Shunt complications are reported to occur at a rate of approximately 26%. One of the less frequent but important complications is that of the pseudocyst. Since Harsh's first mention of a periumbilical cyst associated with a shunt in 1954, 44 cases have been reported in the literature. These are reviewed in addition to 12 cases of our own. From the collected series several features about the etiology and management become apparent. The most common presentation is that of abdominal distension and/or pain rather than shunt malfunction. Diagnosis is then readily made with ultrasonography. Etiologi

BACKGROUND: Pleural effusions and ascites are associated with distressing symptoms like dyspnoea, intestinal obstruction, vomiting, nausea and pain. In patients with underlying malignancy, the prognosis is limited to few months. After unsuccessful medical treatment, surgical and experimental palliative treatment is indicated.
METHODS: This review includes a systematic analysis of surgical, experimental and palliative options.
RESULTS: In patients with pleural effusions, thoracocentesis, permanent percutaneous drainage, thoracoperitoneal shunts as well as pleurodesis by tubes or thoracoscopy

The case was a 71-year-old woman who had right renal cell carcinoma with inferior vena cava invasion. After the nephrectomy, chylous ascites appeared. Despite conservative treatment, chylous ascites persisted. No lymphatic leakage was identified on transpedal lymphoscintigraphy, and retrograde thoracic duct cannulation from the left subclavian vein failed. A chylous pseudoaneurysm was identified on computed tomography and embolized percutaneously; however, chylous ascites persisted. Therefore, we performed transhepatic lymphangiography and embolization before placement of a peritoneovenous shu

Postoperative chylous ascites is a rare condition that can be caused by abdominal and pelvic surgery. The mortality rate associated with untreated postoperative lymphorrhea is as high as 50%. Conservative management is the primary treatment, and most patients improve. However, some patients continue to exhibit high-volume chylous ascites and need invasive intervention. Many surgical series have shown that the outcomes of patients with chylous ascites were unfavorable. Therefore, the need for minimally invasive interventional radiology procedures, such as intranodal lymphangiography, thoracic d

Chylous ascites is an uncommon complication seen in gynecological malignancies. The incidence of postoperative chylous ascites in gynecological malignancies is 0.17%. It is the extravasation of milky chyle into the peritoneal cavity due to lymphatic obstruction or injury and is diagnosed by the detection of triglyceride levels greater than 200 mg/dl in ascitic fluid. The site of leakage can be determined by lymphangiography or lymphoscintigraphy. The majority of the cases of chylous ascites can be treated by conservative methods, such as a dietary modification to a low-fat high-protein diet, b

The accumulation of chylous fluid in the abdominal cavity is an infrequent yet alarming complication in abdominal surgery. Excessive lymphatic leakage is occasionally encountered in the course of operations at the base of the mesentery or retroperitoneum. A source can usually be identified and the leak controlled at the time of laparotomy by suture or clip. In the region of presumed leakage the importance of these efforts is reinforced by the are patient in whom such leakage persists, creating problems in recognition and management postoperatively. The development of this complication in a pat

Significant pleural effusions are infrequently noted in patients with cirrhosis of the liver. A large effusion (hepatic hydrothorax) occasionally appears during the course of the disease. The fluid in the pleural space is believed to be derived from ascitic fluid that may accompany hepatic cirrhosis. Although the exact mechanism is somewhat controversial, it appears that the ascitic fluid is transported directly into the pleural space. A therapeutic thoracentesis, usually accompanied by a paracentesis, may be necessary to relieve acute symptoms. Long-term management, however, centers around el

Portal hypertension, defined by a constant increase of the portal vein pressure over 5 to 10 mmHg, is usually caused by a pre-, intra- or posthepatic obstruction of the portal blood flow. Advances in ultrasound techniques like duplex or colour doppler enabled portal hypertension to be diagnosed safely in a noninvasive way. Invasive procedures for the measurement of portal pressure or blood flow are just applied for scientific and pharmacological questions. The therapy of portal hypertension concentrates on the treatment of its complications: The management of the acute variceal hemorrhage shou

We report the case of a right atrial-ventricular chamber thrombus discovered four years after insertion of a Le Veen shunt for treatment of refractory ascites. A two-dimensional echocardiogram, performed after the discovery of an isolated systolic murmur, demonstrated a "tumorlike" mass seated in both the right atrium and the right ventricle. The mass was surgically removed and histologic examination confirmed that it was a thrombus developed at the tip of the catheter. We propose that two-dimensional echocardiography should be performed periodically for the survey of such intracardiac devices

A woman aged 75 years developed chylous ascites day 5 after emergency abdominal aortic aneurysm repair. She was still on artificial ventilation at the time of diagnosis. The first symptoms were high ventilation pressure, an elevated diaphragm, and abdominal distention. The patient was treated conservatively with drainage and total parenteral nutrition and given tube feeding without long-chain triglycerides. Unfortunately, the patient died of sepsis on day 25. Chylous ascites is an unusual complication of retroperitoneal surgery. Abdominal paracentesis is a simple means to confirm the diagnosis

Chylous ascites is a rare complication in abdominal aortic surgery, there have been thirteen previously reported cases of chylous ascites after aortic surgery, both for aneurysmectomy or revascularization. This complication always represents a difficult problem in patient management because of the serious failures of the nutritional balance, the potentially dangerous immunological deficit and the frequent respiratory function impairment overcoming with various mechanisms. Analysis of ascitic liquid, which shows the aspect and chemical composition of chyle, is required to make the diagnosis. Se

This paper reports a case of refractory ascites in a patient with gastric cancer. A peritoneo-venous shunt(PVS)was inserted in the patient, which contributed to extending the duration of home-based care as well as improving the patient's quality of life. The patient was a female in her 70s. She was diagnosed with gastric cancer and underwent total gastrectomy. Five years and 7 months after the surgery, she was diagnosed with peritoneal recurrence. Ascites temporarily decreased following chemotherapy, but gradually worsened thereafter. Since the patient required frequent puncture drainage for t

Ascites is the most common complication in patients with decompensated cirrhosis. Approximately 50% of patients with compensated cirrhosis will develop ascites over a 10-year period. This occurrence is an important milestone in the natural history of end-stage liver disease because only 50% of patients survive 2 to 5 years (depending on the cause of cirrhosis) after its onset. Salt restriction and diuretics are the mainstays of therapy, and these measures are effective in approximately 90% of patients. Large-volume paracentesis or transjugular intrahepatic portosystemic shunt can be used in pa

Personal experience of 31 patients suffering from intractable ascites due to advanced liver cirrhosis between 1978 and 1987 is reported. Seventeen patients were selected for a peritoneojugular shunt: in 3 patients the Le Veen shunt was performed and in 14 the Denver shunt was preferred. The high postoperative morbidity and mortality due to liver failure, DIC, hepatorenal syndrome, bleeding, sepsis and cerebral thrombosis is pointed out. Careful selection of patients to be submitted to this surgical procedure is essential because of the high morbidity due to ascites reinfusion. DIC has to be di

Hyponatremia complicates ascitic hepatic cirrhosis with frequency and gravity related to the gravity of the cirrhosis itself. When hyponatremia develops, it worsens the already present secondary hyperaldosteronism and makes therapy with spironolactone inefficacious. From a pathophysiologic viewpoint a pathogenetic role in determining hyponatremia is attributable to the reduced plasmatic renal perfusion; in several patients a syndrome of inappropriate ADH secretion develops. Other neurohormonal systems (catecholamines, prostaglandins, natriuretic hormones) are probably very important in modifyi
