Superficial peroneal neuropathy is a known complication of foot and ankle arthroscopy. A 27-year-old man developed pain and paresthesia on the medial side of the dorsum of his left foot after ankle arthroscopy. An electrodiagnostic study revealed conduction abnormality in the medial branch of superficial peroneal nerve, in which neuroma-in-continuity was subsequently detected by ultrasonography. After neuroma excision and nerve graft, the subject's neuropathic pain was substantially improved.

BACKGROUND: Peroneal neuropathy at the fibular head (PNFH) is a mononeuropathy that typically presents with drop foot and sensory abnormalities over the skin area innervated by the peroneal nerve.
OBJECTIVE: The aim of the present study was to evaluate neuropathic pain in patients with PNFH.
METHODS: Patients with clinical and electrodiagnostic features consistent with PNFH associated with weight loss, leg postures, or prolonged sleep were included in the present retrospective cohort study. Nerve conduction studies were performed in the bilateral lower extremities of all patients. The Leeds

BACKGROUND: The scratch collapse test is a recently described provocative test for diagnosis of peripheral nerve compression.
METHODS: The scratch collapse test was studied prospectively in 24 consecutive patients with a diagnosis of common peroneal nerve compression neuropathy. The diagnosis was confirmed by history, physical examination, and electrodiagnostic testing. Provocative testing by the scratch collapse test and Tinel's sign was performed.
RESULTS: The scratch collapse test showed a sensitivity of 0.77 and a specificity of 0.99, while the Tinel's sign showed 0.65 and 0.99, respecti

Adequate hemostasis is extremely important in extensor reconstruction of the knee and possibly all knee injury. Hemarthrosis postoperatively contributes to postoperative discomfort and may increase the risk of infection. Hematoma formation is also responsible for neurovascular complications. Peroneal nerve palsies were associated with hematoma formation in this series and probably represented a pressure palsy. Release of the tourniquet with good hemostasis and a pressure dressing prior to cast application will aid in the prevention of hemarthroses and consequent neurovascular complications. Pe

BACKGROUND: Anatomic and clinical studies show many variants of the superficial peroneal nerve (SPN) course and branching within the compartments and at the suprafascial layer. The anatomy of the transition zone from the compartment to the subcutaneous layer has been occasionally described in the literature, mainly in studies reporting the intraseptal SPN variant in 6.6% to 13.6% of patients affected by the SPN entrapment syndrome. Despite the little evidence available, the knowledge of the transition zone is relevant to avoid iatrogenic lesions to the SPN during fasciotomy, open approaches to

This review summaries the knowledge of the treatment of peroneal nerve palsy. Isolated peroneal nerve palsy is often seen after fracture of the knee or knee dislocation. In cases with chronic peroneal nerve palsy and foot drop, tendon transfer of the posterior tibial muscle tendon to the dorsum of the foot is a possibility to be considered. This procedure is indicated for isolated peroneal nerve palsy with good ankle mobility, good strength of the posterior tibial muscle and no chance of spontaneous remission.

BACKGROUND: The common peroneal nerve (CPN) is an important structure of the lower limb and is at risk of injury during total knee arthroplasty. The aim of this study was to use a tibial reference system to determine the position of the CPN relative to the knee center and popliteus.
METHODS: Two hundred consecutive knee magnetic resonance images at the level of a standard tibial arthroplasty cut were evaluated for (1) distance of the CPN from the posterolateral capsule; (2) angle of the CPN from the center of the tibial anteroposterior axis; and (3) location of CPN with respect to the poplite

BACKGROUND: Common peroneal nerve (CPN) entrapment at the fibular head is the most common nerve entrapment syndrome at the lower limbs. Motor deficits predominate and the risk of persistent functional impairment is the main concern. The objective was to evaluate outcomes of neurolysis and to evaluate the benefits of performing surgery early.
MATERIALS AND METHODS: We retrospectively reviewed the medical charts of 15 patients (mean age, 32 years) treated with neurolysis. The diagnosis was idiopathic CPN entrapment in ten patients, indirect nerve injury with CPN paralysis due to an ankle injury

BACKGROUND: Sciatic nerve lesions comprise the largest subset of lower extremity nerve injuries. Military gunshot injury reports in civilians are not as predominant as wartime experience. Most sciatic nerve injuries result in deficits to the peroneal component. This study aims to report experience with civilian sciatic nerve injuries and to suggest the mechanism behind the peculiarity of the common peroneal component in thigh-level injuries.
METHODS: A retrospective study was conducted on 1,463 civilian patients with military gunshot sciatic nerve injury diagnosed and evaluated by neurophysio

Persistent recalcitrant dorsolateral foot pain after ankle sprain cannot always be explained by known anatomic nerve pathways. To determine whether an impingement of a lateral branch of the deep peroneal nerve might be responsible for atypical pain, we conducted a cadaveric anatomic study to identify the anatomy and course of the nerve. Furthermore, using this information, we conducted a clinical study to determine if targeted treatment to a lateral branch of the deep peroneal nerve would resolve these symptoms. We dissected 22 cadaveric feet to identify a large lateral branch of the deep pero

We investigated the blood supply of the common peroneal nerve. Dissection of 19 lower limbs, including six with intra-vascular injection of latex, allowed gross and microscopic measurements to be made of the blood supply of the common peroneal nerve in the popliteal fossa. This showed that a long segment of the nerve in the vicinity of the fibular neck contained only a few intraneural vessels of fine calibre. By contrast, the tibial nerve received an abundant supply from a constant series of vessels arising directly from the popliteal and posterior tibial arteries. The susceptibility of the co

Peroneal nerve ganglion cysts typically present because of a palpable mass or symptoms and signs of entrapment neuropathy, including pain, diminished sensation, and motor weakness. Surgical treatment is usually successful when performed early, but, when diagnosis is delayed, intraneural growth and invasion may cause irreversible axonal injury and footdrop. This case report illustrates the importance of timely diagnosis when treating a ganglion of the peroneal nerve and reviews the appropriate workup, differential diagnosis, and treatment.

Injury to the deep peroneal nerve in the foot and ankle may result from trauma, repetitive mechanical irritation, or iatrogenic harm. The nerve is most susceptible to injury along its more distal anatomic course. Dissection of 17 cadaver specimens was undertaken to describe the course of the deep peroneal nerve and quantify its branch patterns. In the distal one third of the leg, the nerve was located superficial to the anterior tibial artery between the tibialis anterior and extensor hallucis longus muscles. Typically, the nerve crossed deep to the extensor hallucis longus tendon to enter the

The subcutaneous (sensory) portion of the superficial peroneal nerve was dissected in 25 cadaver lower limbs under loupe magnification. Three distinct branching pattern types were noted. Type A (72%) consisted of a pattern where the nerve penetrated the crural fascia to become subcutaneous at an average distance of 12.3 cm proximal to the ankle joint, then divided at a mean distance of 4.4 cm proximal to the ankle into two major branches: a large medial dorsal cutaneous nerve and a smaller more laterally located intermediate dorsal cutaneous nerve. Type B (16%) consisted of a pattern where bot
