Subcutaneous phaeohyphomycosis caused by Exophiala equina, with susceptibility to eight antifungal drugs.

A case of subcutaneous phaeohyphomycosis caused by Exophiala equina is reported in a 75-year-old female, who showed subcutaneous abscesses on both forearms for 8 months. A lesion was initiated by inoculation with a spine from a tree. Histopathologically, suppurative granulomatous inflammation was present and short hyphal elements were observed. Upon culture greyish-black, velvety colonies of a black yeast were obtained after 3 weeks. The strain grew well at 25 °C, but poorly at 37 °C. After sequencing the internal transcribed spacer domain and the partial β-tubulin gene, the fungus was identif

A case of phaeohyphomycosis caused by Exophiala oligosperma successfully treated with local hyperthermia.

A 58-year-old Japanese woman who was engaged in dairy farming presented with multiple subcutaneous nodules and abscesses on the dorsum of her left hand from 5 months ago. These had been unsuccessfully treated with oral itraconazole. The patient had a history of Sjögren syndrome and diabetes mellitus, for which she had been taking oral prednisolone for 10 years. Direct microscopy of a pus sample treated with potassium hydroxide (KOH) showed brownish-red branching hyphae. In fungal culture, black colonies covered with gray-white villi were formed. Slide culture showed conidiogenesis from an anne

First case of disseminated phaeohyphomycosis in an immunocompetent individual due to Alternaria malorum.

A 27-year-old Iranian, previously healthy male presented with sub-cutaneous necrotic lesions with a localized dermatosis affecting the anterior chest, neck and face. These lesions consisted of singular, well-defined verrucous plaques which gradually developed and disseminated over time. The dermatosis was followed by the development of necrotic swollen lesions localized on the hard palate. The patient did not recall any history of trauma or puncture at any of the sites of infection. While histopathological examination of periodic acid-Schiff (PAS) stained material revealed irregular, unbranche

Cutaneous phaeohyphomycosis caused by Exophiala spinifera in a patient with systemic lupus erythematosus.

Exophiala spinifera can induce both phaeohyphomycosis and chromomycosis. To date there have been 18 human infections caused by E. spinifera in the English literature. A case of E. spinifera-induced phaeohyphomycosis in a patient with systemic lupus erythematosus (SLE) is described. Direct microscopic examination of the pus showed branched, septate and chained hyphae and spores. A dark green velvety colony grew on Sabouraud dextrose agar. Slide culture showed branched, septate hyphae and spine-like annellated conidiophores. Histopathological biopsy revealed yellowish brown hyphae and spores. Th

Subcutaneous phaeohyphomycosis caused by Exophiala xenobiotica in a non-Hodgkin lymphoma patient.

Phaeohyphomycosis is a rare fungal infection that is more commonly associated with immunocompromised patients. We present a case in which a 77-year-old woman with non-Hodgkin lymphoma developed subcutaneous phaeohyphomycosis caused by Exophiala xenobiotica. E. xenobiotica is a dematiaceous hyphomycete that was recently identified as a segregant of the E. jeanselmei complex. The patient was successfully treated with local excision of the lesions and post-surgical oral itraconazole. The latter was administered with the aim of preventing systemic dissemination in this immunocompromised patient.

Exophiala spinifera as a cause of cutaneous phaeohyphomycosis: case study and review of the literature.

Exophiala spinifera has been reported as an agent of cutaneous disease 18 times in the literature. Clinical presentations of cutaneous lesions vary widely, including erythematous papules, verrucous plaques, and deep subcutaneous abscesses. The clinical distribution and course of disease are also variable, depending on the age and immune competency of the patient. Histologic appearance occurs in one of two patterns--phaeohyphomycosis or chromoblastomycosis. While E. spinifera appears to be susceptible to multiple antimicrobial agents in vitro, clinical experience with treatment modalities has b

A case report of phaeohyphomycosis caused by Cladophialophora bantiana treated in a burn unit.

Black molds are a heterogeneous group of fungi that are distributed widely in the environment and that occasionally cause human infection. The spectrum of disease includes mycetomas, chromoblastomycosis, sinusitis, and superficial, cutaneous, subcutaneous, and systemic phaeohyphomycosis. Cladophialophora bantiana, an agent of phaeohyphomycosis, causes rare infections mainly of the central nervous system. Extracerebral involvement is uncommon, and only a few cases have been reported. We present the case of a 32-year-old immunosuppressed female who developed a cutaneous phaeohyphomycosis from C.

Phaeohyphomycosis caused by Chaetomium globosum in an allogeneic bone marrow transplant recipient.

Bone marrow transplant recipients are highly susceptible to opportunistic fungal infections. This is the report, of the first case of a Chaetomium systemic infection described in Brazil. A 34 year-old patient with chronic myeloid leukemia underwent an allogeneic sibling matched bone marrow transplant. Seven months later, he developed systemic infection with enlargement of the axillary and cervical lymph nodes. Culture of the aspirates from both lymph nodes yielded Chaetomium globosum. The infection was successfully treated with amphotericin B. The increasing population of immunosupressed patie

Concurrent chromoblastomycosis and subcutaneous phaeohyphomycosis by different pathogens in a renal transplant recipient.

Chromoblastomycosis and subcutaneous phaeohyphomycosis are implantation mycoses caused by melanized fungi. We present a unique case of concurrent chromoblastomycosis by Fonsecaea monophora and phaeohyphomycosis by Medicopsis romeroi presenting as polypoidal wrist lesion and painless cystic toe lesion respectively in a renal transplant recipient. The diagnosis was confirmed by microbiological (including molecular identification by sequencing of ITS region of rDNA) and histopathological examination. The wrist lesion was surgically excised. Oral itraconazole and topical luliconazole led to healin

Cystic Subcutaneous Phaeohyphomycosis Caused by Cladophialophora Species in an Elderly Leprosy Patient.

Phaeoid (dematiaceous) fungi are a diverse group of species characterized by their production of the pigment dihydroxynaphthalene melanin. Although phaeoid fungi do not usually cause human infections, they are commonly found in nature as contaminants. These fungi are present in decomposing vegetation, rotting timber, and soil. However, an increase in infections is probably due to the increase in the population of individuals with compromised immune systems. An elderly female patient from the southernmost part of India, who was previously treated for leprosy, presented with multiple boggy swell

Diagnosis and successful management of a rare case of phaeohyphomycosis in a Doberman pinscher dog.

Skin affections in canines are common and their treatment depends upon the pathogens involved. Delayed or no response to traditional treatment indicates the involvement of unusual etiology or the multidrug resistance. This report describes the diagnosis and therapeutic management of a rare case of phaeohyphomycosis caused by Alternaria alternata complicated with Pseudomonas aeruginosa associated pyoderma in a Doberman pinscher dog. Identification of the etiological agents was based on microscopy and culture of the skin lesions and further confirmation by polymerase chain reaction (PCR) amplifi

A rare case of phaeohyphomycosis caused by Fonsecaea pedrosoi in a child with nephrotic syndrome.

Dematiaceous fungi are the etiological agents of phaeohyphomycosis and are now increasingly being recognized for causing disease in humans. A high level of suspicion and routine fungal cultures are required to identify these cases. There is no consensus regarding their management. Here, an unusual presentation of phaeohyphomycosis (secondary to Fonsecaea pedrosoi) presenting as a disseminated infection in a case of nephrotic syndrome is described.

Case of cutaneous phaeohyphomycosis caused by Phaeoacremonium sp. in a renal transplant recipient.

We describe a case of cutaneous phaeohyphomycosis in a 61-year-old man receiving re-dialysis treatment for renal failure of a transplanted kidney. He was immunocompromised with steroid and cyclosporin A at onset of an asymptomatic abscess on his right forearm. The abscess arose at the site of a skin injury approximately 1 year prior. Grayish molds isolated from the lesion were morphologically compatible with Phaeoacremonium sp. but nucleotide sequence data of internal transcribed spacer regions of ribosomal RNA gene, actin and β-tubulin genes were unlike those of any described species. He was

Cerebral phaeohyphomycosis due to Cladophialophora bantiana in a Huacaya alpaca (Vicugna pacos).

Cerebral phaeohyphomycosis, caused by Cladophialophora bantiana, was diagnosed in a Huacaya alpaca (Vicugna pacos). An 8-year-old, intact male Huacaya alpaca from a farm in Indiana was found dead at pasture and submitted to the Animal Disease Diagnostic Laboratory at Purdue University for necropsy examination. Gross lesions were limited to the cerebrum, which had a 2×2.5×5 cm, well-demarcated, firm, mottled, pale grey to brown-red mass with a granular texture, centered on the left and right cingulate gyri. Microscopically, granulomatous and necrotizing meningoencephalitis with intralesional pi

Difficult mycoses of the skin: advances in the epidemiology and management of eumycetoma, phaeohyphomycosis and chromoblastomycosis.

PURPOSE OF REVIEW: Eumycetoma, phaeohyphomycosis and chromoblastomycosis are subcutaneous mycoses having in common the fact that they are acquired as a result of penetrating trauma to the skin and may be caused by a large variety of fungi. This article will review recent data regarding the epidemiology and treatment of these infections. RECENT FINDINGS: Recent epidemiologic observations in these mycoses include an increased incidence of phaeohyphomycosis in immunosuppressed patients, the association of polymorphisms in genes involved in innate immunity, the occurrence of eumycetoma caused by

Disseminated phaeohyphomycosis due to Ochroconis gallopavum in the setting of advanced HIV infection.

Disseminated phaeohyphomycosis is a rare and typically fatal infection caused by members of the dematiaceous fungi, and occurs almost universally in the setting of immunocompromise. We herein report a case of systemic phaeohyphomycosis caused by Ochroconis gallopavum in a patient with advanced HIV disease. A possible risk factor for this infection in our patient was heavy marijuana use. This case highlights both the diagnostic and management challenges posed by these infections. To our knowledge, this is the first reported case of disseminated phaeohyphomycosis due to Ochroconis gallopavum in
