The influence of glottal and respiratory factors on aerosol emission during phonation.

INTRODUCTION: Speech-driven aerosol generation plays a key role in airborne disease transmission, yet the physiological mechanisms remain poorly understood. Beyond vocal-fold vibration, airflow and glottal configuration may be key determinants. We tested how phonation type affects aerosol generation while accounting for ventilatory output estimated from exhaled CO₂. METHODS: Five healthy female adults (22-43 years) sustained vowels across six phonation types: modal register, glottal fry, falsetto register, forced whisper, loud modal register, and vowels preceded by/h/. Aerosol concentration a

Medieval Interpretation of Phonation Through the Divine Comedy, by Dante Alighieri.

The anatomic and medical knowledge of people throughout history is unexpectedly evident in some of the poems and texts written by intellectuals of the time. This article attempts to understand the conception of laryngology in the Middle Ages by analyzing the Divine Comedy, written by the Italian poet Dante Alighieri (1265-1321) at the beginning of the 14th century. In the text, Dante mentions the throat several times. He recognizes that the larynx has the dual functions of allowing respiration (dead souls recognize that the poet is alive through movement of his throat when breathing) and speec

Control of Fundamental Frequency in Dysphonic Patients During Phonation and Speech.

PURPOSE: The pitch-shift reflex (PSR) is the adaptation of the fundamental frequency during phonation and speech and describes the auditory feedback control. Speakers without voice and speech disorders mostly show a compensation of the pitch change in the auditory feedback and adapt their fundamental frequency to the opposite direction. Dysphonic patients often display problems with the auditory perception and control of their voice during therapy. Our study focuses on the auditory and kinesthetic control mechanisms of patients with muscle tension dysphonia (MTD) and speakers without voice and

Neuromuscular control of fundamental frequency and glottal posture at phonation onset.

The laryngeal neuromuscular mechanisms for modulating glottal posture and fundamental frequency are of interest in understanding normal laryngeal physiology and treating vocal pathology. The intrinsic laryngeal muscles in an in vivo canine model were electrically activated in a graded fashion to investigate their effects on onset frequency, phonation onset pressure, vocal fold strain, and glottal distance at the vocal processes. Muscle activation plots for these laryngeal parameters were evaluated for the interaction of following pairs of muscle activation conditions: (1) cricothyroid (CT) ver

[Reverse phonation: pathological and therapeutic aspects. Study of a clinical case].

If the phenomenon of reverse voice is well known as a transitional phenomenon occurring during normal phonation, it is less known as a pathological entity. In this connection it seemed interesting to get a fix on this type of phonation. We will illustrate our discussion with one of our patients for which the reverse voice was continuously used instead of the usual blown voice. We will present all clinical and functional investigations conducted with this patient. We also focused on the particular psycho-dysfunctional context this patient was struggling with. Instrumental investigations concern

A phonation therapy approach for Mandarin-English bilingual clients with dysarthria.

This study aims to measure the speech intelligibility of Mandarin-English speakers with dysarthria before and after phonation therapy, in order to determine the effectiveness of this approach. A within-group design was used with two case studies which allowed one to measure therapy variables (single word and sentences); language variables (Mandarin and English); and speech production variables (respiration, phonation, articulation, resonance, and prosody). Both participants demonstrated highly significant improvement in Mandarin intelligibility scores after therapy compared with minimal change

The phonation critical condition in rectangular glottis with wide prephonatory gaps.

The effect of the pressure recovery at glottal exit is introduced to modify the one-mass model. Using the modified one-mass model, the phonation critical condition, including phonation threshold pressure and phonation threshold flow, is analyzed by using the small-amplitude oscillation theory. It is found that the phonation threshold pressure is not sensitive to the change of the prephonatory glottal width at a wide glottal gap. This result agrees with previous experimental observations and suggests that the low slope of dependence of phonation threshold pressure on prephonatory gap found by C

Dynamic MRI of larynx and vocal fold vibrations in normal phonation.

Dynamic magnetic resonance imaging (MRI) of the larynx and vocal folds during phonation was used for measuring the vertical laryngeal movements and the glottal angle of the vocal folds opening and closing in dynamic phase. The data used in this analysis were taken on 10 healthy volunteers during maximal inspiration and the prolonged phonation of the vowels [i] (as in key), [a] (as in car), [u] (as in loop), and the consonant [sh] (as in ship). The results of our MRI data have demonstrated the difference of the vocal folds movement in relation to the vowel and consonant sounds, with a large glo

Some physiological correlates to regular and irregular phonation at the end of an utterance.

This article examines respiratory and acoustic events at the ends of utterances that coincide with the end of a breath. All utterances end with a vowel. Between the start of the final fall in signal amplitude and the end of phonation, the alveolar pressure falls by 1.2-2.6 cm H2O, on average, and glottal area increases by a factor of 4.0 (median). The mean glottal area at the end of phonation ranges from 9.6 to 13.5 mm2 across subjects. Physiological correlates are examined for utterances that end with regular and with irregular phonation. Irregular phonation is usually produced with brief int

[The effect of phonational load on parameters of cardiorespiratory system in hypotonic dysphonia].

Cardiointervalometry was used to investigate functional parameters of the cardiorespiratory system during text reading. A total of healthy women aged from 17 to 28 years and 11 women with hypotonic dysphonia 17 to 31 years of age entered the study. Both groups exhibited a low index of parasympathetic component of cardiorhythm regulation in shortening of the RR-interval. Correlation inhalation/expiration has changed with prolongation of inhalation and shortening of expiration in reading in women suffering from hypotonic dysphonia. Parameters of the respiratory cycle at rest did not correlate wi

Evaluation of a stuttering treatment based on reduction of short phonation intervals.

This paper reports the results of an efficacy study of a stuttering treatment program known as Modifying Phonation Intervals (MPI), which trains stuttering speakers to reduce the frequency of relatively short phonation intervals (PIs) during connected speech across speaking tasks and situations. Five young adult male stuttering speakers were treated in this computer-based program that systematically trains speakers to reduce selected short PIs found to functionally control stuttering. The treatment process was evaluated using multiple-baseline designs. Treatment was largely self-managed and ba

Clinical course of acute laryngeal trauma and associated effects on phonation.

We report the clinical course of blunt laryngeal trauma in three young patients. All three patients underwent several phoniatric examinations as well as indirect microlaryngoscopy and microstroboscopy. The follow-up period ranged from three to eight months. In the first case, there was isolated haemorrhage of the left vocal fold; in the second, dislocation of the arytenoid cartilage with formation of an adhesion in the area of the anterior commissure; and, in the third, non-dislocated fracture of the thyroid cartilage with development of haematoma in the right hemilarynx and transient vocal fo

Experimental evidence in the in vivo canine for the collapsible tube model of phonation.

The in vivo canine model of the larynx was used to measure transglottic pressures and airflow during phonation. Conditions of supraglottal resistance were also simulated. Pressure drop-flow curves were compared with data on collapsible tubes. The in vivo in canine model of the larynx demonstrates a number of features similar to oscillation in collapsible tubes.
