Considerations for development of pharmacy support models for COVID-19 alternate care sites.

PURPOSE: Guidance on alternate care site planning based on the experience of a health-system pharmacy department in preparing for an expected surge in coronavirus disease 2019 (COVID-19) cases is provided. SUMMARY: In disaster response situations such as the COVID-19 pandemic, healthcare institutions may be compelled to transition to a contingency care model in which staffing and supply levels are no longer consistent with daily practice norms and, while usual patient care practices are maintained, establishment of alternate care sites (eg, a convention center) may be necessitated by high pat

Transforming Veteran Care Through Standardized Clinical Pharmacy Practice.

The Veterans Health Administration (VHA) has been a beacon of what team integration including a clinical pharmacist can do to improve patient care. The VHA is the largest integrated health care system in the United States. Clinical Pharmacist Practitioners (CPP) operate under a scope of practice that allows for initiation, discontinuation, and modification of medication therapies in patients with a confirmed diagnosis as part of providing comprehensive medication management (CMM). Standardization related to the "what" and the "how" CMM is practiced within the VHA have enabled the expansion of

How gaps in regulation of compounding pharmacy set the stage for a multistate fungal meningitis outbreak.

OBJECTIVE: To provide an overview of the regulation issues surrounding compounding pharmacy that allowed the United States fungal meningitis outbreak to occur and the changes in regulation that ensued. SUMMARY: In September 2012, a single case report sparked an investigation into a nationwide outbreak of fungal meningitis due to contaminated injectable drugs. The source of the contamination, New England Compounding Center (NECC), was in violation of several state and federal laws and had a history of such violations. The regulation of compounding pharmacies has historically been left to the s

[Development of hospital pharmacy practice using therapeutic drug monitoring].

Therapeutic drug monitoring (TDM) is a developing field in hospital pharmacy that contributes to good clinical response. Optimal concentrations of immunosuppressants, anti-epileptic drugs, and glycopeptide antibiotics are narrow, and these drugs are widely monitored by fluorescence polarization immunoassay (FPIA) in clinical settings. Recently, a chemiluminescence immunoassay (CLIA) has also become available. We compared these two assays in monitoring serum concentrations of cyclosporin A (CsA), carbamazepine (CBZ), phenobarbital, phenytoin, and valproic acid. FPIA estimates of CsA and CBZ con

Phenelzine as a stimulant drug antagonist: a preliminary report.

Phenelzine administration, monitored via a pharmacy-controlled program, was employed in 38 subjects over a 6-month period to prevent amphetamine-type drug abuse, in much the same manner as disulfiram programs are employed against alcohol abuse. Advantages of the program were apparent, with a majority of subjects abstaining during the enforced phenelzine trial. Subjects generally made use of this abstinent period to benefit from a variety of psychotherapeutic modes, and demonstrated enhanced job and school performance and improved marital relationships. Results based on subject and observer rep

Occupational exposure to antineoplastic agents at several departments in a hospital. Environmental contamination and excretion of cyclophosphamide and ifosfamide in urine of exposed workers.

The occupational exposure to cyclophosphamide (CP), ifosfamide (IF), 5-fluorouracil (5FU), and methotrexate (MTX) of 25 pharmacy technicians and nurses from four departments of a hospital was investigated. Previously developed methods for the detection of exposure to some antineoplastic agents were validated. Exposure to CP, IF, 5FU, and MTX was measured by the analysis of these compounds in the environment (air samples and wipe samples from possible contaminated surfaces and objects). Contamination of the work environment was found not only on the working trays of the hoods and on the floors

Nursing considerations in paclitaxel (Taxol) administration.

The administration of paclitaxel (TAXOL) presents some challenges unique among chemotherapeutic agents. Information published and presented to date has focused primarily on the most threatening and clinically significant aspects of paclitaxel administration and resultant side effects. However, there are a number of other issues that, while not as acutely dangerous, can be very significant to the success of paclitaxel therapy and to those medical, pharmacy, and nursing personnel involved in drug administration and patient management. This review will focus on the collaborative role of the nurse

Medicare Part D: Major Shifts With the Inflation Reduction Act and a Way Forward.

The Inflation Reduction Act (IRA) of 2022 introduced major changes in the Part D benefit that aim to improve medication access and correct several of the financial misalignments in the current Part D benefit. The changes address financial obligations of Medicare beneficiaries, the federal government, Part D plan sponsors (i.e., insurance companies), and drug manufacturers. The changes include new brand and biologic manufacturer obligations to beneficiaries eligible for the low-income subsidy. Effects on the drug supply chain and stakeholder behaviors remain to be seen but current financial arr

A two-domain regulatory literacy instrument among Saudi pharmacy interns: a single-center pilot assessment under vision 2030.

BACKGROUND: Saudi Arabia accelerates the nationalization of the pharmacy workforce under Vision 2030, making regulatory proficiency paramount for ensuring patient safety and institutional compliance. While foundational jurisprudence is integrated into Doctor of Pharmacy (PharmD) curricula, a significant knowledge-to-action gap often persists regarding the procedural execution of statutory mandates. OBJECTIVES: This pilot study aimed to develop a bilingual, two-domain regulatory literacy instrument and to compare declarative (basic) vs. procedural (operational) regulatory literacy of final-yea
