We studied 24 patients with Ehlers-Danlos syndrome with particular attention to the thumb and the trapeziometacarpal joint. The 11 males and 13 females had a mean age of 15.9 years. Symptomatic complaints included pinch, grip and twisting weakness; measurements showed dominant hand strength deficits of 71% for tip pinch, 55% for palmar pinch, 46% for key pinch, and 70% for grip. All finger joint ranges of motion were increased. Sixty-six percent of all the patients had trapeziometacarpal subluxation; 29% had dislocation. Sixteen percent had radiographic evidence of arthritis. Early occupationa

OBJECTIVE: This study investigated the relation between self-assessment of upper extremity function and locomotive syndrome in a general population.
METHODS: Using the 25-question Geriatric Locomotive Function (GLFS-25) test, 320 Japanese people (115 men, 205 women, mean age 67.6 years, 40-92 years) were evaluated for locomotive dysfunction. All had completed a self-administered questionnaire including items for sex, weight, height, dominant hand, and the degree of frequency of hand in ADL. We measured the bilateral hand grip and key pinch strength as indicators of hand muscle function. Study

INTRODUCTION: There is no consensus on the best salvage option after a failed trapeziometacarpal prothesis. Conserving the trapezium and inserting a pyrocarbon hemiarthroplasty implant will prevent thumb shortening and preserve the thumb's motion. The aim of this study was to evaluate the clinical and radiological outcomes in the medium term of trapeziometacarpal prothesis revisions done using the CMI pyrocarbon implant.
PATIENTS AND METHODS: This was a retrospective study of 28 patients with a mean age of 63.5 years. The mean time to revision was 6.1 years after the initial arthroplasty. The

Patients with median nerve compression at the carpal tunnel often have poor sensory afferents. Without adequate sensory modulation control, these patients frequently exhibit clumsy performance and excessive force output in the affected hand. We analyzed precision grip function after the sensory recovery of patients with carpal tunnel syndrome (CTS) who underwent carpal tunnel release (CTR). Thirteen CTS patients were evaluated using a custom-designed pinch device and conventional sensory tools before and after CTR to measure sensibility, maximum pinch strength, and anticipated pinch force adju

BACKGROUND: Compensatory thumb metacarpophalangeal joint hyperextension may occur with basilar joint arthritis and subluxation. Instability at the metacarpophalangeal joint must be corrected when the basilar joint is operated on. The authors use a simple surgical procedure that provides correction of thumb metacarpophalangeal joint hyperextension by a free palmaris longus graft to provide volar tenodesis across the metacarpophalangeal joint.
METHODS: Twelve patients (14 extremities) were evaluated at a minimum of 11 months postoperatively. Outcome analysis was performed using the Disability o

BACKGROUND: First carpometacarpal osteoarthritis (CMC1-OA) is a common degenerative condition causing pain, functional loss, and reduced grip strength. While trapeziectomy remains the standard surgical treatment, reconstructive procedures and implant arthroplasties are increasingly used. However, differences in postoperative recovery between these approaches remain unclear. This study provides an indirect pooled comparison of reconstructive, non-reconstructive, and dual-mobility implant procedures for CMC1-OA.
METHODS: Following Preferred Reporting Items for Systematic Reviews and Meta-Analys

This paper describes the development of a set of multiple linear regression equations for predicting maximal voluntary static handgrip strength of males and females, 5-89 years of age, in each hand. Equations are presented for predicting handgrip strength of the left and right hands from: (i) overall body descriptive variables alone--age, sex, body weight and stature, (ii) overall descriptive variables plus pinch strength variables, and (iii) pinch strength variables alone. A data set on 182 persons living in the United States was used for this study. Two-thirds of the observations were used t

PURPOSE: Thumb carpometacarpal (CMC) joint arthritis often presents with metacarpophalangeal (MCP) joint hyperextension. Although CMC joint arthrodesis and trapeziectomy are established treatments, their effects on MCP joint hyperextension and functional outcomes stratified by MCP joint status remain unclear. We assessed correction of MCP joint hyperextension and functional improvement according to preoperative MCP joint status after CMC joint arthrodesis or trapeziectomy and compared outcomes between procedures in patients with preoperative MCP joint hyperextension.
METHODS: Patients with CM

BACKGROUND: In more advanced stages of trapeziometacarpal arthritis, the thumb metacarpal may develop an adduction contracture, leading to secondary hyperextension instability of the metacarpophalangeal joint. Following trapeziometacarpal arthroplasty, uncorrected substantial metacarpophalangeal hyperextension may contribute to ongoing Z-collapse deformity, weakness of pinch and grip, and patient perception of diminished function.
METHODS: We prospectively analyzed a consecutive case series of 55 trapeziometacarpal arthroplasty patients (mean age, 60 years; 7 males) who received simultaneous

BACKGROUND: Restoration of thumb abduction is essential for functional pinch and grasp in individuals with thenar atrophy. The abductor digiti minimi (Huber) transfer for reconstruction of thumb abduction is rarely used in adults. This study evaluates postoperative functional outcomes in a series of 4 adult patients undergoing Huber abductorplasty and contextualizes the results using published benchmarks.
METHODS: Four adults (aged 20-39 y; 2 men, 2 women) with clinically diagnosed thenar atrophy underwent Huber abductorplasty. Etiologies included iatrogenic recurrent motor branch injury, tra

This study aims to investigate the effects of nerve gliding exercise following carpal tunnel release surgery (NGE-CTRS) and the probing factors affecting the effect of NGE-CTRS on hand function. A total of 86 patients after CTRS participated. Grip strength (grip-s), pinch strength (pinch-s), Semmes-Weinstein monofilament test (SWMT), two-point discrimination (2PD), numbness, pain, and Phalen test (Phalen) were measured and compared between pre- and post-NGE-CTRS. The results showed that the combination of surgery and NGE significantly improved the postoperative grip-s, pinch-s, SWMT, 2PD, numb
