Pharmacogenomics of cholesterol-lowering therapy.

The prevention of cardiovascular disease is critically dependent on lipid-lowering therapy, including 3-hydroxymethyl-3-methylglutaryl coenzyme A (HMG-CoA) reductase inhibitors (statins), cholesterol absorption inhibitors, bile acid resins, fibrates, and nicotinic acid. Although these drugs are generally well tolerated, severe adverse effects can occur in a minority of patients. Furthermore, a subset of patients does not respond to cholesterol-lowering therapy with a reduction in coronary heart disease progression. Significant progress has been made in the identification of common DNA sequence

Individualized pharmacogenomics for depressive episodes: A narrative review.

Major depressive disorder (MDD) and bipolar disorder (BD) are highly prevalent psychiatric conditions, both characterized by depressive episodes and associated with substantial disability, marked clinical heterogeneity, and variable responses to pharmacological treatment. Despite the widespread use of antidepressants and antipsychotics, a significant proportion of patients experience delayed therapeutic response, adverse drug reactions, or treatment resistance, due to interindividual biological variability. In this context, pharmacogenomics has emerged as a key strategy to address this challen

Pharmacist assessment of drug-gene interactions and drug-induced phenoconversion in major depressive disorder: a case report.

BACKGROUND: Response to antidepressant therapy is highly variable among individuals. Pharmacogenomic (PGx) testing presents an opportunity to guide drug selection while optimizing therapy outcomes and/or decreasing the risk for toxicity. CASE PRESENTATION: A patient with multiple comorbidities, including severe major depressive disorder (MDD), experienced adverse drug events and undesirable response to multiple antidepressant medications (i.e., bupropion, escitalopram, and venlafaxine). A clinical pharmacist assessed significant drug-gene, drug-drug, and drug-drug-gene interactions as well as

The role of pharmacogenomics in the discontinuation of psychotropic medication: a scoping review protocol.

INTRODUCTION: Discontinuation of psychotropic medication is associated with considerable clinical challenges, including withdrawal symptoms and relapse. Pharmacogenomic testing is increasingly used in clinical practice to optimise pharmacotherapy by guiding drug selection and dosing, however, its role in informing medication discontinuation and tapering strategies remains underexplored. Understanding the influence of genetic variability on withdrawal symptoms could support safer, individualised discontinuation practices. OBJECTIVE: This scoping review aims to map the available evidence on the

Potential of pharmacogenetics in minimizing drug therapy problems in cystic fibrosis.

BACKGROUND: With advancements in CF drug development, people with cystic fibrosis (PwCF) now take a median of seven medications daily, increasing treatment complexity, risk of drug therapy problems (DTPs), and interference with treatment goals. Given that some of these DTPs can be prevented with preemptive pharmacogenetic testing, the overall goal of this study was to test the clinical utility of a multi-gene pharmacogenetics (PGx) panel in potentially reducing DTPs in PwCF. METHODS: A population based retrospective study of patients with CF was conducted at the University of Utah Health Care

Perplexed by PGx? Exploring the impact of pharmacogenomic results on medical management, disclosures and patient behavior.

Pharmacogenomic (PGx) tests represent significant advances in precision medicine. Our aim was to explore perceptions following the return of PGx results, medication management, and disclosure to providers. We surveyed clients who had PGx testing and conducted a chart review of PGx results. Respectively, 84% and 94% of participants found pre- and post-test genetic counseling helpful. There was a significant difference in disclosure, while 6% disclosed results to a pharmacist, 50% disclosed to a physician. Qualitative analysis identified three themes: 1) psychological response; 2) perceived util

Changing vision: a review of pharmacogenetic studies for treatment response in age-related macular degeneration patients.

Nonresponsiveness to age-related macular degeneration (AMD) treatments has become a growing concern in ophthalmology. Disparity among publications that have assessed pharmacogenetic (PGx) connections between AMD disease genes and treatments has delayed the implementation of PGx testing in AMD. We assessed all AMD PGx publications to identify the degree of agreement for publications within similar ethnic cohorts and worldwide, and the causes for differences in study outcomes. There are no accepted genotype-phenotype correlations, either within similar ethnic cohorts or worldwide. The diversity

Pharmacogenomic diversity among Arab populations: A systematic review.

Pharmacogenomics enables precision pharmacotherapy by linking genetic variation to drug response, yet Arab populations are underrepresented in global reference datasets. We systematically synthesized pharmacogenomic allele-frequency evidence across Arab countries, focusing on clinically actionable genes, to describe population variation, identify high-priority variants, and highlight research gaps. We analyzed 295 studies including 94,346 individuals from 19 countries, pooled country-level allele counts for frequently tested variants, and compared pooled estimates with Middle Eastern reference

Pharmacogenomics on the Treatment Response in Patients with Psoriasis: An Updated Review.

The efficacy and the safety of psoriasis medications have been proved in trials, but unideal responses and side effects are noted in clinical practice. Genetic predisposition is known to contribute to the pathogenesis of psoriasis. Hence, pharmacogenomics gives the hint of predictive treatment response individually. This review highlights the current pharmacogenetic and pharmacogenomic studies of medical therapy in psoriasis. HLA-Cw*06 status remains the most promising predictive treatment response in certain drugs. Numerous genetic variants (such as ABC transporter, DNMT3b, MTHFR, ANKLE1, IL-

Case Report: Pharmacogenetically-triggered fatal serotonin syndrome following sequential serotonergic therapy in Parkinson's disease: a case highlighting the role of CYP2D6*10/*10 genotype and multifactorial drug interactions.

Serotonin syndrome (SS) is a potentially life-threatening condition resulting from excessive serotonergic activity in the central nervous system. We present a fatal case of SS complicated by multiple organ failure in a 72-year-old patient with Parkinson's disease following the sequential administration of two selective serotonin reuptake inhibitors (sertraline and escitalopram) while on a stable regimen of the monoamine oxidase-B inhibitor rasagiline. Pharmacogenomic testing revealed a homozygous CYP2D6*10/*10 genotype, conferring an intermediate metabolizer phenotype, which is postulated to h

Pharmacotherapy Risks in Rare Genetic Diseases: Cross-Referencing ACMG Secondary Findings v3.2 List With Clinical Databases.

Clinical genomics and pharmacogenomics have largely remained separate fields, though some genetic variants have overlapping disease risk and drug implications. However, the extent of this overlap is not well studied. To explore this gap, we cross-referenced genes from the American College of Medical Genetics Secondary Findings v3.2 list with genomic databases and drug labeling to identify gene-phenotype pairs with overlapping clinical genomics and pharmacogenomic implications. We searched GeneReviews and PharmGKB (now called ClinPGx) for each gene-phenotype pair and reviewed the FDALabel datab
