Traumatic Injury of the Petrous Part of the Temporal Bone: Keys for Reporting a Complex Diagnosis.

Fractures of the petrous part of the temporal bone are a common lesion of the base of the skull; most of these fractures result from high-energy trauma. In patients with multiple trauma, these injuries can be detected on CT scans of the head and neck, where the direct and indirect signs are usually sufficient to establish the diagnosis. It is important to these fractures because the temporal bone has critical structures and the complexity of this region increases the risk of error unless special care is taken. This article reviews the key anatomical points, the systematization of the imaging f

Giant cell tumor of the petrous temporal bone with direct invasion into the middle ear.

Giant cell tumor (GCT) is an uncommon primary bone tumor. The GCT mostly involves the extremity long bones. Rare is a GCT with tumoral growth in the cranium, especially other than the sphenoid bone. We herein report a 31-year-old male patient presenting with ipsilateral hearing loss and episodes of vertigo due to a large lobulated GCT of the right petrous temporal bone extending into the contiguous middle cranial fossa, infratemporal space, middle ear and external ear canal. He was treated with a macroscopically radical tumor excision followed by conventional megavoltage radiotherapy. The pati

Middle Fossa Approach for Resection of a Petrous Bone Hemangioma Compressing the Geniculate Ganglion.

Facial nerve hemangiomas are a rare entity of skull base lesions that arise within the temporal bone and affect the seventh cranial nerve.1 They are vascular malformations arising from the vascular plexuses surrounding the nerve. Although slow growing and overall benign in nature, they can cause significant facial nerve dysfunction even at small sizes.2 Facial nerve hemangiomas can arise within different segments of the facial nerve within the temporal bone, but most commonly arise near the geniculate ganglion.3 We describe the case of a 34-year-old female who presented with progressive right

A Message from a Narrowed Internal Auditory Canal in a Patient with a Hyperpneumatized Petrous Bone.

Hyperpneumatization of the temporal bone (HPTB) is a rare finding, and its clinical impact is poorly described in literature. We report the case of a 34-year-old woman with no previous otologic condition, complaining of paroxysmal vertigo and right pulsatile tinnitus shortly after a first scuba diving session. Clinical neurotologic assessment found a right vestibular impairment. Cranial tomodensitometry showed a bilateral hyperpneumatization of the petrosal air cell system. Brain magnetic resonance imaging to rule out any retrocochlear pathology was normal, although the cochleo-vestibular nerv

Vestibular schwannoma extending into the tympanic cavity and jugular fossa by invasion of the petrous bone.

Vestibular schwannomas usually originate in the internal acoustic meatus, and gradually extends into the cerebellopontine cistern. Invasive growth into the petrous bone is extremely rare. We describe a case of a vestibular schwannoma that aggressively extended into the petrous bone and extracranial space. This may have arisen because of an unusually peripheral site of origin on the vestibular nerve.

A case of giant cell reparative granuloma of the petrous bone: demonstration of the proliferative component.

BACKGROUND: Giant cell reparative granuloma (GCRG) is an uncommon benign lesion of the bone. It typically arises in the mandible and rarely involves the skull. The cytologic nature and genesis of the involved cells are poorly understood. METHODS AND RESULTS: We report a case of GCRG in the petrous bone of a 3-year-old girl. One year following gross total removal, the granuloma recurred locally and was resected en bloc at the second surgery. Histologically, the lesion was composed of oval or spindle-shaped stroma cells admixed with a number of multinucleated giant cells. Immunohistochemical st

The mysteries of DNA preservation in bone: A comparative study of petrous bones and metacarpal epiphyses using ATR-FTIR spectroscopy.

A comparative analysis of 26 petrous bones and epiphyses of metacarpals from the Second World War era revealed no significant differences in DNA yield or success in STR typing. This unexpected parity in DNA preservation between the petrous bone, a renowned source of endogenous DNA in skeletal remains, and the epiphyses of metacarpals, which are porous and susceptible to taphonomic changes, is surprising. In this study, we introduced ATR-FTIR spectroscopy as an approach to unravel the correlation between bone molecular structure and DNA preservation. Metacarpals and petrous bones with same taph

Evaluating fluctuating and directional asymmetry in postnatal petrous bone growth: a 3D morphometric study.

PURPOSE: The objective was to assess fluctuating asymmetry (FA) and directional asymmetry (DA) of the petrous bone (PB) and the petromastoid V (PMV) in children using 3D morphometric data. METHODS: CT scans of normal children (0 to 30 months) were included. After segmentation, the geometric characteristics were calculated (volume, three inertial semi-axes (ISA), centroid, distance, angle). The FA and DA were studied according to stages of postural development and sex. RESULTS: 116 CT scans were studied (70 boys, 46 girls). The two sides of the PB and PMV were symmetric (FA between 1.87% and 3.

Outcome of different facial nerve managements in petrous bone cholesteatoma patients with facial paralysis.

BACKGROUND: Active Facial nerve (FN) management, including decompression, end-to-end or rerouting anastomosis, and grafting consistently plays an important role in the surgical management of petrous bone cholesteatoma (PBC), and postoperative FN function is also a major concern for surgeons. The aim of this study is to analyze the long-term FN function outcomes in PBC patients with FN paralysis who have underwent different managements and to explore the prognostic factors. METHODS: A retrospective analysis of 160 PBC patients with preoperative FN paralysis was conducted, and long-term FN func

Increased Petrous Bone Uptake on 18 F-PSMA-1007 PET/CT Due to Otospongiosis.

We report the case of a 74-year-old man who had undergone radical prostatectomy for prostatic cancer 6 months earlier. Elevated prostate-specific antigen during follow-up prompted 18 F-prostate-specific membrane antigen (PSMA) ligand PET/CT ( 18 F-PSMA-1007 PET/CT) to search for new manifestations of prostate cancer, revealing an increased focal uptake (SUV max , 5.9) in the left cochlear/pericochlear temporal bone and equivocal PSMA-RADS-3a external iliac nodes. Comparison with cone-beam CT and MRI showed that the focal temporal bone uptake corresponded to the typical morphological features o

Repair of cerebrospinal fluid leak via petrous bone using multilayer technique with hydroxyapatite paste.

OBJECTIVE: Repairing bone defects after drilling in skull base surgery is difficult in deeper locations, the posterior wall of the internal auditory canal, and the petrous apex. These defects traditionally have been repaired by soft tissue, such as fascia, muscle, or adipose tissue, coupled with lumbar drainage for several days postoperatively. METHODS: To repair cerebrospinal fluid (CSF) leakage through air cells in the petrous bone, we applied a multilayer technique using hydroxyapatite cement (HAC) in three patients with postoperative CSF leakage. The opening of air cells was drilled widel

Langerhans cell histiocytosis of the petrous bone with sudden sensorineural hearing loss. Case report.

A 56-year-old woman with Langerhans cell histiocytosis of the petrous bone presented with sudden onset of sensorineural hearing loss and vertigo without other neurological impairment, mimicking idiopathic sudden sensorineural hearing loss. Differential diagnosis was difficult until neuroimaging demonstrated a lesion of the petrous bone. The patient eventually underwent removal of the lesion via the transpetrosal approach, and received postoperative chemotherapy consisting of vinblastine, methotrexate, 6-mercaptopurine, and prednisolone. Although her hearing did not recover, complete remission
