: Between January 1990 and December 1993 a prospective vascular screening protocol including abdominal ultrasonography, Doppler-derived ankel-brachial pressure indexes (ABI), and carotid artery duplex imaging was added to the routine preoperative and annual postoperative

Efficacy of home health care in patients with peripheral vascular disease.

This investigation was designed to study the effects of home health care (HHC) on patients who have been hospitalized with peripheral vascular disease. For a patient to have HHC, the patient had to have a defined wound, educational needs, or both. Sixty patients, 30 with HHC and 30 without, were contacted approximately 30 days after their last hospital discharge. The 30 patients with HHC were deemed to be at increased risk because of multisystem disease with multiple medications, infirmity, early senility, and often complex wounds. In a prospective fashion, each patient was interviewed by eith

The significance of blood pressure measurements in patients with peripheral vascular disease.

The pressure drop across a stenosis is related only to its geometry and the flow through it. Flow may be manipulated by changing peripheral resistance, enabling information to be gained about the severity of the stenosis. As changes in blood flow may produce changes in local blood pressure in patients with peripheral arterial disease, measurements of blood pressure should be made at flow conditions appropriate to the symptoms under investigation, or should be performed by methods which do not intrinsically alter flow.

Editor's Choice - Follow-up of Patients After Revascularisation for Peripheral Arterial Diseases: A Consensus Document From the European Society of Cardiology Working Group on Aorta and Peripheral Vascular Diseases and the European Society for Vascular Surgery.

Peripheral arterial diseases comprise different clinical presentations, from cerebrovascular disease down to lower extremity artery disease, from subclinical to disabling symptoms and events. According to clinical presentation, the patient's general condition, anatomical location and extension of lesions, revascularisation may be needed in addition to best medical treatment. The 2017 European Society of Cardiology guidelines in collaboration with the European Society for Vascular Surgery have addressed the indications for revascularisation. While most cases are amenable to either endovascular

Premature peripheral arterial disease is associated with worse outcomes after endovascular peripheral vascular intervention.

OBJECTIVE: Premature peripheral arterial disease (PAD), defined as onset before the age of 50 years, is increasing in prevalence. The clinical characteristics, treatment patterns, and mid- and late-term outcomes of patients with premature PAD undergoing endovascular peripheral vascular intervention (PVI) are not well-defined. METHODS: The linked Vascular Quality Initiative and Medicare dataset was used to examine those who underwent PVI for arterial occlusive disease from January 2017 to December 2018. The exclusion criteria were acute limb ischemia and concomitant open revascularization. Pat

Differential gene expression in peripheral vascular smooth muscle cells of patients with peripheral artery disease.

BACKGROUND: The role of peripheral vascular smooth muscle cells (VSMCs) in vascular calcification has been overlooked compared with that of the major VSMCs. This study aimed to investigate the differentially expressed genes (DEGs) of peripheral VSMCs in patients with critical limb ischemia (CLI) results from peripheral arterial disease and Chronic Kidney disease (CKD). METHODS: We isolated peripheral VSMCs from the posterior tibial artery of 6 patients with CKD who underwent below-knee amputation for CLI. Using normal human aortic VSMCs as a control, we cultured all samples in normal and high

Editor's Highlight: PCB126 Exposure Increases Risk for Peripheral Vascular Diseases in a Liver Injury Mouse Model.

The liver is vital for xenobiotic and endobiotic metabolism. Previously, we demonstrated that a compromised liver worsened toxicity associated with exposure to polychlorinated biphenyls (PCBs), through disruption of energy homeostasis. However, the role of a compromised liver in defining dioxin-like PCB126 toxicity on the peripheral vasculature and associated inflammatory diseases is yet to be studied. This study investigated the effects of PCB126 on vascular inflammation linked to hepatic dysfunction utilizing a liver injury mouse model. Male C57Bl/6 mice were fed either an amino acid control

Extracellular Matrix Remodelling in the Human Sural Nerve in Peripheral Vascular Disease.

Background and Objectives: Peripheral nerve adaptation to different pathological conditions is accompanied by the remodelling of the nerve's extracellular matrix (ECM). Ischemic conditions caused by peripheral vascular disease are known to affect the function of peripheral nerves; however, the morphological changes to their ECM remain insufficiently examined and understood. Bearing in mind that alterations in collagen I, collagen IV, and laminin content may compromise peri- and endoneurial integrity, the aim of our study was to analyse whether peripheral vascular disease (PVD) induces distinct

The splicing factor kinase SRPK1 is a therapeutic target for peripheral vascular disease.

In peripheral arterial disease (PAD), antiangiogenic VEGF-A165b isoform overexpression in monocytes contributes to impaired collateralization. Serine-arginine protein-kinase-1 (SRPK1) regulates VEGF splicing. To determine whether SRPK1 controlled monocytic VEGF, impairing collateralization, we investigated SRPK1 inhibition and monocyte-specific knockout in mouse models of and in human monocytes from PAD. VEGF-A165b activity was measured by coculture of monocytes from patients with PAD with endothelial cells with SRPK1 inhibition. Mice with impaired revascularization due to soluble-frizzled-rel
