Assessing substance abuse treatment need: a statewide hospital emergency department study.

STUDY OBJECTIVE: Health care providers in hospital emergency departments rarely take substance abuse histories or assess associated treatment need. This study compares documentation of psychoactive drug-related diagnoses for adult ED patients in medical records with treatment need assessed through self-report, toxicologic screening, and Diagnostic and Statistical Manual of Mental Disorders, 4th edition (DSM-IV), criteria. METHODS: A statewide, 2-stage, probability sample survey was conducted in 7 Tennessee general hospital EDs from June 1996 to January 1997. Main outcome measures were the pre

Fatal Intoxication Involving 3-MeO-PCP: A Case Report and Validated Method.

We present in this case report a validated method for accurate quantitative analysis of 3-methoxy phencyclidine (3-MeO-PCP) to determine postmortem blood concentrations of this PCP analog. A 29-year-old male with a history of illicit drug use was found unresponsive in his bed with a bag of white powder next to him. Resuscitation efforts were unsuccessful and the individual was pronounced dead 9 minutes after arrival to the hospital. Initial ELISA screening suggested the presence of PCP in the decedent's blood. However, confirmatory testing revealed no detectable PCP. Instead, a large peak corr

Whole blood deproteinization for drug screening using automatic pipettors.

Automating the initial screening of postmortem blood and other tissues for drugs of abuse requires a pipetting procedure that is compatible with immunoassay screening tests. The deproteinization procedure uses a zinc sulfate-5-sulfosalicylic acid reagent to precipitate blood proteins. This procedure for deproteinization of whole blood and tissue homogenates is compatible with coated tube radioimmunoassays (RIA) and can therefore be used in automating the initial screening step.

Ketamine - An Imperfect Wonder Drug?

Ketamine is a potent sedative and dissociative anesthetic agent that has been used clinically for over 50 years since it was first developed in the 1960 s as an alternative to phencyclidine (PCP). When compared to PCP, ketamine exhibited a much lower incidence of severe side effects, including hallucinations, leading to its increased popularity in clinical practice. Ketamine was initially used as an anesthetic agent, especially in emergency medicine and in surgical procedures where rapid induction and recovery was necessary. However, over the last few decades, ketamine was found to have additi

Unregulated self-medication in an ambulatory surgery population.

The objective of this study was to determine whether unregulated self-medication in an ambulatory surgery population was a significant problem in terms of the incidence and nature of drugs detected. We asked 67 consecutive patients who presented for elective ambulatory surgery at a university medical center to volunteer for serum and urine testing with the assurance of confidentiality. All patients denied nonprescription drug use. Initial testing was for amphetamines, barbiturates, benzodiazepines, cocaine, opiates, phencyclidine (PCP), tetrahydrocannabinol (THC) and alcohol. Of the 64 patient

Determination of phencyclidine by radioimmunoassay.

A radioimmunoassay (RIA) was developed for measurement of phencyclidine (PCP) and its monohydroxy metabolites in urine. Anti-PCP serum was generated in rabbits and the labeled antigen was prepared by radio-iodination (125I) by the Hunter-Greenwood procedure. Adjustment of the antiserum and 125I-antigen concentrations resulted in a dynamic response from 0 to 200 ng/ml and as little as 2 ng/ml was detected. Reagents were stable for at least three months at 2 to 8 degrees C. Most (95%) urine specimens from presumed nonusers contained less than 5 ng/ml and urine specimens from suspected abusers us

Memantine misuse and social networks: A content analysis of Internet self-reports.

PURPOSE: Memantine is a N-methyl-D-aspartate (NMDA) receptor antagonist currently used for moderate-to-severe Alzheimer's disease. Although the risk of memantine abuse is very low, other NMDA receptor antagonists, such as phencyclidine and ketamine, are well known to drug users. The purpose of this study was to collect data on social networks in order to identify unexpected forms of memantine abuse. METHOD: A Google Trends search was used to highlight reddit.com as a major source of social discussions about memantine. Self-reported users experiences posted on reddit.com were recorded and sort

Neurotransmitters and apoptosis in the developing brain.

In the immature mammalian brain during a period of rapid growth (brain growth spurt/synaptogenesis period), neuronal apoptosis can be triggered by the transient blockade of glutamate N-methyl-d-aspartate (NMDA) receptors, or the excessive activation of gamma-aminobutyric acid (GABA(A)) receptors. Apoptogenic agents include anesthetics (ketamine, nitrous oxide, isoflurane, propofol, halothane), anticonvulsants (benzodiazepines, barbiturates), and drugs of abuse (phencyclidine, ketamine, ethanol). In humans, the brain growth spurt period starts in the sixth month of pregnancy and extends to the

The effects of adulterating agents on FPIA analysis of urine for drugs of abuse.

A variety of chemical agents were evaluated to determine their effects on fluorescence polarization immunoassays for drugs of abuse. Sixteen different agents, at concentrations up to 10%, were tested against urine assays for cannabinoids, cocaine (metabolite), amphetamines, opiates, phencyclidine (PCP), and barbiturates. The potential to cause both false positive and false negative results was evaluated, and assays were performed one and seven days after sample adulteration to simulate different collection/testing formats. All six drug assays were susceptible to one or more adulterating agents

Localisation of phencyclidine-induced changes in brain energy metabolism.

The abuse of phencyclidine [1(1-phencylohexyl)piperidine, PCP], commonly referred to as angel dust or hog, is rapidly reaching epidemic proportions. PCP users often appear violent and increases in PCP-implicated homicides and suicides have been reported. In animal studies PCP has been demonstrated in brain up to 48 h after administration, long after blood levels become undetectable. However, there is little further information on the distribution of PCP within the central nervous system with regard to the possible sites of action. Recently, Sokoloff and associates described a new technique whi

Simultaneous analysis of 203 drugs of abuse and metabolites in urine samples using liquid chromatography-tandem mass spectrometry.

The issue of drug abuse is increasingly becoming a significant concern worldwide. However, the simultaneous detection of a wide spectrum of drug of abuse (DOA), especially in biofluids, is challenging due to their diverse and varied physicochemical properties and matrix effects. Herein, we have developed a liquid chromatography-tandem mass spectrometry (LC-MS/MS) method for simultaneous detection of 203 DOAs, including amphetamines, opiates, cathinones, phencyclidines, synthetic cannabinoids, cocaine, and metabolites, in 100 μL of urine. A mass analysis was performed using multiple reaction mo

Recreational ketamine-related deaths notified to the National Programme on Substance Abuse Deaths, England, 1997-2019.

BACKGROUND: Ketamine is a phencyclidine derivative with dissociative anaesthetic properties. Increasing numbers of individuals in England take ketamine recreationally. Information on deaths arising from such use in England is presented. METHODS: Cases were extracted on 31 January 2020 from the National Programme on Substance Abuse Deaths database, based on text searches of the cause of death, coroner's verdict and positive toxicology results for the terms 'ketamine' or 'norketamine'. FINDINGS: During 1997-2005, there were <5 deaths p.a. in which ketamine was implicated. Numbers increased unt

Drugs of Abuse in Meconium.

The determination of fetal drug exposure is of great importance for the future development of the neonate. Meconium, the first fecal material excreted by the newborn is an excellent depository for drugs to which the fetus has been exposed. Correct diagnosis of drug use during pregnancy allows the child to receive specialized treatment and care, which will aid in learning behavioral development. Meconium analysis is gaining significant credibility as an alternative or an additional sample to neonatal or maternal urine, because meconium provides a longer history of drug use than urine and drug c

Structural basis of opioid receptor activation by PCP and ketamine.

Ketamine offers rapid relief for treatment-resistant depression and severe pain in the clinic, providing immediate benefits that traditional medications often fail to deliver. While its antagonistic action at the N-methyl-D-aspartate receptor (NMDAR) is a key mechanism, ketamine's dual nature as both a promising treatment and a drug with abuse potential suggests its therapeutic effects extend beyond NMDAR inhibition. Here we provide structural evidence of human opioid receptors bound to ketamine and its parent analog phencyclidine (PCP), supporting that both ligands can directly bind and activ

Method comparison of EMIT 700 and EMIT II with RIA for drug screening.

The EMIT 700 and EMIT II assays for marijuana, cocaine, opiates, barbiturates, and phencyclidine, as performed on a Hitachi 717 Analyzer, were compared with Roche Abuscreen RIA tests for high-volume drug abuse screening. The EMIT II kits offer some advantages over the EMIT 700 kits for testing large work loads. The EMIT II marijuana test in particular exhibits a calibration curve that promises improved ability to separate negative from positive samples due to the increased absorbance rate separation between the negative and cutoff calibrators. Both EMIT formulations are preferable in terms of

"Smoking wet": respiratory failure related to smoking tainted marijuana cigarettes.

Reports have suggested that the use of a dangerously tainted form of marijuana, referred to in the vernacular as "wet" or "fry," has increased. Marijuana cigarettes are dipped into or laced with other substances, typically formaldehyde, phencyclidine, or both. Inhaling smoke from these cigarettes can cause lung injuries. We report the cases of 2 young adults who presented at our hospital with respiratory failure soon after they had smoked "wet" marijuana cigarettes. In both patients, progressive hypoxemic respiratory failure necessitated rescue therapy with extracorporeal membrane oxygenation.

A comparison of alcoholic and nonalcoholic drug abusers.

A group of 329 hospitalized drug abusers were evaluated for a current or past diagnosis of alcoholism. The alcoholic drug-dependent patients (n = 169, 51.4%) were then compared with the nonalcoholic patients (n = 160, 48.6%) with regard to sociodemographic, clinical and family history characteristics. The alcoholics were significantly more likely to receive a DSM-III diagnosis of major depression, with melancholia; other Axis I diagnoses were equally distributed between the two groups. Antisocial personality disorder was also significantly more prevalent among the alcoholic patients. The alcoh

Dextromethorphan abuse masquerading as a recurrent seizure disorder.

Dextromethorphan (DXM) has unique toxicity that may be difficult to diagnose. We present a case of a young woman who presented to our emergency department (ED) initially diagnosed with recurrent seizures. Paramedics brought a 19-year-old woman to the ED. Witnesses noted "shaking," which the patient did not recall. The patient denied fever, antecedent trauma, or neurological complaint. She was recently administered lamotrigine for bipolar disorder. She was a former alcoholic with no history of developing withdrawal. She admitted to marijuana use but denied use of any other illicit substances. H

[Acting out and psychoactive substances: alcohol, drugs, illicit substances].

In humans, some psychotropic agents (alcohol, drugs, illicit substances) have been suggested to play a role in the occurrence of major behavioural disorders, mainly due to the suppression of psychomotor inhibition. Behavioural disinhibition is a physiological mechanism which allows humans to behave appropriately according to a given environmental situation. The behavioural disinhibition induced by either therapeutic dosage or misuse involves the loss of restraint over certain types of social behaviour and may increase the risk of auto or hetero-aggression and acting out. The increased use of p

Phencyclidine blood concentrations in DRE cases.

Phencyclidine (PCP) concentration was measured in blood obtained from 259 individuals over a two-year period subsequent to Drug Recognition Expert (DRE) evaluation by the Maryland State Police. The purpose of this study was to evaluate the accuracy of the DRE in the identification of PCP-related impairment using the presence of PCP in blood to confirm drug use and to test for a correlation between PCP concentrations in blood and impairment as indicated by DRE evaluation. Of the 259 cases evaluated, 124 were identified as positive for PCP based on DRE evaluation, 130 were positive for PCP based
