Permeability of dura mater: a possible link between cortical spreading depression and migraine pain? A comment.

In the wake of cortical spreading depression (CSD) it has been suggested that noxious substances diffuse through the dura with resulting firing of epidural nerves. In my view this is unlikely because there are good reasons to suggest that there must be a dura-brain barrier. Alternatively collateral branches from the trigeminal nerve to the pia and the dura may signal what is happening with ions and substances on the brain surface during CSD to the epidural space.

[A diffuse metastatic leptomeningeal carcinomatosis from gallbladder cancer; case report].

We encountered a 72-year-old woman with diffuse metastatic leptomeningeal carcinomatosis, who first suffered from occipital pain and died about a month after onset. On postmortem examination, gallbladder cancer (adenocarcinoma) was found to be the primary disease. We focused on its frequency and the metastatic route. On the metastatic route, we obtained the following results: tumor cells infiltrated only the cerebrospinal fluid, but not the areas surrounding the gallbladder cancer (spine or spinal cord) or into the brain parenchyma. A comparative study of the state of cerebrospinal fluid betwe

Interhemispheric axonal sprouting occurs after pial removal in mice.

White matter lacks the kind of plasticity that is present in the cortex, and subcortical injuries often result in permanent neurological deficits. Because cortical regions share common subcortical nuclei, creating new intergyral connections may allow for the bypass of subcortical damage. In this manuscript, a surgical interhemispheric bridge is created in mice, providing a model for an intercortical transpial bypass. To model this bypass, a midline craniotomy followed by interhemispheric (IH) pial removal was performed in C57BL/6 mice, allowing for the juxtaposition of the right and left prefr

Differential Hemodynamic Response of Pial Arterioles Contributes to a Quadriphasic Cerebral Autoregulation Physiology.

Background Cerebrovascular autoregulation (CA) regulates cerebral vascular tone to maintain near-constant cerebral blood flow during fluctuations in cerebral perfusion pressure (CPP). Preclinical and clinical research has challenged the classic triphasic pressure-flow relationship, leaving the normal pressure-flow relationship unclear. Methods and Results We used in vivo imaging of the hemodynamic response in pial arterioles to study CA in a porcine closed cranial window model during nonpharmacological blood pressure manipulation. Red blood cell flux was determined in 52 pial arterioles during

Leptomeningeal Carcinomatosis: A Rare Presentation of Perforated Gastric Cancer.

Leptomeningeal carcinomatosis (LMC) or leptomeningeal metastasis is defined as metastasis to the pia mater, arachnoid, and subarachnoid space. Only very few patients with cancer have LMC. In the practice of general surgeons, this diagnosis is rarely, if ever, encountered. We present a rare case of a patient presenting to ED with worsening headaches over several months that developed acute-onset abdominal pain while being evaluated. Further workup showed free air, and the patient was taken emergently to the OR, where a perforated gastric ulcer was identified and biopsied. Pathology revealed gas

Establishment of a nude mice model of human monocytic leukemia with CNS and multiorgan extramedullary infiltration.

OBJECT: To establish a human monocytic leukemia model with central nervous system infiltration in BALB/c nude mice. METHODS: BALB/c nu/nu mice, pretreated by splenectomy, cytoxan intraperitoneal injection, and sublethal irradiation, were transplanted intravenously with human monocytic leukemic cell line SHI-1. The leukemic cells engrafted in the mice were traced by RT-PCR, histopathological examination, immunohistochemistry and FCM. RESULT: After engraftment of SHI-1 cells in SCI-nu/nu mice, multiple organs were involved and green solid neoplasms were formed in some organs. Paralysis was dev

The trigeminal system: The meningovascular complex- A review.

Supratentorial sensory perception, including pain, is subserved by the trigeminal nerve, in particular, by the branches of its ophthalmic division, which provide an extensive innervation of the dura mater and of the major brain blood vessels. In addition, contrary to previous assumptions, studies on awake patients during surgery have demonstrated that the mechanical stimulation of the pia mater and small cerebral vessels can also produce pain. The trigeminovascular system, located at the interface between the nervous and vascular systems, is therefore perfectly positioned to detect sensory inp

Supratentorial convexity schwannoma unrelated to cranial nerves: Case report and review of the literature.

BACKGROUND: Intracranial schwannoma not related to cranial nerves is rare entity, and difficult to be diagnosed preoperatively. Here, we experienced a case of convexity schwannoma mimicking convexity meningioma, and discuss about the characteristics of such cases based on the past published reports. CASE DESCRIPTION: A 48-year-old man presented with a sudden onset of seizures. Brain magnetic resonance image (MRI) revealed a small mass lesion in the peripheral aspect of the right parieto-frontal lobe. The mass was isointense on T1-weighted and hyperintense on T2-weighted MRI, with homogenous e

Establishment of NOD/SCID mouse model of central nervous system leukemia.

In the present study, we successfully established a NOD/SCID mouse model of central nervous system leukemia by injection of acute monocytic leukemia cell line SHI-1 cells into the lateral ventricle. Immunohistochemistry was used to detect human leukocyte common antigen in brain slices. Nested PCR assay was used to detect MLL/AF6 fusion gene expression. After injection, the condition of the mice gradually progressed to cachexia and death (median survival time, 25 days). Leukemic cells were identified in the lung, bone marrow, and lymph node of one mouse. Brain tissue sections showed invasion in

[A case of IgG4-related disease exclusively affecting pia matter and cerebral parenchyma].

A 61-year-old man presented with slowly progressive cognitive decline. Brain MRI revealed left frontal lobe lesions with gadolinium enhancement along pia mater. Brain biopsy was performed and histopathological findings was compatible with the diagnosis of IgG4-related disease (IgG4-RD). Serum IgG4 level was within a normal range, and no other systemic organs were suggested to be involved by clinical symptoms or laboratory findings. Intravenous methylprednisolone therapy followed by oral prednisone induction markedly improved the cognitive functions and MRI findings detected at the initial diag

[A case of possible elderly onset rheumatoid meningitis without arthritis].

A 93-year-old man was admitted to our hospital with disturbance of consciousness. Brain magnetic resonance imaging (MRI) showed hyperintensity of the subarachnoid space in the left frontal and parietal lobes on diffusion weighted imaging (DWI) and fluid attenuated inversion recovery (FLAIR). Gadolinium-enhancement of the pia mater was also observed. We did not perform biopsy because of a high risk of perioperative complication. Although physical examination found no evidence of the rheumatoid arthritis, rheumatoid factors and anti-cyclic citrullinated peptides antibodies were elevated. He was

Anatomical localization of beta 1 integrin-like immunoreactivity in rat brain.

beta 1 Integrin-like immunoreactivity was localized in rat brain using a polyclonal antibody raised in rabbit against rat liver beta 1 integrin. One-dimensional immunoblotting of whole rat brain membranes indicated that this antiserum recognized a single molecular species at 116,000 M(r), indicative of rat beta 1 integrins. Specific staining of beta 1 integrin-like immunoreactivity was found in the vascular structures of the brain, including microvessels, the ventricular ependymal cells, and pia mater. The pineal gland stained densely, and diffuse staining was present throughout the gray matte

Differentiating leptomeningeal diseases through enhancement patterns: A pictorial essay.

Leptomeningeal diseases represent a diverse group of pathological conditions affecting the pia and arachnoid mater, encompassing a wide spectrum of infective-inflammatory, neoplastic, and vascular etiologies. Early and accurate diagnosis is crucial due to the often severe clinical implications and overlapping imaging features. This review provides a comprehensive overview of commonly encountered and some rare leptomeningeal diseases, emphasizing their pathophysiology, clinical manifestations, and distinctive imaging characteristics. We categorize these conditions based on their underlying etio

Delayed isolated tension intraventricular pneumocephalus after ventriculoperitoneal shunt.

OBJECTIVE AND IMPORTANCE: Delayed and isolated tension intraventricular pneumocephalus without air in the pericerebral subarachnoid spaces is an exceptional complication of a ventriculoperitoneal (VP) shunt. We purpose an explanation for the development of such delayed pneumocephalus. CLINICAL PRESENTATION: A 14-year-old boy developed headaches and right-side hearing loss three months after surgical resection of a cerebellar medulloblastoma and insertion of a VP shunt. CT-scan showed isolated intraventricular pneumocephalus without air in the pericerebral subarachnoid spaces. A petrous bone d

Malignant choroid plexus papilloma of the IV ventricle.

We report a case of malignant choroid plexus papilloma (MCPP) of the IV ventricle in a child with prominent extraventricular expansive growth, mostly into the cerebellopontine angle. Interestingly enough, the tumor was entirely covered by thin, smooth, membranous, fibrous tissue, probably derived from the pia mater. In addition, hydrocephalus was not observed. The reason for the lack of hydrocephalus remains speculative, but it is possible that the presence of pure, nonexpansive, fibrous covering on the tumor might have suppressed the tumor growth to some extent, resulting in progressively inc

Optic nerve-associated connective tissue structures revisited: A histological study using human fetuses and adult cadavers.

Unlike the usual peripheral nerve, the optic nerve accompanies a thick "dural sheath," a thin "sheath of pia mater" (SPM), and multiple "septa," which divides the nerve fibers into fascicles. We collected specimens from 25 adult cadavers and 15 fetuses and revisited the histological architecture of the optic and oculomotor nerves. In the optic chiasma, the meningeal layer of the dura joins the pia to form a thick SPM, and the periosteum of the sphenoid is continuous with the dural sheath at the orbital exit of the bony optic canal. The septa appeared as a cluster of irregularly arrayed fibrous
