Anastomosis technique for high pharyngogastrostomy.

Pharyngogastric anastomosis, otherwise referred to as the "gastric pull-up" procedure, is the most reliable method of reconstruction after laryngopharyngectomy. We currently use a method of gastric to pharyngeal anastomosis that avoids excess tension, and thus decreases the incidence of fistulas and flap failures. With the stomach fully mobilized, the "pull-up" is brought high into the neck using a plastic bag to facilitate delivery via the posterior mediastinum. A curved, U-shaped incision is then made in the fundus of the tongue anteriorly, allowing the posterior nasopharynx to be reached wi

Gastric pull-up operation for replacement of the cervical portion of the esophagus.

The gastric pull-up operation is a useful, effective method for immediate restoration of alimentary continuity after resection of the larynx, pharynx and esophagus for carcinoma of the hypopharynx or cervical portion of the esophagus or both. It is suitable only for those patients whose general condition is sufficiently satisfactory to tolerate the one stage combined abdominal cervical operation. When successfully used, the results are striking in comparison with those usually obtained from other methods of esophageal reconstruction.

Rare Hypopharyngeal Polyp Presenting Without Dysphagia: A Case Report.

Fibrovascular polyps (FVPs) of the hypopharynx are exceedingly rare benign tumors. This case showcases an incidental finding of a fibrovascular hypopharyngeal polyp discovered during a flexible laryngoscopy secondary to ear pain in a 72-year-old male. The patient reported no associated dysphagia, voice changes, dyspnea, or globus sensation. Imaging studies, including a CT neck scan, MRI, and videofluoroscopic swallow study, revealed a 1.2 × 1.3 × 1.4 cm solid polyp attached to the hypopharynx without extending into the esophagus. The patient underwent a successful endoscopic excision of the ph

[The treatment of carcinoma of the pyriform sinus at Santa Creu I Sant Pau Hospital of Barcelona. The role of surgery].

The paper deals with the retrospective review done by the AA. about the treatments used for squamous carcinomata of the pyriform sinus at Santa Creu i Sant Pau Hospital, Barcelona, between the years 1984-1990. In this term 76 sinus pyriform carcinomata were diagnosed, 12 of which (16%) were managed palliatively. In the other 64 cases radical treatment was advised and followed: 5 cases underwent radical cobalt-therapy; polychemotherapy followed by radiotherapy in 27; surgery plus radiotherapy in 7; and polychemotherapy completed with surgery and radiotherapy in the remaining 25 cases. Considera

[Hypopharynx reconstruction of defects after operation on advanced pyriform sinus cancer with remaining laryngeal mucosa flap and pectoralis major myocutaneous flap].

OBJECTIVE: To explore the outcome of hypopharynx reconstruction by using remaining laryngeal mucosa flap and pectoralis major myocutaneous flap in advanced pyriform sinus cancer. METHODS: Twelve patients with pyriform sinus cancers underwent hemilaryngectomy and partial pharyngectomy, two patients underwent cervical esophagectomy at the same time. The defects were reconstructed by remaining laryngeal mucosa flap. Four cases were involved in the bilateral larynx, received total laryngectomy and were repaired by pectoralis major myocutaneous flap. RESULTS: There was no operative fatal case and

Mixed HPV-related Neuroendocrine Carcinoma and HPV-related Squamous Cell Carcinoma of the Base of Tongue in a Patient With Incidental Identification of Synchronous Metastatic Papillary Thyroid Carcinoma.

BACKGROUND: While it is not uncommon in patients with head and neck cancer to present with multiple metachronous primary neoplasms, rarely do these present as a singular mass composed of intertwined, histologically distinct malignant tumors. Sometimes referred to as collision tumors, these entities are poorly understood and only appear in a handful of case studies in the literature. CASE REPORT: Here we present a 58-year-old male diagnosed with a human papillomavirus-related collision tumor consisting of oropharyngeal squamous cell carcinoma and small-cell neuroendocrine carcinoma, as well as

Survival and functional results of Pearson's near-total laryngectomy for larynx and pyriform sinus carcinoma.

BACKGROUND: Current treatment for most T3 and T4 transglottic and pyriform sinus carcinomas is total laryngectomy or total laryngectomy with partial pharyngectomy. Voice rehabilitation usually requires the use of a tracheoesophageal puncture (TEP). Pearson's near-total laryngectomy (NTL) is an option for voice preservation in selected cases with no invasion of the interarythenoid space and limited invasion of the subglottis. The purpose of this study is to report the functional and survival results of 42 consecutive patients who underwent NTL from 1988 to 1995. Patients and Methods The patient

Principles of radical oesophageal surgery.

The standard procedure for cervical oesophageal carcinoma consists of partial pharyngectomy, laryngectomy with tracheostomy, resection of one thyroid lobe and complete oesophagectomy. For intrathoracic and abdominal carcinoma of the oesophagus, subtotal resection leaving a small cervical portion is sufficient. If the carcinoma has not exceeded the organ boundaries or is located below the tracheal bifurcation, the choice of resection either by transthoracic, transmediastinal or endoscopic methods is not critical with respect to the radical nature of the resection. In carcinoma located at or abo

Treatment outcome in patients undergoing surgery for carcinoma larynx and hypopharynx: a follow-up study.

CONCLUSIONS: Compared with larynx cancers salvage rates are poorer for hypopharyngeal cancers and the role of primary surgery seems to be significantly higher and more decisive in overall survival (OS). Hypopharyngeal cancers therefore warrant a closer follow-up after an organ preservation protocol, to detect recurrence at the earliest stage and plan for an effective salvage surgery. OBJECTIVE: To evaluate the survival and morbidity of patients treated by surgery for carcinoma of the larynx and hypopharynx according to site. PATIENTS AND METHODS: All patients who had undergone laryngectomy a

Two modifications of pectoralis major myocutaneous flap (PMMF).

Pectoralis major myocutaneous flap is the most commonly used versatile flap in head and neck reconstructive surgery. The use of entirely tubed pectoralis major myocutaneous flap for reconstruction of the hypopharynx following total laryngectomy and total pharyngectomy has a disadvantage of bulkiness of the flap and poor postoperative deglutition. One-stage reconstruction of the entire hypopharynx utilizing a combination of pectoralis major myocutaneous flap and dermal graft minimizes bulkiness, thus achieving satisfactory to excellent functional results. The operation has been performed on fou

Clinical outcomes following pharyngolaryngectomy reconstruction: a 20-year single-centre study.

BACKGROUND: Advanced malignant neoplasms of the larynx and hypopharynx pose many therapeutic challenges. Total pharyngolaryngectomy and total laryngectomy provide an opportunity to cure these tumours but are associated with significant morbidity. Reconstruction of the pharyngeal defect following total pharyngolaryngectomy demands careful consideration and remains an area of debate within surgical discussions. METHODS: This paper describes a systemic analysis of pharyngeal reconstruction following total pharyngolaryngectomy and total laryngectomy, leveraging data collected over a 20-year perio

Classification system for lateral pharyngotomy: systematic study of anatomic exposure in a human cadaver model.

INTRODUCTION: Lateral pharyngotomy (LP) is a surgical procedure that allows exposure to tumors of the pharynx and supraglottic larynx. This study was undertaken to: (1) propose a classification system of LP used in exposing various sites of the oropharynx, supraglottis, and hypopharynx. (2) Describe the structures visible with each category of LP. MATERIALS AND METHODS: Five tissue-fixed human cadavers from our gross anatomy laboratory were dissected in a manner similar to surgical lateral pharyngotomy. After exposure of the neurovascular structures of the anterior compartment of the neck and

Free jejunal transfer for a 15-year-old girl with synovial sarcoma of the hypopharynx.

INTRODUCTION: Synovial sarcomas are rare, aggressive malignant soft-tissue tumours. Approximately 85-90% of synovial sarcomas occur in the extremities, with only 3% arising in the head and neck region. The hypopharynx as a site of occurrence has only rarely been reported. We describe herein our experience of free jejunal transfer for an adolescent patient with synovial sarcoma of the hypopharynx. CASE REPORT: A 15-year-old girl presented with a 2-month history of swallowing difficulties and hoarseness. Laryngoscopy revealed a mass filling the hypopharynx, almost occluding the larynx. Hypopharyngeal reconstruction with gastro-omental free flap.

Three types of reconstruction are possible following total laryngopharyngectomy (TLP) for advanced hypopharyngeal cancer: locoregional tubularized island flaps, gastric pull-up and free flaps. Gastro-omental free flap (GOFF) is rarely used in this setting. However, because of its composite nature, this flap has the advantage of being able to restore digestive continuity and reconstruct part of the skin of the neck when it needs to be sacrificed because of tumour invasion or poor trophicity. The GOFF is a reliable and robust flap particularly indicated in hostile environments: repeated neck sur

Quality-of-life and functional outcomes following pharyngolaryngectomy: a systematic review of literature.

BACKGROUND: The long-term prognosis of hypopharyngeal cancer is poor. Surgery necessitates pharyngolaryngectomy with flap reconstruction. For such patients, it is important that functional outcomes are preserved to maintain a respectable quality of life. OBJECTIVE OF REVIEW: To identify the functional outcomes following pharyngolaryngectomy with respect to quality of life, speech and swallow through a systematic review of literature. SEARCH STRATEGY: Searches of EBM databases and literature databases using key words: pharyngolaryngectomy, laryngopharyngectomy, swallow, dysphagia, speech and

Pharyngolaryngectomy using a free jejunal graft: a new anastomosis technique.

We describe a technique of staple anastomosis for pharyngolaryngeal reconstruction using a free jejunal graft. Stapled anastomosis can be applied to both the upper and lower pharyngeal and oesophageal repair. Our series of 37 stapled anastomosis developed no post operative leaks. This technique allows an easier method for inferior anastomosis low down in the upper mediastinum where access is often difficult. Swallowing was established in all patients. Primary puncture into oesophagus below the anastomosis, gives good results with a low stricture rate of 11% all of which resolved with a single

Laryngeal flap for hypopharynx reconstruction.

Hypopharynx repair following resection of a pyriform sinus carcinoma may be accomplished primarily or involve a more extensive procedure, such as a free intestinal graft. Described herein is a laryngeal flap which may be used to repair a hypopharyngeal defect when a strip of mucosa, too narrow for a primary repair, is left. This method has been used successfully in 4 patients and has saved these patients from more extensive repairs.
